Typically, the CC applications open at the end of the second week of the month. They'll run for a week, then close for the weekend. Announcements usually get made on Mondays. (This is a general rule, sometimes the dates shift.) All the announcements are in the "Applications" section of the discord.

It will just show up in the list of stuff you have. (You might get a notice on the headset in game, but I'm not positive about that. ) When you filled out the application, you gave them your Meta name. The reward gets tied to that.
