I had an open inguinal hernia repair with mesh done 15 years ago and 2 years ago got the small fatty tissue recurrence. Had it "fixed" 3 months ago using the TAPP method with mesh again. Unfortunately surgeon screwed it up, just placed the mesh, saying he did not see anything else, and left (or did not repair at all) this fat tissue/lipoma in the inguinal canal and it's still there. Size of about 1-2 cm, defect ca. 1 cm, confirmed by 2 U/S and CT post surgery. Its been 3 moths, now I have burning pain in mons pubis and sometimes up along the gron to the oblique abdominal, also some pain above the whole mesh area (10x15cm). What would you recommend to do now? Another surgeon is proposing open revision, fixing the small hernia/lipoma and freeing-up the nerves. Shall I go for it? I'm from Poland, not many dedicated hernia specialist here, just general surgeons. I would wholeheartedly appreciate your view on it. Thank you!

I am sorry to hear of your problems following hernia surgery repair. As I have mentioned, I am unable to offer professional advice on this platform - period. However, I can discuss some concepts: 1. Do all inguinal hernias require repair? I find that the dynamic ultrasound often overcalls a hernia, the fatty lump type (abdominal lining fat protruding). I would not typically operate if patient has no symptoms and no obvious lump. Lastly, there is a natural fatty lump with the sperm tube (lipoma of the cord), this does not need an operation, however, during an inguinal hernias repair it is quite likely to be removed by the surgeon, because it may cause a bulge. 2. Do small inguinal hernias cause symptoms? These can, but typically it is dragging and burning pain during or after activity. Pelvic origin pain due to other causes is very common and needs to be explored. Hernias that do not cause symptoms in men can be observed safely, until they do cause symptoms. 3. Do pain symptoms settle over time after hernia repair? Yes, the great majority of post operative pain settles within 6 months. One has to ask what is the disability after six months? Is it mild, moderate or severe? Trying to localise origin and cause of pain is crucial and complex (as I have tried to explain in my video). 4. Is it possible to have an early recurrence, after an adequately placed mesh laparoscopically? Yes, the mesh may crinkle/ crease or move even if placed adequately , this is quite rare in the hands of experienced surgeons. 5. Should you attempt open re operation for post operative pain arising from laparoscopic repair for recurrence? The risk of losing blood supply to the testicle and other complications rise with each operation, the benefit is unpredictable at best, unless someone truly has a symptomatic inguinal hernia (which was inadequately treated). Performing a neurectomy (division of skin nerves) will cause a patch of numbness, BUT if the presumed nerve entrapment is at a deeper level, it may not address the pain element. As you can see, this is a complex area and trying to find answers is fraught with difficulties. Good luck!

Had tingling and sharp pain in testicles for 5 months, got inguinal hernia surgery bilaterally. 3 months later no tingling but still stuck with sharp intermittent pain in the testicles at 22 years old. Hardest thing I’ve been through.

I am so sorry to hear that. As I explained, patients with pre operative severe pain that does not fit the typical pain associated with hernia (dull ache, dragging and burning type pain, worse on activity) are at a high risk of post operative pain. I really hope you find respite - Good luck!

Thank you for this. It was so informative. My husband was diagnosed with this peri-hilar cancer late July '23. Once he had the stents put in, and the jaundice was relieved, we signed him out of hospital and refused Hospice. He's been home since and back at work a few months later. Alternative treatments have given him another chance. I personally don't think he's free of it, it's very aggressive but he's outlived his prognosis by far.

Can anyone tell me why opioids will affect the hida scan? Its a bacterial infection in the small intestine. It needs to be treated inside first. 1 teaspoon fg h202 in 8oz water with 1 teaspoon dmso. This was very useful. I have two large stones and unless I overeat I have never any symptoms. I had ultrasound and MRI and there was no obstruction or inflamation however dr arranged surgery and try to pressurise me. Otherwise I am healthy and no other issues. I refused and he got very angry and dismissed me. Horrible.

I'm starting to finally feel better and taking it slow with food. Each day getting a little stronger. Still have some pain where my liver is. Using Castor oil packs with heating pad, helps a lot. I heard about tsking high doses of D and bile salts. Thank you for taking the time to write me. I hope your doing good with your surgery. It's not been an easy one for me.
