Respected Sir, which is the draft of the form for applying for Associate Membership under Amendment 2019,where the Associate Member is NOT HOLDING ANY RIGHT, TITLE OR INTEREST IN THE PROPERTY. I believe the earlier Form 5 is applicable now.
