Great video Doc, as you always do. Your point on Ambetter / Centene is correct - high penetration related to the crossover of some moving from safety net to individual health when they grow earnings. However, that company recently made a mistake in cutting out broker assistance/compensation for their products. As you state, their networks are VERY different than other traditional group insurance insurers (BUCA, Oscar, HMOs) and there is now noone to compare the options for a value, need-based decision - only the stress when someone finds out they chose the wrong product. I hope Ambetter / Centene changes their mind, as they do play a valuable role for a segment of the population - but not for most.

This is a mind-blowing knowledge that was delivered in an exceptionally effective way. I have never met anyone like you who would explain utilization management so clearly and in just 15 minutes. I am also impressed by critical thinking and fresh perspectives that you bring into each video. Seems like that Prior Authorization and Institutional Notification became a burden to efficient patient care. How can we make more competition towards the the InterQual or MCG Care Guidelines? What are alternative options?

Thank you for sharing this video. Definitely going to read Metabolical and try to build on idea of Dr. Lustig. You should also make a video about bioengineered food, GMO and expensive access to organic food. When I came to US I gained 30 pounds in three month. Everything was way too sweet and addictive. It took me 6 month of workouts and healthy lifestyle to go back to my normal weight. American food is dangerous because it is really intended to make you sick for the profit. We should be accountable for our choices and bring dramatic positive change into food industry.

This was such a beautiful eulogy Dr. Bricker, thank you for sharing your stories and love for this man. I am in healthcare but he also helped me tremendously when I received a super inflated bill from the ER. There was no daylight between my ask and his answer, he loaded me up with resources and tools to ask the right questions--all information I've subsequently shared with my own patients. I am one of the hundreds he helped so much and it will keep rippling long into the future! May you find comfort among those who loved him too.

Thank you for recognizing Marshall Allen's work, especially with his graduate students, teaching them investigative reporting skills. There are so many families with preventable adverse events, and Marshall helped many of us to understand why we wouldn't be successful finding attorneys, getting waived bills, or compensation. Most importantly, he also made some of us feel like our families mattered. My husband and I met one of Marshall's grad students who reviewed our child's medical records after a preventable adverse event. While our child didn't seem to matter much within our own state, it felt like our child mattered to Marshall and his grad student. I know many others who have had similar experiences with Marshall. Everyone appeared to respect Marshall, whether they were religious or not. He will be missed. He empowered many families harmed. Because of Marshall, I understand deny and defend, code of silence, lies of omission, billing "errors" and so much more. I was glad to make a donation to his family. You did a good job with this video. I am sorry that you lost your friend, but I am glad that you shared more about him than I had known before.

I grew up with Marshall's wife, Sonja, in Kenya! Love this family so much! Marshall was an amazing spirit gone far too soon. Love to Sonja, their kids, and the rest of the family!

I understand your views on VBC being not a great model and why our healthcare ecosystem is in part dysfunctional because of extreme difference of position between cost containment for payers and revenue pressures for the provider. With VBC not being the best solution, I wonder what that says to the 50M members who are enrolled in a MA plan.

Patients should understand and be educated that essentially if you thought your doctor, did a good job you should be giving them five because anything less than five is essentially saying your doctor is awful. But honestly, the real issue here is that press Gainey, and therefore hospitals and practice managers only care about the percentile rankings and win the vast majority of doctors are doing a fairly reasonable good job and their patients are happy with them. They are choosing to essentially grade you with an F when 90% of your patience say you did a great job. Using percentiles when the standard of care is so high is completely crazy and causes lots of administrative intervention and harassment of physicians, even when they are doing an admirable job with their patients.

Insurance companies purposely don’t enter into reasonable contracts with physicians because from their standpoint, they don’t care about the patients out-of-pocket costs, and the less they can pay for the better. Also, much of the out of network billing/surprise billing was coming from private equity owned physician practices where the doctors are doing the work of doctoring but have no idea what the company is billing under their name or how. This has been used to unfairly, smear physicians, and specialties that have been forced into employment by hospital systems and private equity owned so-called “physician staffing companies“ like EMCare, envision, teamhealth, etc. To be clear that increased out of network billing revenue, never makes it to a doctor and turns of their reimbursement or pay, and they likely have no idea that it was even happening.

This video deserves an update about that background process. The arbitration process seem to be working fairly well under the trump administration under the Biden administration. They decided to dramatically increase the cost to physician practices to enter into arbitration. In many cases, the difference between the doctor is asking to be reimbursed, and what the insurance company is willing to pay is less than the cost of the arbitration so essentially under the Biden administration, the insurance companies have been given all of the negotiating power. Essentially the Biden administration is forcing doctors to just eat whatever scraps the insurance company feels like giving them.

I recently began my search in AL for a new PCP. When I needed specialty/sub- specialty care, I was steered to a medical campus 2 hours from my home. The area is known to have extremely heavy traffic all day, every day! I have not been able to get a clear understanding of why all the specialists are leaving a particular hospital, which is a mere 3 blocks from my front door! I would not have given this change a second thought while in my 20s. Now, in my 60s, I appreciate having my medical care providers within a 1 hour drive of my residence. I commend you for taking the time to thoroughly research your topics. Thank you for presenting the topics in a manner that is easy to comprehend. No fancy slides or colorful graphs. A simple whiteboard and felt tip marker are all that is required for a successful presentation. Thank you.

Huge fan of your content! It’s helped me deeply understand the healthcare industry in all aspects.
