@@annikabecht5555 If I'm not mistaken, it's a joke about serving numbers in a deli. Often times, when you make an order, you get a number, and when your order is ready, they'll call your number.

​@annikabecht5555 in case you or someone else reading this didn't get it from the other explanation: on the surface, it sounds as if Seth is unsure how old the guy is. The second level of the joke is that in many delis or at the deli counter in a grocery store, people take paper numbers to represent who's next in line. Then, when the staff are ready to serve a specific number, they call out that number semicolon if no one answers after it's been called a couple of times, they will move on to the next number. So it takes the audience a moment to get the joke - that rather than asking whether the guy was 90 or 91 years old, the idea is that he was calling number 90 in the deli and then moved on to 91 when customer 90 didn't answer.

The joke is that the elderly were afraid their families would see the life-saving procedures THEY needed as too expensive to be worth it as well, and choose to “put them to sleep” instead like they would a sick pet
