Thank you so much for these videos! For the first question in this video, would you mind elaborating on why IV administration wouldn't have the shortest Tmax? I understand inhaled anesthetics are "closer" to the arteries but IV does not require absorption whereas inhaled anesthetics would depend on the blood solubility and MAC of the specific inhaled anesthetic administered. Thank you!
@JooAnnna3 жыл бұрын
I WAS ABOUT COMING TO ASKED THAT BUT IT DONED ON ME THAT IV WILL HAVE TO GO TO THE HEART, THEN LUNGS, THEN BACK TO THE HEART BEFORE REACHING THE ARTERY.
@deepakhiremath61712 жыл бұрын
It should be IV route unless it is a continuous infusion. IV route has no absorption and Cmax reaches almost as soon as drug is injected. Inhaled anesthetics take a little bit longer than IV

eh some of the inhaled barbiturates have multiple volumes of distribution and their time to c max can be measured in hours since they take forever to partition into neural tissue/across BBB. vs an IV bolus of diacetyl morphine. pharmacology in medschool makes no sense to me, just memorizing a bunch of short hand rules that have more exceptions than applications. this is coming from someone who used to be a pharmacist. I guess you are breathing out inhaled anesthetics pretty quickly so some of them dont get to very high concentration and therefore their time to c max is pretty early.... but in general there is no real way to know for sure given how vague the usmle questions are. and i guess iv infusion is slower than iv bolus but thats not clear to me in the question. owell.
