Hello Dr Neil. I thought renin activity is going to be low in both (11 and 17 deficiencies) because both will cause more mineralocorticoid production without the assistance of renin (even though 11 will cause production of a hormone with mineralocorticoid properties instead of aldo). Also this patients will reabsorb more sodium which will turn off the renin activity. I will appreciate if you can clarify this.

I think it points more towards 11 rather than 17 alpha. There is no mention of abnormal virilization symptoms at all. And should we consider prevalence too??

Hey Dr. Neil, I have been following you since my step 1 studies which was now over a year ago. I wanted to say thank you for all these videos and help. I am now finishing up my third year and gearing for residency apps/ step 2. I 100% have my sights set on psychiatry. After all my cores and a few electives, I have found that I get no greater fulfillment than when talking to psych patients. Listening to what they have to say and their stories is just so captivating and I wanted more. As a psychiatrist your self, I was curious if you had any words of advice or things you wished you did differently during applications. I am an IMG! Thank you so much for all your help.

I have no "mistakes" as I had a good outcome... My advice is "Quiet confidence" and know/expect positive outcome.

Dr. Neil, why not 11- hydroxylase deficiency? It also causes hypertension.
@RandyNeilMD2 жыл бұрын
If it were 11 it would have pushed it towards reticularis and had issues with sex hormones…. 11 looks like 21 but with HTN
@littleboychildhood1962 жыл бұрын
Hi Dr Randy, 11 hydroxylase diff still give hypertension. and the questions only give the boy and does not give the xx or xy. HOw to differ btw 11 and 17 diff?
@RandyNeilMD2 жыл бұрын
11 deficiency would push towards the reticularis (more sex hormones); 17 deficiency would push towards Glormerular...

@@RandyNeilMD so in this question, if he was phenotypically "a normal male", it could be 11 hydroxylase deficiency right? Wont there be a history of hypogonadism/feminization/eunuchoid habitus with 17 hydroxylase deficiency?

ACTH isn't the substrate, cholesterol is the substrate... This mistake is a bit distracting honestly. Why do you start the diagram by putting down ACTH?
