This is a horror game. Lammy has schizophrenia and frequently hallucinates objects as a guitar, attempting to play them as one. This includes a baby, the controls to an airplane, and _a chainsaw._ She's a massive danger to herself and others, and yet, nobody seems to notice. Indeed, they encourage her delusions and take advantage of her for their own ends. It's entirely possible what Lammy experiences is not what's really happening to her, and most of the other (extremely uncanny) characters who show up are figments of her imagination with only a loose grounding in reality. The real Patrick most definitely sexually violated Lammy and probably murdered her, as the very next game sequence (in the uncensored version of the game) is Lammy going to hell.
