Agreed. After reading, I think the physio missing is that the placenta secretes human placental lactogen when inhibits insulin production --> insulin insufficiency --> gDM. Other hormones also do this to a lesser extent too (think estrogen and cortisol) which I am assuming is why obese women might be more at risk for gDM. For the actual worksheet, I think we aren't given her BMI to know how at risk she might be for gDM/DM. If she were high risk then she could be screened at this visit. Otherwise, all pregnant women are screened at 24-28 weeks for gDM.

Hey there is a minor mistake at 1:25. hCG has thyrotropin like activity but it is the alpha subunit not the beta (which is exactly why we measure the beta subunit to know hCG level). The beta hCG level should correspond with T4 levels increasing (since youre indirectly measuring the alpha subunit as well), but it the video makes it seem like it is the beta subunit itself.

Video speed controller is a nice add-on for firefox and chrome that lets you speed up AND slow down so you don't have to worry about this. I know your comment is a year old but hopefully this helps out others.

The up arrow at 4:37 should say "CO2 elimination", not production. In response to low CO2 in the blood, free hydrogen ions and HCO3- form CO2 and H2O (catalyzed by carbonic anhydrase), which depletes the concentration of circulating [H+] = increase in pH = alkalosis.

you said in one slide "↑ FREE T4 bc HCG has thyrotropin like activity" and then the next slide, you described that the "total free T4 doesn't change due to ↑estrogen → ↑THBG...." ...maybe in the first part you MEANT to say "↑of T4 in general"???

Because FreeT4 and Total T4 are different things. FreeT4 is circulating in the blood unbound, while some T4 is bound to a Thyroid binding globulin. Total T4 would be both the free unbound T4 and the bound T4.
