I am officially a CNA 06/29/2024!!! Thanks to your videos, I was able to visualize everything you did during my skills. I passed both my test and skills the first try! It’s been a journey, and I’m excited to continue my nursing career. Protip: Study everyday, even if it’s just for a little bit. And be confident!

Hi, just wanted to thank you for your videos I took my first prometric CNA test around 7 months ago fresh out of my class and failed, I felt super discouraged and I thought I just wasn’t meant to be a Nurses Assistant, however, something told me to try again and with the help of reviewing your videos I passed my test yesterday! To anyone that has failed the first time: try again practice and watch all the videos it helped me tremendously, don’t give up! Thank you for the awesome videos they have helped me and so many others. (:

I am not studying to be a CNA but I am caring for my mom who currently has issues standing or walking and your videos are very helpful. We have a gait belt but we're trying to be put it around her and myself. Will give your method a try.

I just want to say thank you for these videos!! I challenged the test and watched these videos a thousand times lol. I also studied with a can test prep book, took several free practice tests, and took the Prometric practices tests online. After everything I did I passed both parts of my test today and I am over the moon happy. These videos were not only informative but they were a huge confidence booster (coming from a 30 something year old who has been far removed from school for over 15 years). Next up is nursing school!

Depending on the State...You're tested on 5 skills. The first one is usually Hand washing And any other four. I heard NY test you in the 24 or 25 skills. Brutal

People should read the description on the videos and realize that every state is different on their requirements...These videos are based on FLORIDA state testing requirements. The videos are REQUIRED to be studied before we take our skills check-offs for the course I take and cover everything our state testing criteria lists on the website for the CNA certification. IF YOU LIVE SOMEWHERE OTHER THAN FLORIDA, MAYBE YOU SHOULD BE LOOKING AT YOUR STATES REQUIREMENTS INSTEAD OF VIDEOS ON FLORIDA STATE REQUIREMENTS.

Just want to thank you for taking the time to post these. I went almost 5 months from the time I took the class till the time that I tested. Thanks to your videos I was able to pass. The funny thing is I heard everybody around me practicing and they were all calling their partners Mrs. Jones 😂😂😂. The girl I tested with even asked me "Do you mind if I call you Misses Jones?".

I have one person, who worked for the Florida State examiners, tell me I have to stand them up and ask if the back of their legs are touching the wheel chair? What is correct so I don't fail this part of the test. Patty doesn't ask but it does say you have to make sure the back of patient legs are touching the wheelchair.

My professor told us to keep the gait belt at arms reach and put shoes on before sitting up the patient just in case the patient cannot sit up on their own (so they don't fall forward on top of us as we put their shoes on)

In the beginning of your skills you don’t want to close the curtain before you wash your hands due to contamination. You want to knock first then introduce yourself, wash your hands then come back into the room and close the curtain 😊 a lease that’s what it says in my book for school .

He's giving her too much assistance for this to serve as a useful demonstration of how to "transfer" someone who's truly physically incapacitated. My 85 y.o. mom is paralyzed on the left side of her body (post stroke) & I'm trying to prepare myself for the worst case scenario-- in the event medicare ends coverage and leaves her with no recourse but to return home-- w me as her primary care giver.

She would most likely need a Hoyer lift. They don't use gait belts at any nursing home I've worked for.

You can certainly do that if you wish, but it is not required for testing. Our videos reflect only required testing standards. Since not all beds in all settings are adjustable (think home care, assisted living. low income convalescent centers, adult day care, etc.) our videos are demonstrating the proper technique to use if beds are not adjustable. The beds in the testing center will be adjustable, so you can elevate the head of the bed if you wish! Thank you for your comment and best of luck!

Ma’am kindly tell me is it allowed for a patient to hold your shoulder before pivoting him on the wheelchair or they should hold the bed for support?.....

They should place their hands on your shoulders (not hold, just rest them there) during the transfer to move them as a unit. Thank you for your question.

I think we need to place the wheelchair near the bed first then we assist the resident to a sitting position using the video instructions, and by raising the HOB fully , right ?

Putting the wheelchair closer to the bed will interfere with swinging the patient's legs around (they will hit the wheelchair and may cause injury). Position the wheelchair within reach (but not too close to the bed), swing the patient around to a sitting position and then put their shoes on and apply the gait belt. You need to have clear access to the patient for all of these activities. Then you can move the wheelchair into position and lock it prior to the transfer. You can raise the HOB if you wish, but this is not a required step for the exam and many patients in many settings may not have adjustable beds (think home care, assisted living, low-income convalescent centers) - you should learn how to accomplish this pivot action with the head of the bed flat as well. The care plan for this patient does not indicate that they cannot use their hands. It only says that the patient cannot take steps and must be placed using a pivot transfer and gait belt. Remember that every patient is different and will have different needs. Always go by the care plan for patient-specific instruction. The care plan for this patient does not indicate that this would be necessary. I'm actually not sure what type of patient that would be necessary for, to be honest. This skill is demonstrated according to testing standards and reflects the care plan instructions used in testing. Hope this helps!

So in this video the wheelchair seems to already be close to the patients bedside will it be this close in reach during the exam ? And if not what should I do because I know walking away too far while patient is already at the edge of the bed isn't the brightest thing to do.

For the exam, the wheelchair will be in the testing room. Bring it close to the bed (but not too close where the patient's feet will get tangled up as you pivot them to a sitting position) before you help the patient sit up. After they are sitting, you have put their shoes on and fastened the gait belt, reach over and bring the wheelchair closer, at a slight angle to the bed, and lock the wheels prior to the transfer. Basically, the wheelchair should be close enough to reach, but not so close that it interferes with the pivot to sitting position. Hope this helps!

You are never required to raise the bed to a comfortable working position (it is not a graded checkpoint). If you did want to raise the bed, it would be right before you put on your gloves. Don't forget to lower the bed at the end of the skill (automatic failure) - the patient expects the floor to be right where they left it!

I love how easily she transfers him to the wheelchair like he's 5 lbs... My mom is heavyset and I'm dying or I'm killing her EVERY TIME I try to move her to her wheelchair.

If i understand correctly, the only skills I MUST use gloves for is: Bedpan, Urine/output, catheter care, and perineal care?

You need gloves if any of the following questions are "Yes": 1) will you be exposed to body fluids 2) will you touch personal skin (breasts, genitals) 3) will you touch non-intact skin (wounds, sores, scratches, incisions). If the answer is "No" to all questions, you do not need gloves. If the answer is "Maybe", wear them. All of the skills you indicated (bedpan, measuring urine output, catheter care and peri care) will all have exposure to body fluids, so gloves are indicated. But you would also have exposure during mouth care and denture care (so gloves are required) and when performing a partial bedbath you are touching personal skin (breasts), so gloves should be worn for that skill as well. Hi First thank you for the videos. So for the state exam with this skills I am confused on the pivot and the legs touching the back of the chair. The state sheet states, "Transfer resident as a pivot completed without having resident take step(s) to reach the wheelchair, and before having resident to sit in the wheelchair, positions with the back of resident's legs against the seat of the wheelchair." Maybe it's the wording, but does this mean I can have them scoot back to have there legs touch the wheel chair. before I sit them down. The sheet also says, we have to slowly sit them down. So i am just a little confused and its not just one swift motion like you do in the video. PLEASE Help

No, the gait belt buckle should be in the front of the patient (like a regular belt). There is no advantage to placing the buckle toward the back and may actually cause the patient to feel odd having a medical device placed on them that they cannot undo themselves as well as the inconvenience of having the patient lean forward to secure and release the gait belt for no apparent reason. Traditionally, gait belts are applied with the buckle facing forward. Hope this helps!

I don't think that one should walk away from the patient to go and get the gait belt. That patient may fall forward, wouldn't it be better to have the gait belt ready to go when the patient is sitting up? And then she walked away again to go and get the wheelchair. That patient could have fallen then as well.

The care plan states that this resident is able to support their own weight but cannot take steps. This person is able to sit. The gait belt was within 2 steps of the patient, as was the wheelchair. You cannot place the wheelchair right next to the bed during the pivot to sitting as it will be in the way of the patient's legs. This method is appropriate for this patient, with his level of function, according to the care plan for this patient. This is the same care plan and level of function that you will experience for the test. Every patient is different and may require different accommodations based on their level of function. Always follow the care plan for the patient that you are working on. Best of luck!

First thank you for making this videos, question in my papers I have that to put two paper towels in the bed under pt foot and then I put pt shoes? is that correct?

The test does not specify when shoes should be put on; only that shoes have been put on before transferring the patient into the wheelchair. You may put them on while the patient is in bed if you wish (using the paper towels you described) but I find that method will deposit dirt onto the sheets and is unsanitary. Therefore, in my videos I demonstrate putting shoes on after the patient has been pivoted into the bed. Either method is acceptable for the exam. Thank you for your comment!

Hi, are the rights of the resident & roles/responsibilities of a CNA are the same regardless of state, or are there significant differences across state lines? I am a CNA in the state of Kansas, and I wondered how different Florida was to Kansas. My course books are not just for Kansas, so I am assuming that that is a "no."

Thank you for your question! Resident rights are universal. The role of the CNA does not change (they are simply a nurse's helper - there to do certain physical tasks that the nurse would do if they had time). However, the actual RESPONSIBILITIES of the CNA may change from state to state. For instance, some states limit what documentation the CNA can put into the patient's legal medical record. The training you receive in each state should review the specific tasks that you can (and cannot) perform as a CNA in that state. Hope this helps!

Hello, thank you for your videos! I'm interested in becoming a CNA, but skills such as this are making me concerned I won't be able to do it. I'm only 28 but I've had a double knee surgery and an ACL reconstruction and I never regained complete strength in my left leg and I have trouble kneeling to the floor as you did in the foot cleaning video. Do you think this would effect my chances at this kind of career? What steps would I take for a patient who is just complete dead weight? Would I also be able to keep a small stool with me for cases such as kneeling to the floor?

I have taken the CNA classes for almost three month and I completed the classes last month it was 90 hours and I pass all my classes exams but then I decided to not move forward in taking the Skills Test because I feel like the CNA career it not really a good fit for me, I want to do more in the Medical field so I'm going to contiune and I'm going take the Patients Care program much better field, I don't want to just clean up patients or feeding them so I'm taking the next step in the Medical Field...

That pivot board and other belt seemed so much more effective, I think my mom would have screamed out in pain with her poor scoliosis crooked back !

For the state exam (which is what this is based on), there is no pivot board. This transfer must be performed using a gait belt and pivot transfer technique (as per the care plan in the beginning of the video). Because every patient is different, and will require different accommodations, there is no "one-size fits all" way of doing things. That is where the care plan comes in. Since every patient is different, the nurse is responsible for assessing the patient and determining the best way of caring for that patient and creating their individualized care plan. CNAs must always follow the care plan. You, as a CNA, are never allowed to alter the care plan in any way because you are not able to assess the potential consequences to the patient. Since the care plan for this skill stated that it must be done with a gait belt and pivot technique, that is what we will do. If we are unable to do that, we would inform the nurse. If the patient had pain, we would inform the nurse. If we thought that the patient was unable to tolerate this technique, we would inform the nurse. But we can never just change the task ourselves. This video reflects the scenario for the exam correctly and demonstrates the proper way to perform this particular style of transfer. Thanks for your comment!

Prometric does not count this as a graded checkpoint. It is a good practice, but not graded on Prometric. Pearson-Vue has slightly different checkpoints. Always go by the Clinical Skills Checklist for your state. Best of luck!

The care plan for this skill (which is unique to every patient based on their specific needs and abilities) states that the patient can stand and bear weight but cannot take steps. Therefore, the care plan instructs us to use a pivot transfer technique as illustrated in the video. If the patient has a different level of ability, it would be reflected in the care plan and different techniques would be ordered. Remember that every patient is different, that is why we have care plans to provide instructions for that specific patient. Thanks!

You CANT pass the gait belt over their heads at ARC in MA it's a complete fail. It has to be around them

The way you put the gait belt on a patient is not a graded checkpoint through Prometric. Reaching around their waist is fine, sliding it over their head is also acceptable, as long as you have control of the ends of the gait belt to prevent injury. You cannot PULL the gait belt along the skin when removing the gait belt, because this can result in friction burns and injury. Hope this helps!
