Nice to see a testimony by someone with a full head of hair. Im just about to turn 46. I have a thick full head of hair and seeing all these testimonials by guys with little to no hair makes me doubt getting on TRT.

Contrary to what some believe, TRT will not cause balding. That is largely a hereditary thing due to the number of DHT receptors each unique man has, to put it short and without a ton of explanation. At 46, if you still have a full head of hair, you should have nothing to worry about under this topic of discussion! I too have a full and very thick head of hair all around and I'm 41 and have been on TRT for 7 years.

@@Roger-Bowman I'm realizing that if this is going to happen for me, I'm gonna have to get a different doctor. She wont prescribe because my levels are at 780. But I experience all of the symptoms. Looking into this, just the other day, I heard of something called testosterone resistance. Which means that even if you have a "normal" level of testosterone that your body will still feel the symptoms of having a much lower level. I think I heard about this from Jay Campbell.

@@robbrown39 Testosterone resistance could be a reason in rare cases with men who may express correlating characteristics with this. However in other cases and something else to consider, is taking a solid look at what your "free" testosterone and SHBG (sex hormone binding globulins) are looking like as well because if these are not in a preferred "zone" - numbers wise, this could also be a culprit here in your case. You can have a "full bucket" of testosterone that is being made endogenously, but if hardly any of it is unbound or free to be used, you will still have all the dreaded symptoms and complications of low T - because essentially, what your body actually has "access to" is in fact very low. Hope that makes sense, just speculating to help you think of the different possibilities as we are all so unique and different. And if that were the case (low "free" T) and given your total testosterone # looks decent, a few OTC supplements may help a little (and I normally don't say that for most gracefully aging men with low T concerns). I think I will upload a video tonight or in the morning discussing this topic of total vs. free T and understanding it all - in a very simple way.

This video is 5 years old. Since TRT is a lifelong undertaking, I'd like to know how the last 5 years have been. Are you still on TRT? Did you ever go off of it? I'm about to start my program, any advice for someone new to using the cream? 1x a day or 2x a day? Where did you run it in at, and did you change areas?

I've been on TRT now for 13 years straight. I've never come off or stopped. All has been and is still good. My first 5.5 years were injections then I switched to cream (for last 7+ years). Start at a moderate dose, more is not better. Re-test and adjust dosing if necessary. Pay attention to other correlated bio markers and address if necessary. As for cream application sites, I have personally not changed areas. That is not to say that would not apply to you or others as some do find it best for say, scrotal or other sites. We are all different and you will dial in what you find works best for you (hopefully) within your first 6 months and get on cruise control after. I know it's a bit vague but medicine/wellness/fitness is rarely a once size fits all so I'm just trying to be honest.

A very honest and informative video. One observation thought, those test levels were smokin high 🙀🤪, with those level I reckon you grew that beard overnight 🤣😂🤣😂, it’s a cracking beard hey 👍👍 Good to hear that you keep on top of your bloods, I get mine done every 6 months (full blood count ) with 300ml of blood drawn every 4 months just to control things. You come across as a pretty well researched and smart guy..

Hello Roger! great video, immediately subscribed! I have a question. I started 4 weeks ago with 10% testosterone and 5% Chrysin. I am 44, my test level was 274 before starting and I will do blood test again end of August. I also take other supplements as Turkesterone and beta ecysterone. do you think I can keep using them together or stick only with the cream? (overall in 1 month using the cream I feel already better, more energised, however I was expecting more, I do 2 clicks a day - same dispenser as yours in the video). Any advice?

Thank you for the compliment and sub. I can't attest to turkesterone and beta ecysterone improving or having solidified, proven synergistic effects on TRT but I also don't see (generally speaking) there being any harm in incorporating them. We really are just all so unique with so many variables (bodies, lifestyle, diet, age, genes, etc) so it's really hard to make a solid statement on these supplements... I have personally, however, used turkesterone with my T creams a few times (cycling 60 days and 60 days off) and I did feel as though it gave me a nice "boost". Now whether that is a placebo effect, in my head or it was legit - I don't know but I do plan on using it again and I feel like it helped. Last: I think it's important to look for a product that has a good standardized extract with a clinical dosing that is 3rd party tested or certificate of analysis and a GMP label. I personally will spend a little extra to see this with my products. Just re-test your labs randomly, especially in your first year of TRT and when or if incorporating new things that you want to see if they make a difference or not. Hope that helps, just my two cents on things here!

Hi Roger. So my levels were much like yours when first using the cream about a year ago. Also my dhea was high, but I’ve been taking 25mgs a day, along with Zinc, Cooper and D3. Was told that they might help bring down my estrogen levels which were also high along with my testosterone levels. After a few months of feeling great my results of feeling great leveled off a bit. I was retested in June of this year and still pretty much the same high levels that you experience. So just a week ago I went in to do labs again and my testosterone and estrogen numbers plummeted to pre testosterone numbers. The only number that stayed high was the DHEA which I’m still supplementing. So my concern and question is how can my body go from being a great candidate for testosterone cream 6 months ago to literally not working at all now. Using the same compounding pharmacy, same formula, (which has always had an estrogen blocker in it) same amount of clicks. Could the body absorption change that much, from fantastic absorption to no absorption? Could I have possibly received a bad vile of cream last time? Lastly how do you ally your cream, where and how long do you rub yours into the skin. Hope to hear back from you.

Hi Maxxie33, sorry for the slow reply - haven't checked in here in a minute on comments. Doesn't sound right that your retest showed low T levels; did you apply your creams within a reasonable timeframe before blood draw I assume? Like say, 6-12 hours or so prior? Assuming you did, it sounds like something isn't right with your creams and I would consider questioning the pharmacy where it came from... As it pertains to where I apply my creams; personally, I have always rotated between the back of my calves (80% of the time) and my shoulders/inside of forearms (other 20% when I know I won't be in contact with my wife).

Hi Roger, thank you for the informative video! I recently started doing TRT, (currently using the cream) at 40mg per day. I was curious, a friend of mine asked me if l should be taking an estrogen blocker due to the new aromatization of test in my body. I had not thought of that at all. Do you think an estrogen blocker is necessary for men doing TRT?

Hi Dave, thank you watching and good question. The short answer is "it depends". That is really a very individual answer to your question, all of which should be monitored and decided with your prescribing TRT doctor. As it pertains to therapeutic and responsible TRT and not your typical and excessive bodybuilder type of approach, in many cases an AI (aromatase inhibitor) such as arimidex or aromasin is very mildly used. There are so many factors that play into the answer such as your dose of testosterone and body composition (body fat to muscle ratio), to name a few influential factors. Given or assuming that your pre-TRT levels of total estrogen or estrodial (aka; "E2") were well within healthy ranges and also given the low dose of transdermal testosterone you're taking (40mg topical is a low dose and not quite enough for most men), I would guess that you would not need any AI at all and simply let the little bit of extra testosterone aromatize over as it would naturally as it most likely would not be much anyway. However, if you were to discover that your 40mg dose is not quite doing the job for you and getting you into an optimized zone of total testosterone and you go up to something such as a 10 to 20% potency (which is 100-200mg per gram of cream)and you get up to around a 1000 or so on your testosterone, you may then find that you need something mild to control any excessive estrogen from conversion from higher levels of testosterone - again, if needed. The odds of this "need" may be higher yet if you are carrying around extra body fat, especially subcutaneous type. A proper diet can also play a small role here too.... Consuming/emphasizing cruciferous vegetables (Like broccoli that is high in DIM) is a good thing and of course avoiding manufactured foods is another. My thought on TRT as it pertains to a correct, therapeutic and a healthy approach to look and feel our best is, less is often more. What I mean is, "more" is not always better and the more shit guys add in the more side effects that come along in time which means more stuff to take and becomes a complicated mess for men who don't truly understand the big complete endocrine and health picture that is going on. So my message is, I would only incorporate things that are needed and optimize hormones within endogenously made, preferred healthy parameters. But again and back to your question in closing; many variables and its important to re-test and re-asses things 6 weeks or so in to make any necessary tweaks or adjustments that may be needed. Every individual is often different but there are of some common threads that do run with a lot of us men on TRT as well. And, it can depend on your doctor and what he thinks and believes as well, which a whole different topic. Educating yourself on the basics in my opinion, is a must when it comes to TRT and finding your optimized happy place with things. This can sometimes take a few months and maybe a little trial and error with your program and even for many men, finding the right doctor that is on the same page as them as well.

Hi Eddie. My creams come from Creams n' Caps pharmacy in MI. I have received quite a bit of products from them through the years and they appear to be a legit and good pharmacy. I have even visited the place in person a few year back and they abide by FDA standards and regulations quite strictly and made me feel good about them. I tend to respond like many men on this particular dose but maybe just a pinch higher than some. However, I also know that I maintain a level of health, diet and training much better than the average guy as well so I suspect that plays into things to some degree for my lab results. Timing of last application of creams and when you go in for blood draw makes a difference also. I applied 2 pumps t cream 8 hours before blood draw. No anastrozole (one - .5 (half) mg taken 3 days before on blood draw). I talked with a guy this week using this same potency who applied 16 hours before blood draw next day (in the afternoon) and his levels were in 700's. Naturally, it will go out of the system over time hence the need to apply daily to keep it up. Hope that helps give some insight.

Today is day 1 for me using test cream. I've been getting injections for about a year and I experience alot of peeks and drops with it. I'm hoping this will help balance it out a bit. Thanks for the insight.

I have been using injections for several years now,, I am very interested in the 20% cream,, what is the brand of cream and is it avaiable at most pharmacys?

@@SteveHofsaess , topical creams at this higher dose are going to be compounded and I don't know of any brand names that will go this high. That is really the main difference for men to be aware of is that these will not be weak like the regular brand name gels at 1-2% that are more commonly scripted by unknowing PCPs. This being said, you will not find these higher dosed creams in the main stream, big name pharmacies but rather being made to a doctor's specifications in and at compounding pharmacies. The creams I use and show here in this video come from Creams n' Caps, which I believe is a very good compounding pharmacy. I have used many products from them for several years and I even toured their pharmacy a few years ago when I used to live near where they operate in Detroit, MI area at that time, and they are pretty legit.

Have you ever had 100mg of the cream instead of 200mg? If so were there any difference in blood works and how you felt over all? Love to hear back from you

Hi Julian. I have always had the 20% (200mg/gm) scripted T cream. However, it takes two small pumps to get this. That being said, taking one pump is 100mg application and I have done this prior to blood work before and although similar the lab # come back just slightly lower (vs 2 pumps at 200mg). I always try to time out my blood draw around 10 +/- hours after my last application of cream. For quite some time now, I only apply one pump at night close/prior to bed time (100mg). For me personally, 200mg (2 pumps of this cream in video) is just a bit much and I try to keep things in check so I don't negatively affect other bio markers revolving around cardiovascular and just general health. Keeping good markers all around and my T levels around 1200 works best for me personally. Hope that helps. I will eventually get around to making another video that is more updated in near future, I think it is overdue as I'm about 4 years in now on T creams here. Just need to get some updated labs soon.

great video. O had used gel in 2009 i believe it was 5% and my test levels never went over 300ng/dl. I have been on test cypionate for 4 years now and it works great but down side is getting pinned weekly. Dont wanna be a pin cushion anymore or at least for some time. I will try having cream ordered by my endo and go from there. Seems it works great.

1500 is (or was) a little high for most guys, yes. I recently re-tested and I am hovering around 1200 which for the most part since starting T cream 4+ years ago is where I typically am. Creams I use and discuss here in video are bioidentical. For those that qualify and may be scripted for by their prescribing doctor, some of the compounding pharmacies will offer "combo" creams as well. For example: 50/50 blend of bio T + stanozole or a 50/50 blend of bio T + nandrolone. To elaborate or clarify; two pumps on these blends could also go as high to reach 200mg/gram (2 pumps = 1 gram), so they are quite effective.

Hello Roger, can you tell me 1) what type of testosterone powder you use for your cream? Testosterone Propionate, Enanthate or another? 2) Did you get a prescription for that or did only order that on line ? 3) I am tring to get the same thing but I don't know where to begin. Hi Frank, thanks for watching. The testosterone is bioidentical and made by/at a compounding pharmacy out of Detroit, MI area. Propionate, enanthate, cypionate are fatty acid chains. This are typically associated with options with injections. These (T injection vials) are suspended in an oil (or water) base with testosterone for injections. Proprionate is a short chain fatty acid and is cleaved off quickly, typically in a day or two - which is why it has a short life after injection and not used as much. Enanthate or cypionate are longer and will therefore have a longer life due to the cleaving off of more pieces of a longer chain over a couple weeks of time. This doesn't apply to creams, which are applied daily and taken into the system via skin. As to the formula each compounding pharmacy uses for their ingredient list/make up of cream, I don't know and pharmacy will not release from what I'm told (very frustrating b/c I want to know every ingredient mine has). I have a legal and doctor prescribed TRT program that I have been on for many years. Testosterone is a very controlled substance here in the United States and is illegal to posses without a prescription. As for you getting started or looking in the right direction; That will vary based on where you reside - ? If you live in the United States, I can give a further reply to provide some guidance there to help you step in the right direction. If you live outside of the United States, I'm sorry but I am not familiar with laws, etc. in other countries.

I'm not USA (canadian) citizen but if I need to meet a doctor there to get a prescription, I will. It is difficult to find a doctor willing to prescribe testosterone or I don't know how to ask. So if you can guide me how you have done that, I will follow the path. I go in USA at least four time an year. Again, thanks a lot.

You can start by contacting a few compounding pharmacies that emphasize they specialize in bioidentical hormone replacement therapy and ask them who some of their best "customers" are or in other words - doctors that send business their way. I think i understand dosing and potency. 20% equals 200mg/ml. What im running into is confusion with the dispensing system. My cream is 200mg/ml but the dispenser is a click system. From what i have read that each click is .25ml or 50mg. I get 2 clicks a day which is 100mg least i think i am correct. Can you please explain your pump system and exactly how many mg's you are consuming a day. Im guessing that the pump system is more than the click system. Maybe .50ml per pump??? Please help. There is very little on this topic of dosing systems with compound creams. Thank you

Thanks for the video. I am 41... around two months ago started the TRT therapy have been using the compounding cream 3 clicks on the shoulder and upper arms. My Test scores after 8 weeks have gone up from 281 to 310 still lower than that of a 80 year old. Any advice .....is the Test suppose to up so slowly? My Dr. Seems very conservative with his dosage I wanna be soaring above 700.

Something isn't right here. Few questions; What is the dose/potency of your cream? If you have a low 1 or 2%, then these numbers would be pretty typical. However, if the potency has a claim of anything between 10-20% and you had applied within the previous 7-10 hours prior to blood draw, your numbers should be much higher than what you're reporting. If this applies to your scenario here, I would question where it is coming from and the regulations (if any) on the compounding pharmacy. Your T numbers do not need weeks and months to "build" per say, so with a good product and appropriate dosing you should see improvements - at least on lab #s, soon. Other physical and mental wellness benefits with TRT do however, take longer to come about as your body adjusts and gets dialed. Last, most guys tend to apply cream on more vascular areas of the body such as back of calf/knee or inside of elbow/forearm areas. If your doctor is too conservative and not willing to script a proper dose or potency of cream, then you can always consider testosterone injections (if your doctor is willing) to get a more appropriate approach.

It makes a huge difference where you apply the cream! If you apply it to your scrotum, the absorption rate goes up dramatically than if you put in on your shoulders and upper arms. I put one click on my scrotum and another click on the upper inner thigh in the morning, plus one click on the upper inner thigh in the evening. My T level went from 295 to over 900 in a few months. The cream potency is 20 percent. I am concerned about putting more than one click on my scrotum, since my T level would probably go too high (I am 72 years old). Putting one click on the scrotum made a huge difference for me.

Personally for myself, there is not much of a difference at all between 1500 and 1000 pertaining to performance (gym, energy, libido, etc). However, I honestly do maintain a very healthy lifestyle, clean eating and I've been dialed in nicely for many years. I do agree with your statement (and others) with saying that is 1000 is high, because it is and is plenty. More or higher #, does not always mean better and I'm no exception here. Fortunately aside from my RBC+HMT being a pinch high (which I will donate some blood soon to help this and someone in need), I have nice #s across the board and feel great. One of the main reasons for this video and its predecessor video I originally did on creams over a year ago, was simply to demonstrate that properly dosed T creams work extremely well and should not be compared to low dosed 1-2% creams as though it is "apples to apples".

I personally have not had any issues here BUT, I am very mindful to avoid it. I do not have any kids but I am married. At night, my wife sleeps to the right of me in bed so I simply apply a pump on bottom of my left foot at night so it doesn't even come close to her (I always sleep on my back like a mummy). And then in the mornings I apply to the back of my calves/knees. I cover this in the last couple minutes of my latest 4 year review video I just posted - you just fast forward to the last 2-3 minutes to jump to where I mention application sites, etc. - kzbin.info/www/bejne/ioLCd4SIdsiahc0&lc=Ugw0vNt-uzIsNI7uiMB4AaABAg I also wash my clothes separately from hers. 10 hours after my last application the night before. I have always done this for the last several years with applying around 10-10:30 pm and then going in for blood draw fasted in the morning around 8-8:30 am. The only thing that has changed at this time versus when I did this video is that I know only apply one pump at night (vs two before and around time of this video).

But of course, you should always consult with prescribing doctor on these things 😉 Nothing wrong with donating blood either, just be mindful of doing too frequently to where you could cause an iron or ferritin deficiency.

Awesome question Albert, I was just having an in depth discussion last week regarding DMSO - but as it pertains to cellulite, inflammation and other topics/uses - many of which I have suggested to others in various cases for the last 15-20 years to explore/consider. The short answer is though; I'm no expert with DMSO but my assumed answer would be yes. However, I would NOT suggest or dive in with a DMSO and then T cream after application approach here. DMSO helps larger particles penetrate through the skin and reach its target site(s) whereas they may not otherwise be able to without DMSO and it's mechanisms of action to help do this. Speaking from experience, compounding pharmacies have done a great job with a lot of these creams and their ability to penetrate and absorb transdermally through the skin quite effectively - hence my video here and experience talking with and helping hundreds of other men working as a patient care coordinator within the HRT space for the last 10 years. Good thought and question to ponder nonetheless! And, I would be curious to hear anyone with feedback doing this. I'm just not willing to try it yet myself, as it applies here with testosterone optimization talk. I would however consider DMSO for inflammation/injury/joint pain, etc combined with MSM or similar compounds - but that is another topic.

Where do you do your labtests? And what does it cost? The normal docter would not do it, only if you have serious problems, wich I don't have. I would like to know what my Test hight and IGH is at this moment and see if it decreases in 5 and 10 years. So I can fill it up in the future to my own natural values of my age now.

Lol... Normal is such a loose term and can vary so much from man to man. Traditionally speaking before their parameter change, Lab Corp (very large testing company) had normal lab values for men placed between 348-1198, which is a large range. Symptoms need to be coupled with labs and more to determine and dial in an appropriate (and tighter) optimized range. But in short to answer; yes, 600-700 is normal but may not "feel" optimized for some men. In other words, it is pretty normal for a 70 year old guy to have total testosterone numbers around 300-400 and also very normal for a young 23 year old guy to have numbers around 1100. As we age, we lose 1-3% a year. Sometimes, this just goes quicker or pre-maturely in some men vs others due to a variety of reasons.

Hi k b. I do not, no. Unless you're taking black market or unregulated products, you are not required to state anything here. Red Cross asks something along the lines of "Are you taking anything that is NOT prescribed by a licensed physician" or something similar. And, everything I take is in fact doctor supervised, legally scripted and comes from a highly regulated and registered pharmacy. In other words, there is essentially no doubt that what I am taking legally, legitimately and doctor scripted is exactly what it is supposed to be and not something tainted or different than what label claims it is. Unfortunately, too many guys roll the dice with black market drugs and this I assume is what places like Red Cross want to avoid contaminating a blood donation. typically, back of my calf/behind knees or underside/inside of my feet. If I know for sure I won't be in contact with my wife, I may switch it up for the hell of it and apply on inside of my forearm/elbow area. Naturally, we have to be mindful to not rub any of the cream off and onto kids or women.

Personally, I apply my cream to the back of my calf muscles (1 pump on back of each one) 90% of the time. Nobody is going to be touching or rubbing against that area and its a vascular area as well. I will also occasionally apply to the inside of my elbow/forearm if I know I will not be in contact or intimate with my wife that day. Many of the guys I consult with do the same here or even apply on the top of their feet if they are being super careful (or, are paranoid) with being around kids. And of course there are those that apply to their scrotum, which I have never done nor do I have any interest in changing up what I have been doing for a few years now to do so (if It isn't broke, don't fix - for me).

Hi Mike. Prior to starting TRT over 8 years ago, my total testosterone levels had dipped down to the 330's for unknown reasons at the time. I will say that I was (as I always have) at the time living a very clean lifestyle with regular training in the gym, meal prepping and eating clean, etc. Fortunately, I had been working as an office manager of an HRT clinic at that time and our on site doctor I was working alongside every day was able to address and help me start moving in a good direction with getting my hormones optimized. I have since never looked back and have felt great since and, have maintained excellent health along the way. My current levels peak each day between 1000-1300 mostly, with total T. Of course there is a slight expected bell curve on this from morning and then through the day after application. Hope that helps to answer your question.

Hey Rodger , I’m 20 and just was prescribed TRT from my doctor in Canada . My NG/dl was 335, am I too young to be on TRT ? My doctor wasn’t concerned he isn’t one to just hand out meds and didn’t have a problem with giving me the TRT, he also gave me a topical cream

Solid question. The answer is, yes. I have used HCG twice a week for almost 8 years since starting TRT. I also supplement with a little zinc daily as well. As long as I don't go above 1400-1500 on total testosterone for too long, I don't get atrophy that is uncomfortable. But mostly, I take HCG to keep a few pathways "flowing" from leydig cells in testicles. My wife and I don't intend on nor really want any kids, so for me fertility isn't my reason either. But for other men, it may be their reason. Naturally with any form of TRT, you will have some extent of testicular atrophy. Personally, I've always used a small dose or two of HCG on a weekly basis since I started TRT back in 2012 to keep them stimulated a little bit periodically and I've never had any issues with pain from any severe atrophy to the point it is painful or extreme.

Good question. Fortunately for me, I have never had an issue or concern with hair loss and I've been on TRT for over 8 years now. I have struggled with acne, I have terrible vision and other struggles but, I do have a thick full head of hair like my biological father does. Although reasons can vary, the most common cause of male pattern balding is due to genetics, often having a family history of it. Research has concluded that male pattern balding does have ties with androgens, which are male sex hormones. These hormones play many roles within the body of course, including the regulation of hair growth. As contradictory as it sounds, these androgen hormones can both help and hurt. The thought that all men with male pattern baldness have too high of a testosterone level rarely holds any validity. If we sit back and think logically about this as well with regards to men prone to male pattern balding, we can surely conclude that most of these younger guys in their late teens and twenties with full heads of thick and healthy hair have much higher testosterone levels than older guys in their 30s, 40s and 50s that happen to have much less hair. But again, balding in most cases is due to genetics and most men should blame their father or grandfather - not testosterone. In some cases, testosterone or TRT "may" speed up the process of one's balding time clock. Trying to figure out male pattern balding without having all the answers can get a bit “patchy” and confusing, so I can empathize.

Roger Bowman's Fitness & Wellness Coaching thanks for your response. One last thing when I am on Test Gel in the winter especially I get cough with a lot of mucus. Also my LDL shoots up every-time. I ain’t sure what the cough has to do with the gel but there is some link since I get it every time I use it in the winter.
