Totally agree with your presentation. After a GI performed colonoscopy on me, they diagnosed Grade IV hemorrhoids, and referred me to a proctologist for surgery. The Proctologist examined me using an anoscope and determined that I had Grade II roids that could be managed without surgery. Two years later, I had another flare up, and wanted the proctologist to do another anoscopy. Unfortunately, he had left the area, and we no longer have a proctologist. The GI, of course, now wants to do another colonoscopy, and refused to do anoscopy. It’s obvious that colonoscopies bring in much more money than the in-office anoscopy. So, I’ll probably drive 90 miles to visit a proctologist. Business people are running most doctor’s practices in our area, forcing private practitioners to merge or leave the area. Aggravating!

Great information, I had a similar situation where it was easier to get an appointment with a Gastroenterologist, but I waited for an opportunity with a colorectal surgeon. Since there will be follow up visits, it is worth getting the right diagnosis and treatment started with the best doctor. I also agree Gastroenterologist are easy to find, colorectal surgeons are not- at least where I live.

the thing that worries me is the possibility that some surgeons are blind to see and address the rootcause of illness. their immediate (and maybe only,) solution is surgery.

I wish I knew this you mention my experience I did not know where to go because I thought I had hemorrhoids, they had a list of what office did, my surprise was when I saw the surgeon he told me I needed a specialist in rectum, I got a little scared because why I need to see a surgeon, besides that I had 2 rectal exam now I am going to the 3rd one. I wish I had found your video before since my insurance doesn't require referrals

Hi doc. Can you please give your opinion on anal streching or dilation in theatre by a surgoen for anal fissures and hemorrhoids. Can you also perhaps explain why when i eat foods that i am sensitive to like dairy and gluten i have severe anal pain. I have kept a food diary and the pain only occurs with these foods and i am suspecting that, that is what is preventing my fissure from fully healing. Have an anal fissure now for 10 months.

I just went to a GI MD/PHd and she said I had a hemorrhoids and she recommended a colonoscopy since I hadn't done one and she wasn't sure. I find it very traumatizing to be put into "twilight" for such a procedure. Now I have to find someone to pick me up and drop me off. : (

I had the same experience when all the doctor wanted to talk about was colonoscopy which wasn’t going to solve the hemorrhoids so why on earth can’t they just be honest when you call to ask questions

Can a GI refer me to a proctologist?
@karend6491

GI doctors are useless if you need help with your colon. I've had colorectal cancer and hemorrhoids. The ONLY thing the GI doc has useful for is a colonoscopy. That was useful in finding the cancer very early stage 1 age 43). The GI docs gave me so much incorrect information about hemorrhoid care and colon health that it's astounding! I've seen 5 over the last 6 years. The last told me to get my internal hemorrhoids banded, and for the external, go to the ER! They do not want to talk about colon health. They want to do colonoscopies. That's it!

"GI doctors are limited and can only treat small hemorrhoids" - banding is perfectly reasonable and recommended up to and including grade 3 hemorrhoids. Only grade 4 or external absolutely require surgery. The vast majority of hemorrhoids are neither external nor grade 4. So to say GI doctors can only treat "small hemorrhoids" is misleading to say the least.

You're entitled to your own opinion. I'm biased and I openly admit it! But try telling that to the hundreds of patients that come to see me after seeing many GI doctors. You might be surprised in what you hear. Do what is right for you!

There's a difference between what is possible and what is optimal. If you have watched Dr. Chung's videos on hemorrhoid banding you would understand that larger hemorrhoids aren't treated as successfully with banding. A colorectal surgeon like Dr. Chung can assess the probability of a positive outcome based on the size and shape of the hemorrhoid better than a G.I., who has no experience or expertise in surgery. The G.I can only perform banding; what are the odds that they would recommend anything else? Dr. Chung has also admitted that there are surgeons who may not be as experienced and honest as he is and perform unnecessary surgeries, with poor outcomes. He has heard his patients' horror stories enough to caution them against following the advice of a G.I. or G.P.; I would trust him!

@@kathcares There are other office procedures GI can do like sclerotherapy, injection, infrared therapy (proctologists can do these as well). But the main idea is the same. These procedures cannot treat large hemorrhoids and the patients will end up being referred to a surgeon. Not trying to put GI or other medical doctors down. BUT! Everyone deserves a chance to hear more information and make a decision for themselves. We all have CHOICE!

@@yourfriendlyproctologist I needed Carpel Tunnel surgery on both my hands. The first doc I saw wanted to put me under general anesthesia and perform both surgeries simultaneously! Great for her, expensive and difficult for me. Possible, but definitely not optimal. The second doc performed the surgeries with a nerve block in his surgical center 3 months apart. My pain and inconvenience were minimal. And the cost was significantly lower. The other issue was that the first doc scheduled the procedure without my consent, and was upset when I refused! Both were qualified surgeons, but the second was a hand specialist. Big difference! And you would think that the hand specialist would have been more expensive.
Can I keep hemorrhoids away with exercise?
