If you are sitting at home worried about your headset being loose before following brendons words of wisdom go grab your headset spacers (if you have any)under your stem if you can move them with your hands you need to adjust/add tension to that headset cap bolt. Good quick way to check pre/post ride

my headset is shot, if i loosen it anymore i get clunking ,but i dont wanna tighten it more coz it already feels indexed

I have a similar issue with a 1989 Trek 1000 which uses a quill stem and every few months or so I find the headset has loosened and is able to rack back and forth, so I just retighten it. Wondering if it's fine to just keep retightening or should I get it replaced with a modern headset?

+@bsamexican on that type of headset you may be missing the washer with the tab that fits in groove. Usually goes between the adjuster and the lock nut. It’s also possible that the fork is a little to tall to allow the lock nut to tighten down all the way. That said, threaded headsets do need adjustment more often. A newer headset could be a help.

They used to make a locking top nut for quill headsets. I have one on my old Raleigh mtb. Once installed never needed adjustment.

If you use threaded headset common problem is setting the stem too high above insertion line, that damages the threads
