As of 2021, professional services (those billed by any licensed provider eligible to bill for their individual service) is typically subject to the 1995/1997 documentation guidelines, whereas facility services do not have a single guideline. Rather, CMS has instructed facilities to develop their own tool (guideline) and consistently apply it. (See the 2008 OPPS Final Rule). Common tools/methodologies are points, the ACEP model, and the AHA/AHIMA model.
