There is a difference between just dressing people and designing a costume. The costume designer is the head of the costume department, and there is no bottom of the food chain. However, production assistants are considered an entry job in a film in order to gain experience.

Costume designer is a creative head, if you like to see like a "food chain" in a film the "bottom" would be production assistants
