As a nurse I worked on several dementia units. We had a new resident come in, very sweet, but in time she started having behaviors that we were having difficulty dealing with. Eventually her blood work showed she had Syphilis. This was so tragic because it meant 2 things- for it to eat away at her brain and cause her dementia, she had to have had it for many years. Number 2- she most likely got it from her husband ,and he never told her he had it. Men usually have symptoms. Of course there could have been other scenarios, but the family thought them unlikely. So tragic. My other experience with Syphillis was on Career day in High School. I had chosen to tour the State Hospital- our local mental institution- where one of the female patients was admitted with it. I wasn't allowed to know her story, but she was totally out of touch with reality and unable to care for herself. This is from having sex people, and not disclosing your indiscretions to your current, or future partner. It's tragic.

Retired nurse here. Years ago (60’s - ?). Every hospital patient had syphilis test as part of routine admission. Had a male patient about 10 yrs ago who was 101 yrs old. Tested positive - got a huge dose of penicillin in the nursing home.

I grew up in Alberta. I remember going to several remote towns for field trips and seeing community health warnings everywhere for Syphilis. Fort Macmurray, slave lake, high level….it blew my mind. Remote areas seem to be the most hard hit.

This infection is far from being a disease of the past, but it is on the rise in high-income countries and still endemic in low- and middle-income countries. If left untreated, syphilis can progress to affect a patient`s cardiovascular and central nervous systems, potentially leading to death. The most significant death toll due to syphilis, however, is due to the ability of its causative agent, the spirochete Treponema pallidum subsp. pallidum, to cross the placenta and infect the fetus. Congenital syphilis is the most common infection associated with fetal loss or stillbirth in low-income settings, with an estimated ~1.4 million pregnant women infected every year in the world, resulting in 305,000 cases of fetal loss or stillbirth, and 215,000 infants born prematurely and/or with clinical syphilis. In the US, syphilis still affects primarily men who have sex with men, but the recent increase in cases among women of reproductive age has resulted in 2,855 cases of congenital syphilis in 2021 versus the 334 reported in 2012. Lastly, there is evidence that symptomatic syphilis increases 2- to 5-fold the risk of HIV acquisition and transmission. A great sexual life is a must-have, but be safe everyone!

I have been wondering about the trends in STI transmission because even with the popularity of porn at an all-time high, it seems like sex education has become much less prevalent and that Americans are far more reluctant to talk about sex openly than in decades past. Which can lead to increased ignorance and more reckless behavior. In my day, asking a guy to get tested or wear a condom was a struggle, sometimes by the reaction you'd think you were asking to look over his bank statement. So I can't imagine what it's like now trying to practice safer sex.

Americans (especially the younger crowd) talk about sex a lot, but they don’t talk about safe sex or using protection. They only focus on the pleasure and lust aspect of sex, but not the serious consequences that can come with it. Normalizing hook-up culture has made this worse and it’s one of the reasons why we see higher rates of STD’s/STI’s.

I believe a significant percentage of Americans are wholly uneducated to the point of being dangerous about anything to do with the reproductive system of female or male, and about sexual development, pregnancy and sexual behaviour, because of insane Christian and other Abrahamic ultra conservative backwards nonsense, where they pretend if we don't don't about sex, no one will have it, except in marriage. What utter havoc that must cause. I hear about girls taking it up the arse thinking they are virgins because hey haven't let the penis into their precious vagina. I hear about kids being abused and not having the language to talk to someone about it. I hear about women not understanding how they even got pregnant. EDUCATE. The more you demystify sex, the less thrilling it is. Conservative culture kids are the worst for early sexual behaviour done in ignorance, because they have made sex so taboo and shameful it is deeply exciting, and their ignorance means they will do stuff and get knocked up or get diseases because contraception, barrier methods, the pill and Plan B are things they can't talk about, access or believe is sinful.

Love your videos I've been watching your content for awhile now. I just wanted to say my 35 yr old son was diagnosed with syphilis a year ago he's been through two rounds of heavy antibiotics and also had a spinal tap when the doctor did blood work and saw his numbers weren't coming down. Syphilis is not curable you will always have it. You can get your numbers down to safe place but will always test positive and always be a carrier. Wanted to let people know that. Thank you ❤

He went through two rounds of antibiotics and the doctor also had him go in for a spinal tap to make sure it hadn't traveled to his brain which it hadn't thank goodness. The doctor said that his numbers could just run a little higher than others and he's being doing fine since. Thank you for asking it's a terrible disease that could've easily been prevented.

@@xmx005 it’s so selfish! Even in couples who have an agreed upon “arrangement.” I know a couple where one partner wasn’t using protection 100% of the time and also had wayy more sexual partners. Now they both have herpes. Luckily, it’s type 1 so not as severe. This is why therapy needs to be normalized and easily accessible too, because that behavior was driven by underlying issues. However, they refused to seek help and just wanted to push it down and “power through.”

I remember seeing a patient many years ago with tertiary syphilis who had been raped by her father who had the disease and transmitted it to her as a child. To see what that disease had done to that woman’s brain impacted me so much. Thank you so much for all of your videos, but especially this one, as so many people are so cavalier about unprotected sex now. Now with such effective AIDS cocktails, there is far less awareness of the dangers of unprotected sex and the STI’s that can occur.

I used to see Tertiary Syphillis when I first started my nursing career in the early 1990's, we'd see a few women whose husbands brought home Syphillis in WWII and they all were not caught until it was too late. They were in the end stages of the spirochete infection destroying their neurological symptom.

I’m a nurse practitioner and work at a wound care clinic. I got a hospital consult for someone with “cellulitis” but turned out he had a bad rash that was also on his palms and feet. I included syphilis as part of my workup and sure enough that’s what it was! He was there for a whole other reason and just happened to be “lucky” enough to have a rash while there.

I was dating somebody last summer and we always use protection and he kept on going on and on about not wanting to use protection anymore and promised he was clean. I finally convinced him to go and get tested and lo and behold he had syphilis. I went and got tested as soon as I found out and even though the test came back negative I still asked to go on the treatment regimen just in case. Can't be too careful out there

A final note for the end of this wonderfully informative video... If you do ever contract syphilis and are successfully treated, you will from that moment on always test positive for it however it doesn't mean you still have the infection. As I understand it, it is measured in something called titres. Basically you're blood remembers that you have had the infection and when the titres are measured at a low level it means the infection is no longer present. However if it rises again it means you have been exposed to a new infection.

I have the impression a few years back HIV served as a driver to catalyse everybody's attention around safer sex practices and testing. Unfortunately in recent times HIV is not a focus anymore in the public debate and other infections such as syphilis - which as u say is a bacterial infection and not a virus - tend to go unnoticed.

@@pamelarodriguez5674 yes there are multiple reasons in fact that span from the absence of campaigns (and related budget), sexual education in schools, under financing of health services (in the countries where welfare provides for your health needs such as in my country)... also the way some concepts have been mainstreamed. See post exposure prophylaxis for example: an infected person with undetectable viral load is not infective in terms of the hit virus but could still be a vehicle of any other bacterial infection. It is a natural relapse from the years of terror in the 80s and 90s but at the same time this natural relapse should be managed properly while people have largely been abandoned to themselves or to some opinionated KZbinr with no medical background. Especially in countries like the USA where the costs of testing and therapy are super high this is quite dangerous.

@@themiasmastream I believe it is right and desirable to finally dismantle the association between sex and disease in terms of principles.. it's the way we are doing it that is a little sloppy: we are not going towards sexual education investing money in preventions, schooling, free healthcare for all but we are in fact regressing to pre HIV silence. Also the state invites people to test but at the same time, once you test and you happen to infect anyone, treats you like a serial killer on a killing spree while nobody ever questions the "victim" or the state itself that is ultimately - at least in most cases - the one that failed both parties not providing proper education and tools/free and equitable medical assistance etc etc. This is very contradictory and does not help in my opinion.

@@amandamccallum6796 I live in British Columbia, Canada and there is no precautions. Our little town has this pandemic right now, 2 of my friend's doctor's told her it is spreading. Stay single and wrap it or get them tested before you get into a relationship.

My ex went to the ER because he had a rash on his foot and they told him it was syphilis he jumped my butt when he got home thinking I gave him syphilis. I stopped and asked him, did they do any blood work or tests of any kind? No. Guess what?!? It was an allergic reaction to the laundry detergent!

I have a friend who used to work in public health and she said that syphillis is becoming antibiotic resistant. It is wise for you and your partner to get STD tests before sexual activity.

My friends son died of syphillus , TN , 15 years ago. He kept being misdiagnosed as schizophrenic and wouldn’t take the prescribed meds because they didn’t help his brain that was eaten away like Swiss cheese by the time he was seeking medical help. All mental health hospitals should test for this and hopefully prevent , or at least track and trace an infected persons partners and get them medical intervention. I didn’t know that syphyllis was still a problem in our country or anywhere . Thanks for educating everyone as most don’t realize it’s out there and can damage and kill you .

Great information here, thank you Dr. Dray. Also, I’ve been coming across quiet a few videos by tiktokers and influencers regarding “Sephora kids”. These are the preteen and younger children buying skincare and cosmetics at Sephora. When I was growing up my mother taught me about appropriate skincare at the appropriate age. She explained why some products could actually harm my skin if I used them on my little girl face and body. Could you make a video on this. Some of these kids are buying retinols, chemical exfoliates, etc… very little supervision from their parents. Basically they let the child pop whatever they want in their basket, hand over a card at the register and let these kids go to town. Some parents need to learn ignorance is NOT bliss when it comes to unhealthy skincare habits that are forming very early.

this is a great PSA. it’s sad to see so many of my peers engaging in such frequent unsafe sex, often out of feeling pressured. your health and wellbeing (both mental and physical) are nothing to play with. in addition, anyone who doesn’t want to practice safe sex clearly doesn’t value you or have your best interests in mind- so you should probably rethink letting them gamble with your health. it’s also important to get tested if you are sexually active even if you are in a relationship or married. many people believe testing is only for those engaging in high risk behaviors but unfortunately there are rising cases of women being infected with syphilis and hiv from their SOs. i wish everyone reading this good health and ability to advocate for your wellbeing and boundaries surrounding safe sex 💪🏼

FYI Neurosyphilis can happen at any stage, as well as syphilis of the eye. So def be aware of that, as I had it even tho was at secondary syphilis stage. Now treated and fully recovered

I never have had a textbook symptom or positive bloodwork for STDs. But for some reason I always have a suspicion and fear I might have syphillis. Considering this is in my algorithm feed it makes me think it knows something I don't. Or my roommates have it and the algos can't discern between our web activity.

I had it before. Only reaction was throat issues that felt like the flu, and an ear infection. I'm sure you can deduce how I got it in my throat. Treatment was 3 shots of bicillin over 3 weeks.

My college roommate found out his partner had cheated when he got an anonymous call from the Dept of health informing him that he had been exposed to syphilis. In NM it's still legally required to disclose your partners to the state when you're diagnosed with syphilis.

Guys are very lax about protection and most girls know this. So im always shocked when I hear someone say they had unprotected sex with a dating app match. Like 😳 you realize if they did that with you…they’ve prob done that with a bunch of other girls too??… (all my friends are straight women so that’s the only perspective I can speak on). But a lot of that behavior also stems from low self-esteem and being self-destructive.

“Safe sex” doesn’t necessarily prevent this and just because you have one sexual partner doesn’t mean you’re safe. The gold standard is either do not have sex or get screened periodically if you have a sexual partner.

It's probably also due to the fact that the only recent time that safe sex and condom use were encouraged were doing the AIDS crisis. Unless things are kept in the news constantly, people will slack off and think things are fine. This also used to be taught in schools so don't know if health/sex classes are a thing these days.

I was shocked with the numbers of new syphilis cases during my work in a health clinic. The treatment is cheap, the public power must do something to increase testing and treatment. I am in favour of active search with health care providers visiting people and providing guidance. Here in Brazil we have this for free but some areas are still underserved. If not cured in time, syphilis can be severely damaging for the brain

My mum works at a small gynecology practitioner in Southern Germany and more and more cases with Syphilis come in. It is mandatory to test for all the sexually transmitted diseases in Germany when you’re pregnant. They diagnosed a young women who just got married and must have gotten it from her husband. So it’s not just in the US, it’s a similar rise in Europe.

Back in 1982 my then husband had Gonorrhea. He had difficulty urinating and I sent him to the doctor. Turned out he had Gonorrhea! I was 8.months pregnant at the time and they tested me but thankfully I didn't have it. He tried to gaslight me into thinking I gave it to him! Dude! I'm 8 months pregnant with your baby! I divorced him and never remarried.
