Eh there is a fairly good chance Fugaku would put his foot down and point out the absurdity that they want a degenerate drunken gambler that left the village for years over him, which they would have no good counter argument for and likely have to give him the chance to make his case. Further Tsunade could still be brought to the village it just would include Shishui going along to get Tsunade to come back to heal Itachi, with Shisui obviously having the ability to solve her PTSD problem with his eye. The main difference is Naruto is more a tag along or doing it for Sasuke.

Shino couldve used his parasitic insects to eat away at Jirobo's Chakra consuming earth prison and escape much quicker. Choji still would have fought him and that wouldve stayed the same. Shinobu would have used the bugs to attack kidomaru from afar and avoid being hit by any arrows with insects throwing off the angles and such. Kiba wouldve fought sakon and ukon as is Canon. Shikamaru wouldve fought Tayuya. But Neji wouldve tried to fight Kimimaro instead of Naruto having to wait for Lee meaning Naruto couldve caught up to Sasuke much sooner and with more Chakra left. When Gaara appears to help Lee and Neji, either gaara wouldve been told to move on and help Naruto or Neji wouldve let Gaara stay and go himself. Therefore Sasuke would have been stuck in a 2 on 1 fight against Naruto and Neji or Gaara, most likely leading to Sasuke's retrieval instead of his escape.

Zetsu would probably manipulate Sasuke through bringing up Sakura's death and how Naruto's growth exceeds his by leaps and bounds. There should be a what if on what if Mikoto convinced Hiruzen the Uchiha adopt Naruto given she's Kushina's best friend. The only logical case would be Shisui stating it himself. And since it's one of the only 2 jutsus in the series stated to even have cooldown (second being Orochimaru's true form to change bodies) - other MS abilities aren't good example to compare.

Bro we could go at this all day, point blank Donzo should of been removed from the beginning, because it seems to me that he never wanted peace he truly wanted the uchia clan to rise up against the village, had the third hokage realized what was happening something could of been done. Removing Donzo would of solved alot of problems. Shisui and itachi would of still been alive as well as the clan, sasuke would of never had to go through what he went through, going down the path of hatred and also itachi would of been happy with izumi and not get sick because of the overuse of his sharringan.
