One way to play the game by ruling with an iron fist is to make it so that people have to pay for access to food and medicine - then have lots of military personnel who are well paid. They'll get first dibs to services, ensuring their happiness, and thus, loyalty. Then you can use them to kill any would-be rebels. Personally, I prefer the "stay in power by actually caring for your citizens" route, but I AM a Hufflepuff...
