14 years of nursing here. More words mean more ammunition for the lawyers. Keep charting to a bare minimum. If there are check boxes, use those and don’t double chart. Focus on giving excellent care and your patient’s are less likely to sue you. Paperwork and bureaucracy steal millions of nursing hours.

I recently retired after 30+ years as an RN in a very busy neuro PCU and I am so glad to finally be done with charting!!! Charting is only done to CYA! My advice: keep it minimally simple but complete. The more you chart the easier it is for lawyers to pick it apart. Also, avoid double charting as this opens up a possibility of inconsistencies in your documentation. Use check offs and flow sheets when possible only elaborating if necessary. When a patient's condition changes always document the exact time and your verbatim interaction with the pcp. Never wait until the end of your shift to try and catch up on your charting, always set small goals like open your charts before med pass, update before lunch, etc waiting until the end of the shift just makes it more difficult, it takes longer as you are physically and mentally exhausted and trying to accurately remember everything that happened for 4+ patients over a 12 hour period is close to impossible and you will most definitely be at least an hour late.

Yep, and then the manager breathes down our necks about getting out late a few minutes because they are over budget. I always think to myself go after the habitual ones to get their act together not the ones doing their jobs in a timely fashion 🙃 If im a little late getting out it's for a legitimate reason (i.e. we had a code, the charge nurse decided I needed to give report on 5-6 patients to 5-6 different nurses and at least one of those dayshift nurses showed up late, we got 12 admits, etc)

Oh my gosh this made my day. RN for 44 years and now retired. I started as a volunteer at age 14 with steel bedpans that we used to have to empty and scrub and steam in the hopper😬😆😂. I was told to chart "will continue with nursing plan of care" instead of will continue to monitor because if pulled in court that can used that against you saying " well you went home and weren't there to continue to monitor." Maybe this if fine, it just scared me when another RN told me this, so I've been doing charting it this way since.

Don't forget the speed typer that just wants to get all of their charting done so they don't have to stay after the shift. And the priceless quotations: "Pt (patient) called this nurse a "b-word" when they went to get him/her up to chair as this nurse didn't know pt wanted a cup of coffee that pt "always has when they wake up in the morning". Pt reminded that language is inappropriate and to make sure to ask for a cup of coffee in the future. Will continue to monitor."

From what I saw on my clinical rotations during paramedic training, I am SO glad that I don't have to chart like a nurse. I seriously never figured out how you were supposed to fully chart AND care for your patients, and truth be told neither could most nurses. If they are honest they will tell you that you either need to rush patient care (and lower your standard you might have had coming out of school) or you need to rush charting and accept the occasional reprimand of your superior. The only way to not do both is constant, unpaid overtime. And let's face it: Nurses are not paid enough to do regular overtime without pay.

Ok so different types of charters: the quoter (extra useful for when a patient refuses their medicine), the safety maintained at all times one, the CYA charter, the passive aggressively roast the doctor charter, the omit certain details so that management doesn’t crawl over your ass the next day and cause more problems charter

I love the superuser! It is so true that they’re only there for like 10 minutes and you never know when they’re going to be there and you never can remember the questions that you need to ask them. And when they do explain something it’s super complicated.The charting system does suck so bad and I can’t believe how much Hospital’s pay for them. They are so antiquated it’s like I’m using an IBM from the 80s.
