I’ve enjoyed your videos from gear review to medicine for a while and this one was no exception. Very detailed and thought out. Although I know it’s nonsense I still have a hard time using Lidocaine with Epi when it comes to distal circulation. I actually don’t have any issues using plain lidocaine as it usually gives me more then ample enough time but that’s just my experience. Now if you wanted some different suggestions, try placing the pickup parallel with the wound on the surface of the intact skin. I find that way I can go in and come out in one motion usually and know how big of a bite I’ll be taking on both sides of the wound. One other advantage is it anchors the skin down so that the skin doesn’t come upward as much before the needle pierces through the skin from underneath. I found the same idea worked with an open needle-driver when I had to remove the TASER barb from a patient’s face. The open sides stabilized the external skin while I pulled the barb out with the help of the hemostat clamped on to the barb for better purchase / control. Thanks again for the great channel. Wow, I'm going to try and avoid getting shot in the face with a taser! Great tip though on stabilizing the skin! How is NYC? I talked to a recruiter about coming out there last month, but by the time I got there I would have been cancelled because it seems like things were calming down.

@@SnareMan Well to be fair to NYPD I imagine he scared them as the pt was 6' 6" & had cocaine onboard. Strangely he was not under arrest & I felt to me he was nothing but a gentleman. NYC has calmed down and I'm starting to see non-COVID pt's now. Our Governor is going slow re-opening the State. Once he does we suspect we will get a spike in volume as well as come autumn. The most annoying thing is working for 12+ hours with an N95 as well as all the donning & doffing of PPE. During the height of the surge we had PA's / NP's from other dept's in the ED but I never saw anyone from other hospitals let alone states (other then traveler nurses).

I would appreciate your opinion about carrying a Surgical Stapler when you are far from clinics and hospitals, for example when you are camping, hiking, visiting nature in a remote location. And if you think it is a good idea, what training is needed? And what specific models could you recommend? Thank you.

It doesn't seem unreasonable as staplers require almost no training. Lightly reapproximate the tissue, apply light pressure and then put in the staple. Not sure on the model as I haven't really shopped for them, but I would get one with larger width staples in case the wound is larger.

I'm not sure of the official legality of it or if you necessarily need to be officially certified. I guess maybe it would depend on the laws where you live and the situation you are in.
