My husband has had mini strokes and even a brain bleed. No migraines or headaches. We were told Frontotemporal but it sounds more complicated than that. Communication and speech issues, brain bleeds, urinary incontenance, confusion….are the major but not all the symptoms.

I ask , with huge respect - if you might publish your degrees , specialised qualifications and training in this important subject and furthermore , how many years of clinical practice do you have ? Any papers written , published , acknowledged that we could refer to ? I ask for this for the sake of reassurance . Thank you.

My dad is 58 and has been diagnosed with vascular dementia I'm concerned as my mother says he's still an adult able to make his own decisions. But the decisions he's making are killing him

My mother has it as well...there comes a time where they can't make right decisions and you will have to do it for them. Even if they fight you on it it's in their best interest.

​@@coldgin2023it's the fighting that takes the toll for me. I am becoming so mean dealing with my mother. One minute I'm dealing with a 3 year old in the body of an almost 70 year old, and the next she's normal. She will fake things to manipulate people to get her way, and it gets harder and harder not to spew venom at her.

Thanks for this wonderful segment. Your careful organization and targeted delivery of what could easily become overwhelming detail is much appreciated; my notes wrote themselves. Also grateful that you stick to the medical lingo - I always feel more confident engaging my Dad’s doctors when I have the right words instead of watered down descriptions. Also makes further research that much easier - wish I’d come here first!

Does anyone deal with a family member that has changed personality? Also anger issues, 1 day they wake up and are depressed see everything negative, they accuse you of things. They interprete things the wrong way. Take things literally…and maybe couple hours later they are normal :( What do you do if the accusing is bad, who can help (?) and will a doctor keep the secret of not telling the person that you want them checked out because they get angry if they know that you called for them to be checked out 😔😢 tiring. Sad situation

Thank you, Drs. Sherzai. I don't know how to get my husband help. He's been having significant memory issues for the past several years and his PCP and a neurologist (I insisted he see one), blew off my concerns. My husband doesn't think its anything more than occasional forgetfulness, but its way beyond that. I've had to take over all financial and household functions because his executive function is seriously impaired and it's greatly affected daily life. Memory issues started in his early 60's; he's now 72. For quite a few years, I noticed changes, but he kept saying he was just burned out at work. He worked 70+ hour weeks and was not home much. But after he retired, it became quite clear just how much he was struggling. How do I find a memory specialist who will do a full work up and give us some answers?

If your PCP and your local neurologist were not concerned, I recommend that you contact the nearest ADRC (Alzheimer's Disease Research Center) authorized by the NIH (National Institute of Health) and volunteer your husband to join their research programs especially their "Longitudinal Brain Health Research Programs" to see if they can find out the reasons for your observed symptoms. There are 35 ADRCs within the US. Good luck.

I have Antiphospholipid syndrome. Multiple tias . Severe periventricular deep white matter disease. Chronic small vessel disease. . Fasciculations myoclonus dizzy balance weakness intermittent blurred vision. Im scared 😢

Can you speak on the topic of how it occurs following cardiac arrest where a person has been worked on (cpr) for several minutes in a cardiology? Specifically undiagnosed Anoxia following a cardiac event. No neurologic work up and rehabilitation where later someone has mood and cognitive deficits emerging 6 months after the event. This seems to be a topic I do not hear but see. Thank you for your education and sharing your wealth of knowledge.

Does imaging such as MRI detect this ?

Yes it can definitely detect signs of vascular disease.

I was recently told my 76 year old mom has vascular dementia because of fatty deposits in her veins and or arteries (I don't remember of its one or the other or both) She has had open heart surgery (4 years ago) and is diabetic 😟

It’s critical to do a more thorough evaluation, as a vascular, cognitive impairment, and dementia is amenable to aggressive therapy and lifestyle intervention. Much love and success in your journey!

@TheBrainDocs what kind of therapy would you recommend? She lives in a home now where she'll be until she passes and her caregivers are wonderful. She also has a cardiologist appointment very soon.

Hi Helen, I have Cadasil also and live in the United States. There are a few Facebook groups for people diagnosed with Cadasil to join if you are not a member already. One is CADASIL support and the other is CADASIL-ians. This disease is rough and many drs don’t know enough about it to give much help or hope to us, the FB support groups have been very helpful to me! Take care.😊
