Sama, kun, and chan are all titles, like in the US we say mr and mrs. Yuki-Sama is for people who are bellow him either in a job or rank, like the maid at the beginning. Kun and chan are what friends call each other girls are chan like toko-chan and boys are kun like tetsua-kun. But because toko-chan knows yuki was a girl she addresses him as such. As for San its just a general usage.
