Thank you for this insightful video. Question: if MINOCA is caused by non-obstructive thrombus formation, might the pt have elevated d-dimer levels?

Yes it can be elevated in MINOCA.

My dad had an MI on March 20 2021,he had a stent placed in the lad and on November 5 ,2022 he again got some chest pain and got admitted and the cardiologist did the angiogram and told he has some minor blockage in one of the right artery and gave some medications telling the blockage would be cleared by medicine itself and has given the appointment on 21 November 2022 if again there's variations in the ECG then he told that a stent has to be placed there ,my question is ,is stent necessary or will the blockage reduce by itself for medications prescribed

I am not sure what sort of blockage is present in the right coronary artery, and the need for a stent depends on the severity and extent of the blockage.

As I haven't seen and examined your father, and as he is not my patient, I can only provide educational information. I can not comment on whether a specific treatment can, or can not be used. His physician who has seen and examined him would be the person to ask. Alternatively, you can seek a second opinion from a local cardiologist, but he/she will need to see your father first.

When I had my first heart attack in the spring of 2017, they found just one blockage and it was a 95% obstruction midway down in the LAD artery. During my angiogram, they examined all of my heart's blood vessels and found no evidence, whatsoever, of any other plaque (not even small amounts) in all of the arteries of my heart. After placing the stent (NSTEMI), I could tell that the cardiologist who did the operation was stunned as he'd never seen this kind of a presentation: it wasn't MINOCA and it wasn't your typical plaque-induced heart attack. I was told that when a person (particularly one over 60) has a plaque related heart attack, there's almost always plaque, here and there, all through the heart. I had only one ... just ONE ... piece of plaque in my heart. But it was a big one!

Yes, depending on the angiogram findings and other possible causes that leads to elevated troponin/ heart damage.

@@DoctorBoladCardiology if the angiogram shows no blockage, blood tests show elevated troponin levels and patient had a viral infection before the attack. Would it more likely be minoca or myocarditis?

Sir my father under go twice angiography one two year back but now it is confirm heart attack with trop i level 12000 But angiography reveals no blockage in arteries. But there is one small branch besides one of artery is blocked where stenting procedure also cant be done. Please reccomend what to do?

It is not uncommon for individuals with coronary artery disease to have small vessel disease, in which a stent can not be placed due to the small caliber size. This is best managed medically with cardioprotective medications. Your doctor who performed the angiogram would be the best person to advise you.

The cause of the leak can be due to temporary occlusion like a coronary spasm, plaque disruption or micorvascular disease which does not affect the large arteries of the heart. In addition, in some cases an etiology can not be found.

I'm 25 year old and recently suffered from minoca. My question is what could be the external cause of this occuring. For example diet, lifestyle, etc. Also, are statins necessary in patients with already normal levels of cholesterol.

At the age of 25, with normal cholesterol levels and normal coronaries, statins are not necessary. MINOCA can be due to multiple causes, and might not be related to coronary arteries.

@@ThePatil-z9t when I was taken to the emergency and after my angiography, I was discharged with the diagnosis muscular pain. After that I consulted another doctor who diagnosed me with minoca and started me on statins and blood thinners. I started experiencing few side effects and consulted another doctor. According to him I suffer from an episode of myocarditis and he stopped all my medications except 10mg inderal two times daily for my sometimes high heart rate. it's been almost 5 months since the incident, I kept increasing my walk time from 5mins Upto 2 hours without symptoms. Then I got back into the gym and am already back at my peak strength pre episode. Just have the odd palpitations here and there while resting (never during exercise) which the doctor has linked to anxiety.

Hi I have also experienced MINOCA las year at the age of 25 my coronaries were also normal but I had really bad Insect bite on neck and neck was swollen itching all over and then chest pain started. What your doctors said r you on meds for life? Did they tell you why that happened?

How to rule out Spasm.I am 28 I suffered same MINOCA before 8 months.I am not sure what exactly caused it so does the doctors.And I have no scars on heart muscles at all so how can the conclude it was a heart attack.My heart is all normal apart from Mild LVH.So did the doctors midiagnosed a Pulmonary Embolism for a heart attack?

Hai sir l have doubt my husband diagnosed minoca heart disease. He take all medicine same like heart attach medicine.he is all days morning time 40sec chest pain develops. Sir this any problem.pain morning time 40-1minute after reduced.

With Minoca, there is no obstruction of the coronary arteries. Has his coronary arteries been evaluated by cardiac cath. Sometimes, even though the coronary arteries are open, some people get chest pain similar to that with obstruction, and the medicines can be similar. His doctor who is looking after him would be the best person to advise him.
