On Friday 2 October 2015, a USAF Lockheed C-130J-30 registration 08-3174, tried to takeoff in the middle of the night, but a NVG case wouldn't let it happen. Don't forget to sub, like and comment for more videos.

@@Randomly_Browsing that appears to be the case (no pun intended). Between the expedited offload/onload and the visual limitations of the NVG’s inside the cockpit, PLUS any possibility they didn’t do a full control free-movement check during taxi….. all the holes in the Swiss cheese align

From the AF accident investigation report; "The ERO checklist did not include a step requiring the pilots to check the flight controls prior to departure and therefore, it was incumbent on the MP and the MCP to remember to remove the hard-shell NVG case" . A simple one line addition to the ERO checklist would have been ONE thing that COULD have prevented this mishap. Also, the 130-J has no FE. A good FE may prevented it too.

I haven't flown the C-130, but having done many an ERO in another Lockheed transport that accident report makes no sense to me if it states that as the cause of the accident. In My airplane The ERO checklist replaced the after landing and parking/shutdown checklists, it did not replace the before takeoff checklist, which is always required and contains a flight control check (along with setting flaps, trim, and other critical items before flight). Adding a flight control check to the ERO checklist would be redundant and might have saved this crew, if they had not also been distracted there and had actually complied with it. A flight control check seems misplaced during an ERO other than the fact that apparently propping up the elevator during onload/offload seems to be a thing on the C-130, so you want be sure it is clear before moving off.. But in any case the before takeoff checklist must contain a flight control check.

The inference of: "it was incumbent on the MP and the MCP to remember to remove the hard-shell NVG case" can be read as implying that no memory aid to ensure freedom of flight controls was in place. Reading the complete report might not leave one with that implication.

The accident recreation board noted the case was inconspicuous to all the pilots under the lighting conditions so there might need to be some marking on it that would show in the crew vision but one of those hindsight items.

Never would have happened if they went through the proper take off sequence. That’s why they are there and you are ordered to complete every time. He had checked his range of motion of the various flight surfaces he would have found out then and he’d been super relieved RIP

Do military flight have to do pre check lists like private and passenger planes, Maybe tidy dashboard should be added to the list thrn they should have spotted it and night vision goggles be put on when they start to taxi then they eoukd have seen it or once theve got to flight level cruise

It should be mandatory to check all controls and their correct movement before engine startup. Every model pilot knows how essential this is, even after a few flights. Why isn`t it mandatory for the big ones?

As a model airplane and helicopter pilot for 43 years i can proudly say: i never lost control due to a defect or malfunction over a model and had only 3 crashes. One was in my beginner time, the second as i started with helicopters. The third and last was 8 years ago when i was flying reversed very low and my small plane was hit by a sudden blast over the trees, pushing it to the ground. I stopped flying 3 years ago because i noticed problems estimating how far or close my plane was after it was lined up for landing. Eyes don`t get better when you get old, and before causing an accident, i decided to stop with this beautiful hobby. My buddies always said i`m taking it too serious. They had crashes due to malfunctions, i had none. One of them crashed his 8 kg aerobatic plane into a parked car just after the start, luckily nobody was inside the car. The reason was a faulty servo on the ailerons, causing short circuits when moved into one direction, making the receiver fail. So he lost control completely. The damage caused to the car was around 20.000 bucks. He checked his controls only before the first start, had 2 flights without any issues, then he started to the 3rd after 1 hour and crashed. That`s the reason why i asked my question about mandatory control checks.

A good, solid aviation accident report, for the most part. But it does contain one error: your location caption is wrong. The province of "Nangahar" is a province of Afghanistan, not a province of the non-existent nation "Middle East".

I doubt they were on goggles yet unless they take off without landing lights and all the lights on at the airport. We usually transition to goggles after extinguishing landing lights on so forth.
