Have there not been cases of the guard waving his green flag, or as I've read the 'ding-ding-go' bell, and the driver starting off even though the signal is against them?

You'd hope not! The Guard shouldn't despatch the train with the signal against them.

@@AnthonyDawsonHistory - they shouldn't, *but* . . . in many stations, due to platforms being curved, it's impossible for the guard to see the starting signal. In any case, the Rule Book was crystal clear; the responsibility for ensuring that the starting signal was off was that of the *driver* - not the guard. The guard checking the aspect of the signal was an additional check, and did not in any way relieve the driver of his responsibility in that respect.

@BPS&D - yes; that error has been the cause of a number of SPADs, some with fatal consequences, but the inquiries carried out by the Railway Inspectorate / HSE / RAIB always put the responsibility squarely where it belongs - on the driver. From the earliest days of signalling to the present day, there is no division of responsibility; all the rule books stae unequivocally that *the driver is responsible for checking the signal aspect before proceeding* - not the guard (whose role is only an additional check).
