Well. To answer your question. The barrier is only in that one spot. The reason no one can get out through the other hole is because the ruins door are locked from the outside and no one from the outside knows there is a hole in there except at Toriel and Flowey. O one can enter the ruins, they can only leave… also, it isn’t like they can dog pile out of the ruins. Theoretically, Flowey can leave without the human souls by boroughing through the ground, but who would ever think of doing such a crime as truely despicable as that.

Except counter argument there is a monster thay can fly in the ruins also when the monsters where first trapped (if I remember correctly) they're home were in the ruins before migrating to other parts of the underground. A much easier explanation is that the barrier is more like an orb that covers the entire underground or something idk I just woke up.
