Mad Max 2 - The stuntman's foot didn't 'hit debris'. The wrecked vehicle he collided with on his motorbike wasn't fastened down as firmly as it might have been. It lifted up with the impact, and he flew out of control, overshooting the landing pad. Nonetheless, he was back on set some days later, leg in a cast, tied to one of the other vehicles during the big chase scene climax. I know all this because he himself told me what happened. BTW, he wasn't driving a car. Did you watch the clip?

In "It's a Wonderful Life" Uncle Billy's drunken crash with the trash cans off screen wasn't scripted. Rather, a prop man knocked over something, which make the loud crashing noise. His response "I'm alright. I'm all right" was perfect, and was kept in the film.

Since the Monty Python’s Holy Grail started as a sketch about knights using coconuts instead of real horses, I find it hard to believe that they ever considered using real horses for the movie.

In the original Mad Max, a similar mishap sees the stuntman taking a motorcycle to the neck. That clearly wasn't intentional, and it must have hurt. Even so, the shot was good enough to make it into the opening montage of Mad Max 2: The Road Warrior.

According to the actor himself (the scene above is NOT from when it happened) he did not break character as the knife (which while having a blade was very dull) because it hit his clavicle and didn't go in very deep. He said if it went lower, and into meat, he would not have been able to stay in character. Bonus, Jane sent him a case of beer to apologize. LOL

My understanding is that Ford was supposed to use his iconic whip, but they couldn't get it to work because he was so sick, so they did the pistol shooting instead. The sickness could have caused the diarrhea, so that could be part of it.

Your subtitle "unscripted moments" should have been your title. You have a few great examples of accidents (e.g. Tora, Tora, Tora) but many of these are not accidents. Many, such as The Burbs, True Lies and Dark Knight are clearly not accidents. Just actors showing their skill at improvisation. You might stretch the definition of accident to include examples like in the Passenger, Ghost Busters and the Grinch as there is an "accident" but then the actors are improvising. Some such as the Monty Python crew using coconuts and DiCaprio eating bison liver have nothing to do with accidents, improvisation or even being unscripted. There are many other examples you could have used that were true accidents. DiCaprio breaking the glass in Django is a perfect example of an accident that was left in the scene.. If you want to stretch the concept, the whole scene with the Wampa on Hoth supposedly happened due to an accident but an accident off screen. Mark Hamill was injured in a car accident between movies and it's been reported that the Wampa scene at the start of the movie was added to explain why he had damage to his face. Or looked differently after facial work related to that accident.

Cow liver is a brighter more reddish color, and Bison liver is easily obtained from specialty butchers, especially out west, where farmed bison are very common.

The Aston Martin was a montage of 2 cars, the first attempt the car was pretty much pranged but didn't flip. The flying was from the first attempt and for real destroyed a $100K Panavision camera.

The narrator literally makes shit up based on his interpretation of context clues. Jim Carrey didn't ask for HIS line because he'd forgotten it, he asked the boy to redeliver his line so he could redo his reaction... Same with the mad max scene. His foot didn't hit some debris, his legs literally hit the whole car.
