You have to fill out the tax information from Twitch for them to pay you I believe. If you can't do that you can always set up a donation button.

I just got approved to become affiliated and I fill in all my details do everything right get to stage 3 with the tax set up thing and it says to sign online or email. I sign online the it says to exit and that I have done things right. But then when I leave it says that I haven’t taken the option to sign the signature online and that I must email it.
