Just meticulous dissection and sharp dissection with cautery Some people operate under hypotensive anesthesia but I dont ask for one

All are good when performed well. Extraperitoneal approaches have slightly less ileus compared to TAPP

@@Gayrat-Abdiev Thank you for the feedback. Here we have used a 15 x 12 cm medium weight polypropylene mesh. We routinely perform e-TEP in all Inguinal hernias nowadays except in strangulated hernias and very large irreducible ones, recurrent hernias after lap repair, previous lower abdominal scar then we prefer to perform a TAPP in these patients. We perform TAPP in teaching programs as it helps teach anatomy more objectively in a plane the surgeons are used to

Operated on a case of left lateral wall fibroid sir of size 4x5 cm. Difficulty which I faced was during division of the vault. I could not easily do the posterior division the uterus started rotating to the heavier side. Any tips for that sir ?

Myoma spiral for manipulation helps. Can leave the IP ligament till the end to avoid rotation. If exposure is an issue you can remove the fibroid upfront and proceed
