In 2019 I was diagnosed with endometriosis after 20 years of pain. The pills disguised my pain, but every time I didn't want to take it anymore, the pain was awful. I remember once my parents took me to the ER and the doctor practically laughed at me, saying that was normal and part of being a woman, I remember so clearly, I was 19 years old. In 2018 the pain got worse, adding vomit and diarrhea, almost an entering night the very first day of my period every month. I think I've seen so many gynecologists and all said the same, I have dysmenorrhea, deal with it. Then a kind of keloid appears in my belly button (2015). A dermatologist removed it, but it appeared again with a black fluid after a few months. I've tried to get pregnant since 2017 and nothing has happened, then in 2018 I went to an oncologist for the "keloid" and he just asked me some questions and without checking me, he found out that I had endometriosis. I had a laparoscopy and confirmed,, severe endometriosis and I'm not able to have a baby because of that. I've trying to find the causes and nobody knows. I'm from Chile and there's so little information about that, just in the capital city there's an endometriosis specialist, but in the city where I live, there's no one. We have a foundation, that gives some advice of the condition, but doesn't exist like a record of the quantity of women who suffer from this. They just have like a statistic (3 of 5 women have it). So, without that, it's hard to research about the root cause of this.

Thank you for providing this information! Are ultrasounds always very accurate? I recently had a vaginal ultrasound and my uterus was measured at 3.8 cm (longest measurement) which seems very small compared to what is normal!

Actually.. what about studying the male body? If a couple con't conceive due to male factor which is 50% of the infertility cases, the woman will be sent to IVF, pumped full of hormons and get her ovaries ripped apart. Assisted reproduction techiques put all the burden on women, and so the male problem becomes a female health issuue.

My fertility appointment is in a week and a half after waiting since February! This is so encouraging. Through my job, fertility care is handled through a different insurance branch, and they called and said basically a dumbed down version of what you shared here in regards to 3 iuis and then try IVF. Just nice to know the right information is being shared. Thanks for your education!

Is the last test she mentioned AMH? Or something else? My doc tested a bunch of hormones and not sure if it included the one she mentioned. My doc did reccomend the felopean tube check and sperm check too, so will give them a go.

In October 2023 I fond out I was pregnant - unexpectedly. We had given up after trying scine 2019. Kind of just thought "Okay, it's not going to happen." So we where over the moon excited. Sadly, I lost my precious baby boy in December 2023 at 13 weeks and 5 days. I had a prefect ultrasound just 6 days before. He was sucking his thumb, moving around and had a strong heart beat. The DNA testing I had done came back fine. He was prefect. The doctors don't know why it happened. I asked if it was stress as we learnt we had to be out of our apartment shortly after I fond out I was pregnant. As they planned to fully renovate the building. So we where in the progress of buying our first home when it happened. They said no. So here I am, turning 40 in 3 days and I've been trying to get pregnant again scine January.

Thank you for this daughter. It was amazing podcast episode. I’m 38. I just finished my first round of IVF stimulating medication’s to try and get ready for a frozen MBO transfer in October. Unfortunately, we only got one little embryo that could be biopsy and frozen. it was a miracle. We got her because at seven of them, but they five nobody was developed enough. She finished on day six but that was the only one that made it. I feel like it could be my air quality is not just my age, but I wonder if I’ve always had poor quality and then my husband is also Subtle he has motility and more problems we did do you know and I meant we just absolutely cannot afford donors, sperm or egg donors so really I’m willing to give it one more shot and all I can do is pray to God that this embryo is viable and can become a healthy baby. I want to do another round immediately, but I think I have to wait to see my doctor in another week or so, is it worth continue coq? Is it too late to start. I’ve taken on and off for 2 yrs and was just exhausted taken 15 supplements a day. I’m post bariatric so I had 5 simple supplements there plus fertility. Could get tad daunting sometimes but so worth it. After 200lb weight loss in healthy and good bmi 27 bmi, and we are desperate to start our family after 20 yrs constantly getting turned away and looked over. 😢 we just want a little bundle of babe of our own. Infertility is aweful

Thank you Dr for this insight. Truly the best advice ever given by an RE. That happen to me with femera attempts in 2015 IUI in 2017. Finally got a dr to give me a HsG after 10 yrs and I had blocked right tube. She couldn’t figure out why or how it was blocked never mentioned scaring of any type no endo no nothing I’m just 100% unexplained infertility past 20 yrs 😢 hubby is sub fertile as well good numbers below optimal motility and morphology. Finally health wise 200 lbs down I qualify for IVF and we have the money and coverage but it’s been a long horrible 15 year strugle

Do you know the percentage of incarcerated uterus resulting from retroverted uteri? Also, you mentioned there is no evidence that retroverted uteri can cause fertility issues. I'm having trouble finding such studies, can you link to them? Also, I read on pg 12 of the Obstetrics and Gynecology 6th Edition text book that “The retroverted, retroflexed uterus has three particular clinical associations: (1) it is especially difficult to estimate gestational age by bimanual examination, (2) it is associated with dyspareunia and dysmenorrhea, and (3) its position behind and below the sacral promontory may lead to the obstetric complication of uterine inculcation” (p. 12). “Estimation of gestational age in the late part of the first trimester may be difficult when the uterus is the RVRF or RV positions. The risk of uterine perforation during procedures such as dilation & curettage or insertion of an intrauterine device is increased in a woman with a retroflexed or anteflexed uterus. Applying traction on the cervix to pull the uterine canal into a straight line can greatly reduce this risk” (p. 37). There’s also a rare but serious condition called incarcerated uterus. Uterine incarceration is a complication whereby a growing retroverted uterus becomes wedged into the pelvis after the first trimester of pregnancy causing dyspareunia or urinary outflow obstruction. “Women presenting with urinary retention in the second trimester should have a pelvic examination performed to exclude uterine incarceration.” Then there is the issue of retroversion leading to prolapse...“Recent data demonstrate that the prevalence of grade 2-4 uterine prolapse for a retroverted uterus is 4.5 times that for an anteverted uterus. Alternatively, 69% grade 2-4 uterine prolapse involves the retroverted uterus.” source (9) The retroverted uterus: ignored to date but core to prolapse.

Among all the videos I've watched this is the most informative! Thank you, Doctor Lora. However, they usually don't tell how long you wait to pass out. Usually, it's on HOW you're going pass it out, HOW you grieve, HOW you deal with it, it's helpful but they don't talk about the anxiety of waiting, the timeline etc., and really just seeing a relatable picture of how it is to really experience a missed miscarriage. Had a missed miscarriage two months ago and it was unexpected. 1. There were no symptoms, just having the gut feel of "I think my baby is not here anymore" but you still physically feel all pregnancy symptoms. 2. With all the pregnancy symptoms, my stomach is also growing despite my sac being empty. 3. I found out through my weekly checkups with my doctor through ultrasound. She did not make the final call until I was diagnosed by another doctor, she made sure that I was checked thoroughly before making the final call. (I went to 3 local OB-GYNE and had an ultrasound for confirmation that the pregnancy was no longer viable) --- note that before the sac being empty, there was growth inside the sac, it was not empty, but what could've been the baby didn't progress and it kind of melted? or reabsorbed? hence the term "anembryonic pregnancy" coz there's no longer an embryo inside. --- doc also said, that it was because of chromosomal abnormalities meaning, the pregnancy could not progress because the embryo did not have enough nutrients or is not fit to grow. What happened after knowing... 1. After making sure that my gestational sac was already empty, she gave me meds that will help with contractions so that I'll start the process of passing it out. 2. She also recommended that I took rest, so my body can prepare for the delivery, and recuperate easily afterwards. 3. We were sad of course, we spent almost a week of just mourning about our baby. We had friends who sent us food because we can't function normally, we were definitely not okay. How was the passing out... Painful. It was not sharp pain, but gradual ascending pain. The cramps we normally feel as we pass out a blood clot during a period times 10 or 20. I can't stand or sit down because it feels uncomfortable. The most comfortable position is either squatting or child's pose (yoga). Timeline: *I started taking meds for a week, then had a checkup after, when the doctor saw that the sac was not budging, she gave me a week more of meds. *Second week of meds done and had my checkup, and the sac again was not budging. So, she recommended another week of meds again. She also advised that I stop taking the folic acid but continue the vitamins that'll aid my blood. *On the third week I was starting to get paranoid because there were moms stating that I can get an infection if my sac won't be taken out. Others would also comment that I should get my uterus massaged, because that'll aid the process. However, upon my checkup with another doctor, she debunked all these myths, and she stated that the process I'm going through is normal and that I have nothing to worry about but wait. So, she added my meds with evening primrose oil capsule to be a vag suppository along with the contraction meds for the week. *4th week I came back to have it a checkup again, and the doc said, my cervx is softening already so I just have to wait. And right at the end of my 4th week, I finished my meds and I experienced unusual, weird cramps, the pain was tolerable, but it felt like your energy is being drained by it. After that cramp, I spotted. Then had small spots all throughout the week. *Next Sunday came, and that's when I started having contractions. It was really as if I was giving birth, the pain was increasing, from a tolerable pain to something like I don't want to do this again kind of pain. -------It started with subtle cramps at first, then a blood clot after, then it would cramp and a blood clot comes out again and again, it happened a lot of times along with blood flowing. Then after 3 hours, there's these very big cramps that it took 5 episodes before it was over. I was tired and sleepy already because it started at 10pm and around 2-ish I was already sitting on the bathroom floor resting my back against the wall, and by the time that the pain was gone, and I surrendered already, that's when the sac came out. After the ordeal, I washed up, to clean my sweats off, wore an adult diaper, and jumped on my bed to sleep. It was the best sleep of my life! ------We saved it, so to give a proper burial, and to show it to my doc, to see if I was missing something. Also, she recommended to have it checked in the laboratory to identify the cause. *After little blob came out, we had our checkup after two days, coz I had to rest my body. I was sore in the under area a little bit. There's no pain, but there's that little soreness that comes with it. The doc advised another week of meds just to clear my uterus out because there were still some tissues that can be seen in the ultrasound. I was heavily bleeding a bit. *After a week, went back to the doc, and had a clearance from the doc. She then gave me probiotic capsules to take for the next 30 days to restore the flora and good bacteria below. Also, she advised that the next attempt for pregnancy will be after 6months, so that my body can properly heal, and I'll be able to carry and give birth naturally. Contact with the husband is advised 6weeks after bleeding. The intensity of my bleeding has decreased. As of this moment, I'm still having a few spots, but they're no longer potent blood. It's pinkish mixed with white fluid, just like the initial spotting. Over all, from the knowledge of having a missed miscarriage, it took a month of waiting til it passes out, two weeks of manageable bleeding (period like) and another week of light spotting. I am hopeful for the next pregnancy, because now I think I'm ready. This first one, we didn't even have vitamins on us, and we were eating somewhat unhealthy foods. I'm not even on folic acid. So yea, I hope this helps to all the mothers, who experienced a missed miscarriage. :D

Thank you so much for the information. My husband and I have 4 kids currently and we miscarried twice in a two year span. The last miscarriage was with twins. We were devastated, I was so emotional and asked my husband to please get a vasectomy. He got the procedure and kept telling our friends he was going in for a surgery lol. We now think that we should have waited until we were not in such a bad headspace because now we are seeking advice on reversing the procedure. He is 46 now and I am 36 so I hope it is not too late for us.

For someone on cyclical HRT,should they delay their progesterone until they’ve ovulated? Typically, the instruction is to take it days 1-12 of the month, but what if someone has a longer cycle?

I was a less than 3% possible for my parents. They went through 13 years of fertility before the impossible became possible. With God all things are possible. I’m a living miracle, my mom had a 8.5 cm cyst on her ovary while I was in utero. If not removed or not found sooner it would’ve erupted and killed me, they surgically removed it when I was 5 months gestation. Took her uterus out and placed it on a surgical table to reach the cyst. And then I came a week late to keep em waiting.

Hey on the day where you are fertile make love at least twice, morning and evening, because there is like a 12 hour window, that you wouldn't want to miss. Many will know and be disappointed that it didn't work YET, but some might not know, ... ❤

I take Coq10. Ubichinol 100 mg daily. The question remains - should that be continued when pregnant ? My doctor says that has not been tested, so to stop in case of pregnancy in other words.

It can still happen. We got pregnant with our son (and he stuck) after 9 years of trying. We're currently trying for baby number 2 but I don't think it will happen and I'm probably in perimenopause.

I know girl, I have been trying to get pregnant going on seven years. I have regular periods every month and everything. It's secondary infertility for me, but my husband doesn't have a child of his own, so it sucks for him especially. I just turned forty, as well, so... Also, I can't adopt because I'm a felon, so there's that. Hard to see so many people around you just throw around their ability to get pregnant and they are ungrateful sometimes, too. Not to mention the ones that abuse and neglect their six kids.... 😡 I wish you good luck and blessings.
