I greatly appreciate the care with which you acquire your target before every burst. Watching many, many other floater lasering videos, I couldn't help but think that many times the operation of the laser seems careless and hasty.

Many people fail to understand the physics involved with lasers. They usually think of a tight strait beam, like a laser pointer. But as far as I know, and please correct me if I'm wrong, the laser beam is tweaked with lenses to focus it on the correct spot, tight enough to vaporize the floater, but not the retina behind it if the laser missed target. A true story about lasers. I worked for a copier company as a technician. They are just very big version of laser printers. Instead of desktops, these are the size of washing machines. They work by drawing an image on the xerographic drum, which is spinning at at several revolutions per second. And the laser is bouncing off a hexagonal mirror, that is spinning, to move the laser across the drum. Indeed, moving so fast that the laser needs to be pretty powerful. So laser safety is a priority. And one of those safety items is an interlock that prevents the laser from activation, when doors on the machine are open. Like a washing machine stopping the spin cycle, so your long hair doesn't drop in and get caught, yanking you into the washing machine, and going for a long painful ride, if you are lucky 😵💫. Well, I had this one machine, a color laser printer, that used a single laser to print all three colors. It was having a problem, where it was printing nothing. After some troubleshooting, I surmised that there was a mechanical problem with shutters, that blocked the laser. While testing my theory (I'll skip the boring stuff now, and get to the meat of it) I had to leave the door open to test. A safety interlock cheater was placed on the machine, that makes the machine think the door is closed. (mistake #1) . As I was attempting to repair the machine, I was using the flat black wrench, a non reflective tool, to make adjustments. It was the wrong size. Did not make any sense, but I've seen stranger things than that. So I used the next size up, (mistake #2) that was a chrome tool. Fully reflective, and I'm sure you see where this is going. As soon as I placed the tool in the machine, I placed it right in the lasers path... FLASH! A very bright light bounced off the tool. The laser light is not visible light, it is infrared light. The flash overwhelmed me. And I fell right on my ass. And I was completely blind. I said a few expletives, and the girls that worked in that office asked, "Bob? Are you ok?" Me, "Well, I dont know. I might be permanently blind now. I'll let you know in a few minutes. I sat there. Resigned to being permanently blind for the rest of my life. How am I getting home? What will I do now? Whats my wife going to think? It took about a minute. And my vision slowly returned. Moral of the story, respect all lasers. Even those cute little laser pointers to play with cats. Can damage you vision. Can make you permanently blind, very fast. Think what the lens in you eye does. It focuses light down to a tight spot on the back of your eye. So even a very weak laser becomes a very strong focused laser beam. I think what saved my vision was that the tool was cylindrical in shape. So the beam reflecting off was spread out A LOT by the time it traveled the 2 feet to my eyes.

One of the most dangerous types is a vitreous hemorrhage -- small bleeds in the eye when the vitreous detaches. Of course you don't mention that, because you don't TREAT those riskier types of floaters. Instead you tell people in this video that if they see 'dark' floaters, that's because 'you're seeing the shadow'. NO, it could very well be because they've suffered a vitreous hemorrhage. After three treatments on both eyes, involving 3x10 hour round trips and two hotel stayovers, not only was there no improvement, but he ended up putting a hole in the back of my lens capsule, which eventually led to vitrectomy and lens replacement. Some years before, Dr. Johnson was very kind in corresponding with me. If anyone can perform successful YAG vitreolysis, I believe he can. I still have floaters but no flashes. Cost me $1200 usd for laser treatment that I possibly didn’t need but it was what the doctor recommended so that’s what we did. FYI, I’m 53 and male.

Many many thanks doctor for providing useful information ,how floaters can be treated without doing any harm to the retina .
@Thefloaterdoctor12 жыл бұрын
The simple and short answer is to stay away from the retina. The longer answer involves describing the physics involved where the laser is not like the novelty lasers that you can buy at the store or even your awareness of hell, military and industrial lasers work. Those are thin culminated beams that if you were to miss a target the laser will continue on to the next target. That would be bad inside the eye. Instead, the YAG lasers have a cone-shaped energy profile, the laser is only delivered at the tip of the cone, that is, at one point in space. Primary responsibility is to know where that point is located in space and to keep it safely away from the retina and the lens. The advantage I have is 16 years of experience and over 12,000 procedures. As of this writing. There are many doctors who have YAG lasers but man oh man be careful with those who just occasionally treat or dabble with the procedure.

I have them in my right eye only, and likely about 70 of them, and I highly doubt you can get rid of them , but hey, if you can, I'm all in. An eye specialist here being an Opthamologist told me about 40 years ago, they were just dead skin cells, and the light cannot shine thru them properly, and they show up as floaters, in the retina, back part of the eye, that receives images. I don't see how you can get rid of them, considering that ? Just listened to an opthamologist who was being interviewed by the Mayo Clinic, and he said these are located in the Virous Gel of the eye, and they are parts that have compacted. He went on to say, that by having them treated by Lazer could make the problem worse, by blowing up the existing floater, and then it spreads into many different smaller pieces ? so he says, it's best to just leave them. With that being said, medical science is advancing all the time, and this gentleman just may be able to do what he says, and I'd certainly be up for that. Love to get rid of them as their very distracting. I'm sure there would be hundreds of thousands of people with these. Really good video here, and it's made me think ? BTW - about 40 years ago, I only had maybe 2 floaters, now it's about 70.
@Thefloaterdoctor13 жыл бұрын
I find it interesting when doctors say "Wouldn't you rather have one or two main floaters rather than a bunch of smaller ones. It betrays and reveals their lack of understanding of how people see floaters. You don't see the floaters, you are 'aware of the shadows' of the floaters. Small floaters - if far enough away from the retina may not even be seen, or they will be much less bothersome than a larger and intermittantly obstructive floater. I can not comment on yours specifically. Maybe yours are too close to the retina, and it all may be a moot point with you.

It's fascinating that the technology exists for a common problem as eye floaters. This procedure should be available through out the many institutions that deal with the eye. I also find it somewhat disturbing that the cost for such a procedure is so out of reach for the average person. I understand that the equipment is quite costly and the actual experience of the treating doctor to be indispensable. Having said that the cost of cataract surgery with a team of specialist is much more involved and less costly. It would be nice to see those that treat eye floaters charge a reasonable amount of money so that many more individuals can benefit from the procedure and regain some quality of life.
@Thefloaterdoctor12 жыл бұрын
That is a valid sentiment. I have not changed or increased my professional fees in over 12 years even though all my other expenses increase (rent, insurance, etc.) Treatment costs about the same as (fill in the blank here... fixing your transmission, a high end TV, etc.) It is impossible to answer this concern to the satisfaction of everyone.

Great info. When I first had floaters 8 years ago (In my late 20's at the time), I was stress & frustrated for months cause it tend to distracted my vision. What I noticed as years progress is that my floaters seems to be less less dense but more spread out like a thin cobweb. For whatever reason, its not as destracting as when it was smaller and dense. I remember reading your page about the YAG laser treatment back in 2012 ... now 8 years later I'm disappointed that the medical world still doesnt care about eye floaters. I havent really pay attention to the floaters anymore in past few years , although its usually more distracting when I'm tired, but I notice that its really becoming more and more spread out but more transparant. Any idea what actually happen? I have it in both eyes around 8 years ago and it actually appeared around the same time.

The appearance of the Shadows, or rather the appearance of the Shadows for the disturbance of the vision that you are aware of can change depending on where the floaters are located in the eye particularly in the anterior - posterior position. Very small and microscopic floaters that are very close to the red tonight with 10-day have more dense in distinct Shadows that you are aware of. If these seeing floaters move even one or two MMS further away from the retina, the Shadows that they cast can be softer and more diffuse and possibly even slightly larger. The position of the floater within the eye in regards to how close it is to the retina is absolutely critical. So it is possible for things to change over time although not in the way that some doctors would suggest. They will often tell floater sufferers that they'll just go away, or the brain of get used to it, but the reality is some may benefit by naturally moving a little further away from the retina where they are just a little bit less noticeable. This is more so in younger people whose floaters are very close to the retina. The people I usually treat are those where things are quite well settled in stable in the middle part of the eye where they generally will not get better on their own.

@@Thefloaterdoctor1Thanks alot for the respond and info doctor. For some reason I didnt get notification from YT for your reply until today! Yea , I read the info on your page that the floaters on younger people are different than those on older people. Now I could stare at white wall or blue sky, its still there but instead of looking like a thick eel/snake , now it looks like thin cobweb. Whatever causing the floaters shape to changed, its certainly benifits me. Sure there are times it could still be distracting , I have mild myopia and I remember it really bothers me in first 1-2 years especially when I dont put my glasses on. However, its certainly not as distracting anymore as I know I've been back to my old habit that is reading books , smartphone or doing sports activities without glasses. www.thefloaterdoctor.com/the-yag-laser Please see this detailed explanation of how the laser works, and it will better explain how even missing the target is not dangerous as you think. In the hands of an experienced laser floater specialist, the risk to the retina statistically approaches 'zero' because you keep the focal point of the laser far enough away from the retina and lens. Laser treatment can cause some cloudy floaters to re-form differently. Are these new floaters, or re-aggregation of just-treated floater collagen? It is asemantic question.

Thank´s a lot for sharing this information. But what happens to the membrane when you hit a weiss-ring floater? Isn´t it affected too?

Yes it is, although not as responsive to the laser. The membranes can be broken up and hopefully moved out of the way. Like I tell my patients, if you want to look out the window, you don't have to burn the drapes, just move them off to the side and out of the way.

This is very interesting. I'm curious if you have issues with cataract lens replacement patients? In my case, I have had replacement lens surgery to partially remediate a posterior subcapsular cataract. The posterior portion is still there and I've just now been diagnosed with a posterior vitreous detachment. A lot going on, but I still have pretty good vision - all other symptoms aside. Any advice is appreciated!
@Thefloaterdoctor12 жыл бұрын
Are you asking if I have any issues with patients who have had cataract surgery? Well, part of the answer is that I see these patients all the time. Since my bread and butter and typical patients are over the age of 45 to 50, a reasonable percentage of them will have already had cataract surgery. What does that do for me? Well if they are the so-called premium lenses there is a small aperture and a lot of optical glare and aberration that I have to do with. It does not increase the risk to the patient, but does decrease the efficiency and energy that can be actually be delivered. This may translate to some of the posterior located floaters being more difficult to treat or maybe not treated at all and quite possibly more overall treatment to compensate for the lack of efficiency.

Hello, I am watching your videos, trying to find information why eye floaters happen in younger population, if they are supposed to be a natural part of the aging process (and therefore expected to happen for those that are 40+), but I can't find this info, not even on your website. It only says it is related to "AGE, MYOPIA (near-sighted), TRAUMA, and UNKNOWN". It is the "unknown" that I am interested in, as I can't accept that as an explanation. Everything has to have a cause, so I have to ask you: if a patient was young and did not have trauma or wasn't near sighted, was there anything else he or she had in common with other patients from the same age group? Like, did they share the same blood type, shared a history of headaches, wearing contact lenses for the sole purpose of changing their eye color, had a job where they lift heavy objects etc? Is there absolutely nothing nobody found that these patients had in common? Thank you.
@Thefloaterdoctor19 ай бұрын
It is not well studied and so 'unknown'. It might be the entropy of nature going to more disorder, it could be oxidation and inflammation perhaps from illness or the widespread low grade smoldering inflammation associated with pro-inflammatory foods: oxalates, phytanic acids, ultra-processed foods and seed oils/ fast foods.

Can bromelain dissolve eye floaters? Thanks
@Thefloaterdoctor1 Жыл бұрын
Short answer: The blood brain barrier (equivalent is retina-vitreous barrier) prevents molecules larger than 400 Dalton from getting into the vitreous space. Bromelain is lager than 20,000 Dalton, so ingested bromelain should not be able to get into the eye. Injected directly into the eye would be different and it would have a non-selective enzyme action to all the protein material, not just the specific floaters.

Hello Doctor, thanks for your explanations of this great innovation at treating eye floaters. Are there any chances of these floaters recurring later even after treatment? How much does it cost and do you have branch office in the UK? A friend has floaters first in the left eye. seeing strands of floaters and dark ones with flashes of light which later resolved leaving only strands and dryness of eyes at night. He woke up recently to discover that the right eye has taken over with strands and a dark floater rolling about and flashes. Optometrists have see these and only pray they do not get worse. Caterracts also affect the right eye which is being referred to eye specialist hospital. My friend is almost 60 and believes there must be some food responsible for the floaters and slight detachment of the retina or something he uses like laser hair combes or even lack of drinking of enough water. Are there any relationships between these as according to him, his parents are older and none has ever experienced floaters. Many thanks.

It is pretty unusual to have eye pressure problems at your age, but regardless, I share your concern about eye pressure elevations as a risk, but it depends on the type of floater. Large, dense, cloudy floaters put the patient more at risk, but these are rare floaters for a young patient. What is the effect of the laser on the retina? Can you comment on the risks.
@Thefloaterdoctor13 жыл бұрын
The 'backstop' for the laser is not the retina. The energy is delivered in a cone-shaped' energy pattern so that the energy is delivered at one point in space, not along a linear collimated path. My responsibility is to know where that energy is delivered and keep the focal point of that energy away from the retina and lens. If that is done correctly, there is no effect on the retina.

@Thefloaterdoctor12 жыл бұрын
For a floater that is in good position in the eye, the laser treatment is a great option as it avoids the common and potential complications of an invasive vitrectomy surgery. How do you deal with potentially large floater(s) that could cause glaucoma? And, are you or will you in the future be using nanoparticles technology to make floater removal surgery safe?
@Thefloaterdoctor1 Жыл бұрын
1. Taking into consideration other associated risk factors, we can choose not to do any treatment (always an option), but I have had good results with doing much lighter/milder/less aggressive treatments initially to see how the eye responds by checking the pressure the next day. If all is well, continue with lighter treatments. This has mitigated risk significantly even though the overall risk is quite low. 2. Gold Nanoparticles is theoretical and investigational. It will require an injection into the ye prior to treatment and although it might make the current laser technology much more efficient, it would not necessarily allow us to treat microscopic floaters very close to the retina which is the main problem with floaters in younger patients.

It's hard to say with absolute certainty as there's so many variables involved. I would use your description of the movement as well as the definition of the shadow itself. And, if so, I guess this would mean that using the laser would definitely help since once you got rid of all or close to all of them, you would be finished with treatment? But what about when the vitreous detaches (PVD)...does that then create more floaters beyond the original count of collagen particles in the vitreous?

No they can not. The microscopic, thin floaters in younger people are usually located too close to the retina and there it is not safe to treat with the laser. Doctor I have to congratulate you, and commend you, because I'm a patient, ok I have eye floaters, and I been researching this treatment for so long telling the doctors that I want to do Victreolysis which is what you're doing, but they say that it is sci-fi or it doesn't exist or too risky, Everything you're telling us that they would say they do say... Thing is after looking for years I found someone that does the treatment here in Brazil which is my country, but I'm not 100% satisfied... the Doctor is nice very kind but, his method is way different than yours, probably because he doesn't treat eye floaters as much as you do? which comes to the fact that he is not as experienced as you are, and why I'm saying that? well he treats my floaters in a timespan of 5 minutes... I need each session to be around 15 minutes minimal and you know why? well because other than treating them too fast, which gives a minimal difference after the treatment, he also gives us a waiting time of 90 days between sessions, so that's too long for someone suffering with anxiety because of a single eye floater in special in my left eye, I have many floaters but I have one in special that is more focused than the others, and is probably the one that is closer to the eye lens, regardless he does treat every floater so I give props for him for that. Keep up the good work! We're happy that people like you exist, because for us that suffers from eye floaters no price is too high to get rid of these pests, so we can have the privilege of looking to the bright sky again. I only wish more people would adopt the victreolysis, because the other solution for eye floaters woudl be victretomy and no one wants to do that, is too risky compared to victreolysis. It fascinates me and brings me joy when you tell us that the bubbles that forms after shooting a floater aren't particles that are going to make more floaters, but simply bubbles that will dissolve into gas. Thank you so much for reading my insight about the subject.

There is no absolute, cookbook way of treating eye floaters with the laser. Those of us who do treat floaters frequently still have very different techniques and business models in the experience of the patients would be different. I do spend a lot of time with each patient and the typical treatment session may be 15 minutes to 45 minutes even. The number of shots of the laser could be a few hundred to several hundred individually aimed and fired shots of the laser I am very meticulous and doggedly determined with my treatments.
