From what I understand it seems to be deeply metaphorical. Carcinogenesis is a form of cancer where normal cells turn into cancer cells, but it seems to draw a paralle between the disease and some sort of interpersonal struggle, particularly in familial relations. The album makes many references to familial terms like "matriarch" and "fathers" so there is some definitely a sort of metaphorical meaning behind the lyrics, I just can't decipher them yet. But great comment actually
