Dr Rich, you are the only implant denture specialist who has even mentioned if a person has little money to possibly look into only two implants on the bottom. I have told this to a few of my friends about how the upper regular denture stays in pretty good with adhesive but the lower denture is a total pain for many. Many like myself just take it out to eat. You are the only one to ever mention just getting two implants on the bottom for a snap in over denture as an option. I I have read about snap on dentures and watched many dentists say that only two won’t work as well on here and they are full of crap!!!! People do not understand with regular snap on over dentures the ones with out the bar that the rest of the denture still rests on the gums. So a little adhesive and two implants would be much better than just a regular lower denture. This way a person could save thousands of dollars and still have a pretty decent set up. You are the only one kind enough to even mention it as an option. You have helped me a ton!!! With all of your information and you are a seriously good person. Many dentists on here act like they care and they do care just not enough for me. You are a good person Dr Rich!!!!!!

I have a full denture on the top and on the bottom I'm finishing up healing from have 2 implants placed to hold the bottom denture. I cant wait to have the bottom back in and be able to eat without having to take it out while eating. Thank you for your videos!!

I wish you were in VALDOSTA, GEORGIA. My 26 teeth pulled july 7, 2021.dentist wont consider all on 4 until 4 more months. Imeediate wear raw spots within 4 hours. Have the money to do procedures you show on KZbin. But cannot locate dentist to fix me...sometimes, I choke on a peanut butter and jelly sandwich. On meds for depression. I need snap in teeth so, so bad. Watched hundreds of videos, only yours explained what I need to get my life back. Still watching .very helpful.

Thank you Dr Rich. I am in the early stages of having a full upper denture fitted under the aged care plan which we have in Australia. I had to qualify in several areas to become eligible and have now been accepted,. The specialist explained several options but pointed out that at my age (76) I would be better off with a bar retained overdenture . One of the main reasons which I appreciated, was that considering my age and also future years, was that cleaning of all on 4 could become an issue with an ageing patient versus an easier option of a removable denture . Also considering the fact that I have worn a partial denture of 6 front teeth for many years, that the bone in my upper jaw would have receded over time making it troublesome to have implants at the front. I hope all this makes sense and I feel confident with her advice as all this is new to me and very difficult for me to make my own decision on something so important and unfamiliar me. Your program reinforced my confidence and your explanation of options was excellent. Oh, Hollywood is awaiting you. 😀

@@VeryNiceSmileDental Hi Dr Rich. 2 weeks ago I wrote to you regarding the beginning of my journey to have upper overdentures and you asked me to keep you up to date. Well, I went to the Royal Melbourne Dental Hospital last week and had scans taken and then ventured to the clinic where I was told that he did not have good news for me. As it turned out, my upper jawbone is not suitable to accept implants due to it being so thin. The specialist drew a diagram on his whiteboard showing what a normal jawbone looks like compared to mine. I have not mentioned this, but I suffer from Adult ADHD and I am 76 years of age. I have had upper dentures since December last year and I absolutely cannot bear them in my mouth, I continually gag and sometimes vomit mainly from the pallet aggravating me along the top of my mouth to the rear. To be honest, I was shattered to learn that I may face this situation for the rest of my life. It was mentioned that a bone graft would normally be considered in this situation but he was trying to say nicely that it would not be recommended at my age and it could sometimes be a risky option. So there we have it. Not good at all. I have asked for a second opinion at RMDH and that is currently being arranged but I hold little hope of a solution, In the meantime, I have been racking my brain for a solution and I am asking you for your opinion on the following possibility. Understanding that my jawbone would not accept a stable implant, would it not be possible to design an implant that actually saddles the jawbone and be secured by a screw that actually goes through the cross section of bone with a countersunk head to avoid bulging into the gum. Admittedly, the implant would most likely have to be custom made to suit the shape of the bone in each individual implant but it should give stability whereas a vertically mounted “screw in type” implant would probably smash the bone. What I am imagining is, e.g. drill a vertical hole in the bone, insert (not screw) the implant. The implant would have a peg going into the hole and a saddle over the bone, then screw and secure the implant crossways, Maybe I am dreaming but I ask your opinion. @@MrAuldphart Sorry to hear about your situation. I agree that you should get a second opinion to see if bone grafts and or sinus lifts are a possibility. Another possible option may be zygomatic implants. They are secured into your cheekbones. Here is the website of a dental surgeon in the UK where you can find some more information about zygomatic implants, surreydentalimplants.co.uk/zygomatic-implants/ The design you are describing sounds like a "subperiosteal" implant. They were used back in the 1960's and 70's but fell out of favor.

Hi, not very common in my experience. Do your best cleaning under the bars and around the implants. It would be a good idea to still see your dentist twice a year and have them clean under the bars and around the implants to remove any tartar the will buildup.

Hello DR Rich, I have the supported bar over dentures on top and on bottom. I just had my wax tryins the other day and had to have the bars tightened to my regular size implants. I wanted share that my bottom bar supported over denture is on 2 implants and has four locators on the bar itself. I have 4 implants on top and 2 implants on the bottom. I have bars for both and they are making my finals as we speak. I believe it will be another 4 weeks most likely. What do you think about a bar on the bottom attached to 2 full size implants and yet the bar on top has four locators? My dentist is excellent, and he said the dentures will also be supported by my gums as well.... Wondered what you thought of this set up? Do you think this protects the implants being attached to a bar better from getting more wear and tear on the implants themselves? Thank you Dr rich for all you do!

Hi! Bar overdentures are great. They hold the dentures in more securely. The bar itself holds up the denture along with the gums. As long as the lower bar is strong enough you should be fine. Nowadays the bars are milled out of titanium and are very strong. Years ago the bars were cast and not as strong, I had a few patients whose cast bars broke. Hi, locator overdentures would be less expensive, but if you need a bar, you need a bar. I would need to see you in person, otherwise all I can say is, "It depends." A regular denture without implants would be the least expensive option.

Hi, I'm not understanding your question. What do you mean by regular implant supported denture, are you talking about a snap in overdenture? An implant overdenture with a bar is implant supported, a snap in overdenture is supported by your gums and the implants only hold it into place.

Yes, absolutely. Think of the implants as legs of a table. The more spread out the legs are the more stable the table will be. Usually where the implants can be placed will be determined by your jawbone to an extent.

Hi, screw retained means that the bridge can be removed by a dentist in a dental office, but it can't be removed by the patient at home. Thanks for the informative video. I have a friend that is getting overdentures soon. Clearchoice wants $14K for Implant Retained overdentures and $26K Implant Supported overdentures with a "hybrid arch"...then it goes up from there - $48K for full upper and lower arch with a titanium bar...4 implants on top and 4 on bottom. Then $57K for the All on 4. I knew it would be expensive, but not THIS expensive. Does this sound right to you?

Hi, It sounds like you are getting all on 4 type of teeth. Those stay in, your dentist can remove them but you can't remove them at home. Overdentures are usually removable by the patient at home.

Its confused me as he says he doesn't do all on four or mini implants for upper but I still wouldn't be able to take out myself, apparently it would be overdenture but after the research I been doing I can't find what exactly this could be.....I don't have £2000 per screw anyway 😟 and I had to desperately hide my tears when he priced that up, is there any chance I could end up loosing too much bone on upper jaw for ordinary dentures that's my biggest fear as I was stupidly on my part was talked into letting a dentist remove the last 3 I had up top except for 1 right molar crown she wanted my last four lower front teeth too after snapping of my 2 lower canines, leaving the roots in thank God after research I cancelled her taking what are now the last 4 lower jaw with a snapped off root either side!! It started off my main worry was for the lower jaw when she was gonna remove all hibut the implant guy assured me that would be ok while I still have those 4 ( I'm even gonna cancel my root extractions just to save that bone area!! But on top I did have the right molar crown then left canine and 2 premolars b4 they were took by my new dentist, she said they were rotten just like the front four......the implant guy said front lower 4 are strong healthy just need a good clean up and polish so that's why we discussed the upper jaw implants instead of lower but when u don't got that kind of money even with savings it's so devastating I just don't ever want to become a denture cripple and I'm terrified as I'm only 36 now and been wearing partial dentures happily with no problemsfor 10 years but since latest extractions been left almost edentuous I'm struggling to fight bone and gum loss I now have learned about and to stay strong when the thought of becoming a denture cripple literally make me suicidal as selfish as that may sound to others who see this, I know the main issues is with lower but is it same with uppers that one day there could not be enough bone to hold dentures to....I hope not but want the truth if anyone awnsers 😁😬

@@chantellevidal7662 Wow, sorry to hear about your situation. In my experience, a lot of people do well with an upper full denture. Bone loss will happen after you have teeth pulled but how quickly it occurs is different for different people. For most people it takes years to lose enough bone that the denture won't stay in place.

Then told me to call immediately if the pain gets worse. He also said I would probably feel better this morning and I do but just still a very minor ache

Glad someone is actually honest, but how in the world is a single bar of metal $5000, and $500 for a "plastic cap". Not saying a dentist shouldn't be paid but let's be honest you know the right people that metal frame work probably only cost $500 or less with paying the people to make it and the "plastic cap" is probably all made the same size in a factory and you can get them for $5. And why is the "plastic cap not a metal cap". Why aren't the caps included in the implants? And why are you the only dentist that actually that will take the time to explain!

Hi again. The extra cost for the metal bar is not just for the bar itself, there is a lot more work involved. An implant level impression needs to be taken and the bar has to be made to fit passively on the implants. This usually involves a few visits. Also the overdenture will have a metal framework in it that fits tightly over the bar. Just the lab costs alone for this type of overdenture are several thousand dollars, not including the implants. The caps are made in a factory but are much more than $5 each. The caps are included with the cost of the locator (snap on) posts that are placed on the implants. The snap in inserts are plastic because they need to engage a small lip on the snap on post. They are a wear item and need to be replaced from time to time. It's less expensive to replace the plastic snap than it is to replace the snap in post. I hope I answered your questions. Let me know if you have any others or if you need more information. Do you have a metal bar in your mouth or just individual posts on the implants? Snaps on a bar tend to last longer. On individual posts, if the posts are parallel to each other, my patients tend to get a few years out of each set of snaps. The longevity will vary from person to person.
