In this video we demonstrate and discuss the importance of attempting manual detorsion of testicular torsions. Our hope is that this video encourages healthcare providers around the world to be more confident in their abilities and to understand the importance of quickly reducing testicular torsions before transferring for definitive care.

Was going to say the same thing. Most young men, would be to embarrassed to say anything, let alone to mom!! Also, thankful mom brought him into the ER & they had the equipment, staff, etc. that could make things better. Hopefully the surgery went well. Love Dr. M’s videos.

@fahad habib You would be shocked. When it comes to genitalia, pain, discomfort, and smells are embarrassing to males AND females, people sometimes wait too long to get help, and irreversible damage has already happened.

Glad the child didn't try to tough it out and felt comfortable enough to discuss a potentially embarrassing issue with his mother. Thanks to mom for recognizing her son had an emergency. Hope the procedure was a complete success!

A big shout out to Dr. Larry and the staff around him for the patient care they provide. I'm an RN at a Critical Access Hospital (CAH) E.D and a big fan of your videos. When you started talking about lack of ultrasound or immediate access to specialists, you really got my attention. In addition to limited transport to the higher level of care, we face those issues almost every shift. The fact that you addressed this is heartening because it is frustrating. Your work-around solutions are quite helpful. With respect and admiration to you and those around you!

When I was 13 I had the worst testicle pain in my life when I was gaming at 2 in the morning . Quickly searched on the internet for what it could be. Luckily, I clicked on the Wikipedia article for testicular tortion. It said, and still reads to this day “Non-surgical correction can sometimes be accomplished by manually rotating the testicle in the opposite direction (i.e., outward, towards the thigh); if this is initially unsuccessful, a forced manual rotation in the other direction may correct the problem.[7]: 149  The success rate of manual detorsion is not known with confidence.” So I tried it out and immediately got the biggest pain relief of my life. And my testicle is fine to this day 10 years later. Since it says the success rate is unknown, and has for 10+ years, is there anywhere I can even report this success story?

Poor kid. I can only imagine how painful this was. His mum did a good job bringing him to the hospital. I hope the operation went well and all is fixed now. For me as a lay person these videos are so interesting. The cases are about real people with problems anyone of us can get. I learned a lot by viewing them . Thank you and your patients for sharing this with us.

Great job to both the boy and the doctors who helped him. I have never heard of this before and when Dr. Larry explained it it made perfect sense. The RN who came in to get him ready for surgery needs a refresher on procedures that need to be sterile: putting in IVs, taking blood etc because you don’t go touching your hair or face with your gloves and then touch anything that goes in or on a patient because the littlest thing can transfer bacteria or virus to the patient especially with an open wound, newly closed surgical site, an port, IV, pic line which can cause a life threatening health situation for the patient. I have had it happen to me and I was lucky to survive it because the life threatening infection was a severe form of bacteria that wasn’t killed by high doses of radiation daily that the area it entered my body because the radiation killed my sweat glands and hair follicles in the same area. As the infection spread it started causing neurological symptoms, but I didn’t know what was normal or abnormal having never had radiation while still in recovery from 2 major spinal surgeries that had major complications that nearly killed when my airway was compromised and because of that oxygen wasn’t going to my brain and I suffered a brain injury. When the infection spread it went to my brain, blood, spinal/brain fluid, and bone and because it was in the brain it made my injury worse. The exact infection wasn’t diagnosed until my Neurosurgeon was doing major removal and replacement of the damaged titanium hardware used in my spinal fusion surgeries that became damaged when I started having blackouts (neurological symptoms) multiple times a day that caused me to fall unconscious to the floor. The infection caused an increase in spinal/brain fluid and combined with everything else any pressure on the back of my head and upper back caused my central nervous system to go haywire and actually caused more damage to it and increased my severe nerve damage even more. During the repair surgery my Neurosurgeon removed the excess spinal/brain fluid which looked clear, but when it was cultured in pathology they were shocked to discover the infection I had been living with for 11 months. It would take another 10 months to completely kill it. I had to get a pic line placed in my left arm because I was going to have to administer a IV type of custom made antibiotic along with a blood thinner and another medication on a strict schedule around the clock. I also had to take 2 high dose medications orally that helped the IV antibiotics to get down into the bone to kill any infection in the bone. My medications and supplies were delivered by the hospital weekly and a home health nurse came weekly to change my dressing on the pic line and draw blood for the bloodwork that monitored the amount of infection still in my blood/body along with making sure that my liver and kidneys were not being damaged among other things. If you don’t know what a pic line is, it’s a tiny tube that gets inserted into an artery usually in the arm and the tube is carefully threaded up into your heart. Because it is in the heart you have to warm up the medications ,which are kept in the refrigerator , and slowly inject them because if you don’t it can shock the heart and cause more health complications. I was very out of it when this was placed because I was on some major pain medication in addition to whatever they administered before I went down to have the procedure done , so I didn’t know that it went into my heart until the day it was removed. It was crazy watching her pulling it out because it kept coming for what seemed like forever. It was removed after 3.5 months, but I had to continue taking 2 oral medications for 6 more months. You don’t want a patient to get sepsis because you didn’t follow sterile procedures/protocols because it could kill them, cause them to have surgery to remove the infection, if the patient has a port or a central line, used in cancer patients or patients who have severe health issues that require IV medications while at home , and you don’t follow proper sterile protocols and they get an infection that is sepsis they have to have that port or line removed surgically as well as any infection they can see before they can put a new port in to replace the old one.

As a mother of two young boys I’m very grateful for this young man and his mother for agreeing to share this. I didn’t grow up with a male figure so I had no idea something like this could happen until discussions with mates over the past 20yrs. I have severe endometriosis (as well as other chronic pain conditions) so I can empathise that it would be excruciatingly painful.

I had TT in 1973 when I was in the third grade. After playing football in the park I told my parents about the awful pain. The Dr. did a doppler ultrasound, took me to surgery and anchored my testicle so they wouldn't twist again. The worst part about this whole incident was that I had to sit on the bench and couldn't participate in PE.

For my young boys and girls, if you are in pain in your 'private areas' even if it is embarrassing, tell a trusted adult. It could be something very serious such as cancer or some other threatening disease.

@@nicolaikirkwood1788not everyone has their parents in their lives. You also have a lot of people who only have one parent. If you only have one parent and that parent is the opposite gender you might not feel very comfortable telling that parent. Especially when that parent may or may not be comfortable dealing with problems that only the opposite gender fully grasps.

Props to you for saving this young man's prospects! Sadly for a 14 year old patient of mine, who had been afflicted with the condition for over week at the time of presentation to my OR, the urologist ended up having to conduct an orchiectomy.

My son was 17 woke up fine went to summer school and boom extreme pain. I took him to gp and she said go to the ER. They tried to manually fix but then They then sent him via EMS to another hospital for emergency surgery by urologist. So scarry and super painful. Dr stitched both down to prevent from reoccurring.

My 16 yr old son had a twisted testicle and needed emergency surgery!!! He ended up having them both stitched to their individual walls so it would never happen again! Pretty scary at the time!!! He’s 23 now and absolutely no problems at all!!!

@@lmellick that’s what the surgeon said too!!! Trouble was my son did it the night before playing football!! Came home and told me it was aching but no pain,until the morning!! I tried to get him up for his fist gcse exam but he was in so much pain he was grey! I got an urgent doctors appt with me and he was in the room 30 seconds before he came out with a letter for the hospital!! My doctor had called ahead to expect him. About 3 hrs later he was done,and the surgeon said he was actually lucky to not only save the testicle but his life!! I really hadn’t realised it could be so bad 😟

We were on the last day of a holiday, luckily in our own country, when my son aged 16, woke up in severe pain. Luckily near where we were staying I had noticed a local doctor and took my son there. He immediately got my son booked in to the nearest hospital which was about 12 miles away, and we went and he had the operation to untwist and stitch down the twisted testicle, and the one that wasn't twisted.

ER doctor here not sure how I ended up here BUT yes ! I have seen this plenty of times sadly... Must attempted manual detorsion specially when Urology is not close by or pt will need transfer.

It’s called testicular torsion, it happened to me my sophomore year in high school. Kind of a bummer because everybody on the football team knew what happened to me😂

On the subject, if swelling is severe, or ultrasound is inconclusive, a nuclear medicine testicular blood flow exam is a good choice. Be sure to ask your nuclear department to have an orderable testicular exam protocol. I have seen these and other exams come through in the past that do not have an active protocol available. It's not uncommon to hear the response, "We don't do those here." Ultimately, time is then wasted. Even though ultrasound is conclusive in the majority of these cases, have everything at your disposal. Never assume that you can easily order an emergency exam unless you know your facility has it on the books!

Awesome save and great ultrasound! I noticed the ultrasound tech was using color flow doppler. I would advocate for the EM doc using POCUS to use power doppler instead as this will lead to fewer false results. Power Doppler will detect lower flow states such as the testicle and thyroid. Once again, great work Larry!

I lost my testicle today because of negligence. I had the signs and was even diagnosed by a doctor, but an emt put doubt in my mind and a few hours ago, they performed surgery on my testicle and i had to have one removed. Testicular torsion sucks. Only 14 years old when it happened.

my son had torsion to his (L) testicle without a lot of pain, if you can believe that. after about 6 hrs and swelling making it uncomfortable he finally said something to me. unfortunately we weren't able to save his testicle. The Doctors were amazed that he had little to no pain. They stitched his (r) testicle in place so it wouldn't twist after they removed the (L) one.

I can't even imagine what that procedure costs but I'll bet it isn't Urgent Care cheap. Does medical Insurance cover that kind of thing? I'd bet it's not on the list of covered procedures.

Audrey, you can't tell by looking at it. You go with the most likely direction. If it doesn't return blood flow and relieve pain, you go the opposite direction.

@@lmellick I can turn a little bit the testicle to the left but once i turn it its in a horitzontal position, do I have to keep going? Im kind of scared it could make the situation worse

@@FatMonkeySexMonkey I just had my first that was rotated the "wrong" direction. I came to a hard stop as I was just making it worse and it was wound so tight that it had no more room to go. When I went the correct direction for this testicle (closing the book), it was much easier. However, I didn't go as much as I should have because I was still trying to figure out if I was going the wrong direction. Ultrasound helps to clarify if you have achieved blood flow. Even at surgery the testicle was 270 degrees, but was easy to unwind per the surgery resident. The testicle was still saved after two days of torsion. But I need to follow up and find out if it is still doing well.

Sonographer, quick question here. I noticed there were images labelled appendix then scrotum in the same imaging exam. Is this a POCUS or was the sonographer scanning? And if it was sonographer do you have an order you put this type of study under that covers both? They have always been separated into scrotal and abd ltd in places I have worked. Thanks.

Thanks for sharing this video, a lay person could attempt this maneuver easily by the description you gave and potentially prevent necrosis of the testicle. My son is 37 and suffers from Schizophrenia, he seems to have testicular torsion without severe pain, just left sided abdominal pain, nausea and vomiting for 2 days. This morning when he took a shower he noticed his left side was descended and painful. I'm a PT and could check his cremasteric reflex and attempt to derotate, but he's refusing care from me or the medical community. I'm fearful about what's going to happen in the near future if he doesn't consent. Is it possible that a testicle that appears lower than the other one is twisted? Are there other things that could cause the same symptoms?

There may be other causes, but it most commonly needs the anatomical defect that allows the testicle to swing freely like a bell clapper.

Seriously lmfao, i was breaking down about having to get surgery but i just got out of surgery for a minor torsion 2 hours ago and with my balls stitched to my sack to prevent it in the further and my sack sewn back together I’m still in less pain than when I was with the torsion

You go as far as transmuquous fentanil citrate for pain management ?!?! I know it has a fast onset and the pain is very sharp in general, but wouldnt 50/50% nitrous oxyde, oxygen work just fine ?

@@glioblastome the only problem with opioids is that pain isn’t being treated . We have people suffering and choosing not to get surgery they need because there will be no pain care after . The substance doesn’t cause addiction. Especially if you subscribe to addiction being a disease, the substance doesn’t cause a disease. Pain patients and people with disabilities are in crisis because of this misplaced war on opioids . Addiction is addressed by figuring out the underlying cause, undiagnosed mental illness, trauma, misdiagnosis, an autistic adult who is undiagnosed, pain not being treated, anxiety not being treated. These are reasons someone self medicates. People have more risk of self medicating with alcohol or drugs when they don’t get proper medication from their provider. Obviously the misplaced war on opioids didn’t fix the problem, it was made worse . The less than 2% of patients who misused opioids prescribed , weren’t dying of fentanyl overdoses. The problem got worse .

If you don’t remove it the dead testicle here’s something’s that can happen It can cause infection in the remaining testicle/sack Your are carrying deadweight And many bad stuff so if you’re testy dies it’s best removed

The nerve providing sensation of pain to the testicle travels along with the cord suspending the testicle. Once it twists the sensation to testicle can be lost eventually along with the pain like an extremity going to sleep. That is why patients can go home and sleep through the night at times with a dying testicle.

Is Manual detorsion as easy as it sounds ? I attempted it on the right Ball and it came at a stop on both directions ( it was in pain too and i have a history of torsion on it but it detorsed)

Not all the time. When it isn't easy, you need to do the Torsed Testicle Traction technique that I described. See similar video.

You don't know 100%. You go opposite the most common direction of torsion and if it seems to make things worse, you go the other direction.

Sorry to hear about your loss. Don't be depressed. Loss of a single testicle is easily compensated by the remaining testicle. You can still have children and there is no significant impact on hormone production.

if one of your testicles or both are horizontal then you have bell clapper, i have it in my left one and I just had the surgery to correct it and it wasn’t fun but def worth it

I’m pretty sure my testicle is dead, I told my Grandfather about it and asked him can I go to the doctor about it yesterday, he kept saying nothing was wrong and it was a sign of growing up. So, Today I feel no pain like that no more. When I get up and go to the bathroom and put pressure on it it hurts. Pretty sure I have one testicle. Also, I don’t know if Have Testicle Torison or not. Any Advice ? Also, I have some of the symptoms of Testicle Torison but I don’t have 3 of them, Which are Nasea, Vomiting or fever

@@joeglennaz well about that I told my parent about it and he basically said I pulled my groin because I was doing work and I was lifting heavy things so after the day my testicle was hurting it just stop hurting. I use the bathroom ok and everything seems fine.
