Thank you for sharing this video. I was a little freaked out by double incision where the just remove the entire nipple itself and was looking for different methods where they don't do that cause I've heard a lot of ppl say that they feel NOTHING in their nipples and i definitely do not want that. I know sensation is not guaranteed here but for me, it's a better try and choice than double incison.

some surgeons hide the scars under the armpits my issue with most surgeries is the fact the scaring in most is obvious and does not give the image of a Male chest. I hope some day FTM surgery will get to MTF level so we arent just getting a masectomy with nipple grafts.

Really appreciate this video! It helped me see that I really probably want to ensure I have a surgeon who at least does the Inverted T to help weigh out what I want; but so far all pros with minimal cons! I'm fat, being completely flat would be weird.

Weird question, If the patient had a 48 lbs. ovarian cyst removed and has a scar from the lower rib cage down to the pelvic region how would that effect the surgery?

hey idk if you ever got an answer for this but a friend of mine got top surgery and had a scar on his ribs. the scar didn't move and didn't impact the surgery. of course you can discuss this with a surgeon when it comes time, situations vary etc. but I thought some input might be appreciated!

Thank you for this excellent and informative video, it really helps with visualizing the different techniques. Do all surgeons using the inverted t scar pattern for radical reductions or top surgery use the inferior pedicle or do some use a different pedicle?

That is a great question, there are other pedicles that can be used. The other most commonly used pedicle is the superomedial pedicle, where the blood supply comes from the inner, top of the chest. This pedicle is chosen for individuals that are interested in having more fullness above the nipple, rather than below it as is the case in the inferior pedicle. There are other scar patterns in addition to the inverted T that are used in radical reduction; however the inverted T allows for the most skin and tissue reduction, so it is most commonly used in radical reductions, when the patient wants as much tissue removed as possible.

Though there are definitely anecdotal cases of people who can chestfeed after some types of top surgery, you should definitely not count on this, and for sure it will not be possible after procedures other than aggressive breast reduction and possibly buttonhole / inverted T. hope im understanding it right, so an inverted T procedure would not allow for the development of a muscular chest? i know the chest won’t be as flat as with other surgeries, but if someone worked out their chest would it end up looking like pecs or would it be more like fat? not sure if that makes sense, hope it does! ^^

Inverted-T is typically not the recommended procedure for patients who want to build a muscular-looking chest, but it largely depends on the amount of chest tissue that is present before surgery. Great video! Would someone that went through a breast reduction prior to top surgery be a candidate for these procedures as opposed to double incision?

Most commonly, patients are not candidates for buttonhole or inverted-T when they have had a breast reduction procedure previously. The reason is that the first reduction alters the blood flow pathway to the nipple and areola, and then the second surgery (the top surgery when done via inverted-T or buttonhole) can interrupt this blood flow and threaten the survival of the nipples and areolas.

i'm late as hell i know lol but i'm confused as to why you can't get a flat result with these surgeries. there seems to be a wide enough opening to get out most of the breast tissue and contour it. why is it different?

Thank you for your inquiry! There is actual tissue left behind, which is attached to the areola. We call this tissue the "pedicle" and it remains behind. It gets compressed by the skin but is still present and will often lead to a mound of one sort or another. It is not an issue of having a wide enough opening. it is an issue of leaving behind pedicle as an essential component of these procedure types. The "pedicle" is the medical term for the tissue that contains the blood and nerve supply and is why it's an essential component; without the pedicle the nipple and areolar wouldn't survive the procedure. Question: I understand that the inverted T procedure aims to preserve nipple sensation, but is it possible to receive the surgery and get a result without nipples? I am nonbinary and don't wish to have nipples in my end result, but I prefer the more full shape that inverted T results can give as I want a more gender neutral chest that can be presented femininely or masculinely. Is it possible to receive the same shape with double incision? I haven't seen double incision results before that have the fuller shape, the only examples I've seen that I like are inverted T, but I don't want nipples/nipple grafts.

Yes, we perform this procedure, but when done with nipples it is called a "buried pedicle technique." It is useful for patients to send in images for their consult, in which the images are the goals of maximum fullness and shape, with a body type that is similar to the patient's body type. The images can be “male” or “female” or nonbinary, and cis or trans, and pre-op or post-op - the main goal is the amount of tissue left over being communicated via the photos. We hope this helps!
