Very educational and you may have just saved me a lot of heartache and health issues, I was set to have all on 4 top and bottom next week and you have I changed my whole outlook on all on 4, that has been my fear and exactly why I have put the surgery off for the past 8 mo, I knew there was more information out there on the long term outcome of these all on 4, they are pushing these surgeries too hard and you have answered my questions that I have been searching for, my gut feeling was telling me don’t do it, your knowledge and honesty is so appreciated ❤️

Holy cow this was informative!! I about died when I saw the sawed off teeth and bone! I'd rather die than have that done!! I had three implants done in Mexico but it wasn't a quick fix. It was a two visit ordeal over about 7 months. I am happy with the work they did so far. I wasn't too thrilled with the corporate fools I saw in the States. I think they did things to my teeth that wasn't needed just to make money. I don't mind paying for an honest dentist regardless of where they are. You are honest and the world needs a lot more people like you. This was well done!

Before a surgeon cuts the bone away, the patient not only should be informed but also told the ramifications. I had no idea this was part of the all on 4 process until it was too late. I'm devastated because the bulkiness is absolutely awful and I did not know about bridges until afterwards. I am miserable.

Fabulous presentation….I’ve been reviewing all the major dentists and their protocol for years….around the World. Must admit yours is superior. Although many AO4/6 can look attractive, eventually they realize their going to be living with a hockey puck the remainder of their years…could be tough…

Hi Dr I want to thank you for educating us on this matter, I am recently working in an all on four dental lab in Dallas Texas and I think about a year ago I work in a lower all on four for a Dr McFadden, I don't know if it was yours but if it was I hope everything went well with it, I work on a lot of cases and I try my best to do what I can in the circumstances, I also say a prayer for every patient thanks again, this video remains me that the work I do is for an other human and I have to do my very best.

Try as I have, I have not yet found another dentist in that area who follows the same protocol as we do. But with the information you glean from many of my videos, hopefully, you can interview some prosthodontists and know which questions to ask and be sure to ask to see lips retracted photos of their work. I wish I had not found this information, NOT!!! I’m in the first year of the A on 4 process , and not very happy, this practice got my lower arch, wrong, because it loosens up, are. As well as the most front post is painful so I can forget eating corn on the cob or a apple. So as you can imagine, I’m the fish, hamburger, chili, or all the other things that we tend to eat with teeth problems. So tomorrow I’m calling the office to put them on notice that we need to fix this. FYI , my first two temps on the bottom broke, and I’m thinking that they should have taken the hint that something was wrong with the bottom.

Been feeling so bad😥😢 abt dreading & worrying how im guna get the money, or even in possible debt 2 get the All-On-4 or similar to, i heard even regular teeth implants cld be bad, & tht was 1 of t/ main reasons i wanted them 2 stop bone reduction but they have to remove too much of it just 2 get t/ All-On-4 procedure. 4get that. THANK U SO MUCH 4 UR WORK AND INFO. Im fine with the healthy bone & mouth i do have and even dentures 4 now. Now i kno where 2 go, and if anything snap-on wld be the best and still give me the option 2 convert later on if i decide i may wana go thru with it. Not all docs r gud thank u 4 being 1 that is👍💯

Thank you for all the information. Since these are bridges, what is the rationale for having the entire arch be one piece rather than placing several teeth at a time across the implants? Also, do you have a referral service for people who are very far away from your practice? I have searched near me and every provider offers all-on-4. Edit: Also what do you even call this in terms of being able to Google locations? When I search full mouth dental implant bridges, all the results discuss all-on-four and might mention traditional bridges as well. It's very difficult to search for because of the overlapping terms (traditional bridges on crowned natural teeth) and because all-on-4 is so much more common. Edit 2: I hope I don't sound ungrateful in asking about other providers--I just saw on your site you all cater to out-of-towners. I am disabled and truly mostly bedbound, not able to fly, and have many, many chronic health issues besides my dentition, so finding something local is more realistic--although I'm in a stressful situation where after living with my caregivers for the last 20 years am moving cross country (not by choice)--so what is local for me will be changing soon--I know I said traveling is difficult which sounds at odds with moving cross country, but I honestly don't even know if I will make this cross country car trip (I'll be on a mattress in the back of a van with oxygen) and it will be the most herculean thing I've had to do. I've searched for providers both where I am now and where I'm moving to and don't see or haven't been ablet to find providers offering what you do. EDIT 3: Searching more the closest I've been able to find to what you offer elsewhere is "3 on 6" --3 individual bridges on 6 implants--it has no artificial gum and uses the natural gum. What do you think of that option?

Thank you ! We almost always do one bridge per arch. Only when we can get eight implants in do we do separate bridges. I will tell you that I have gone back and forth on one piece versus multiple pieces. In 2023, 95% of our treatments were a one piece bridge. Not everyone has enough bone for eight implants. Also, I used to say, they are easier to clean, which is true, but only in two spots where the three bridges meet. Finally, if an implant should fail on a one piece bridge we can often take that implant out and modify the bridge without adding implants or months of treatment. I hope this helps. Sadly, there are so few dentists who do this treatment, we can't find them either. We could do a Zoom call if you have a panoramic x-ray. All the best!!

All the cases you have shown have excellent bone condition it's easy to do :implant bridge' in those cases but patients with bad bone and uneven bone you'll have to make space for an fp3 prosthesis.. in good bone cases you are absolutely right.. but majority of our patients don't come with ideal bone

Really appreciate your professionalism and compassion. I was good until you mentioned taking tissue from the roof of the mouth. I have read testimonies of people who suffered much pain and difficulty eating from that. Can it be taken elsewhere? Bridges do sound like the best option but I think perhaps I may only afford the All on 4's. Both Arches. I have a virtual appt. with you set for march 7, at 2pm. I'm on Maui Hawaii. When I say, "tissue from the roof of the mouth", I do not mean taking it from... I mean making the incision favoring the roof (palate) of the mouth. An incision for the implants must be made, either way. By favoring the roof it allows me to bring more of the good tissue (keratinized) to the cheek side of the implant which gives many benefits. It is not a soft tissue graft from the palate but rather using what is already there, in abundance. I hope this answers your question. We have trouble opening the "sent" 3D scans. Are these the same ? just different marketing name? Hello, great video. It is well needed. All on 4 treatments are being touted as the most beautiful solution in dental history. When I asked for this treatment at a dentist I had been going to for years, the lab wouldn't quote a price for the zirconium because they said the prognosis is so poor, they don't even make them. Because I had worn a round house fixed bridge for so long, under which the back had no teeth, I had significant bone loss. And since I refused a sinus-lift, I was told my only option was an upper overdenture. I do however have a capped tooth at the back of my upper left jaw, the six in the front were extracted and bone grafting and mini-implant placement was done four months ago. There was no temporary, which was fine with me as I have to wear a mask outdoors anyway, and I couldn't imagine anything near my gums after surgery. My question is, do you offer other options (than over-dentures) for those who are clearly in need of a sinus lift? Thank you!

Many patients have bone that other dentists ignore or don't see. And, there are other sinus lift procedures that are not as invasive. I recommend a CT scan and a new treatment plan.

Thank you Dr for such a wonderful and detailed video.I would like to ask if the treatment you offer of an 'implant bridge' is possible for a case wherein there is almost no bone in the maxillary molar regions(all 3 molars) and the sinus has dropped way down.How will the implant-bridge be supported in such a case?To put it simply 'how is an edentulous jaw made suitable for implant-bridge'?..Do you do bone grafting first in such cases?.Eagerly awaiting your reply🙏🏼

Yes. With bone level implants we use straight abutments 95% of the time. 5% of the time we must use angled abutments because of bone anatomy. However, when you don't remove bone as in the all-on-4 process you can successfully use an angled abutment when needed.

@@babacanoflaz1403 Absolutely! If the teeth have a good long-term prognosis (good chance to survive 10-years) it is worth considering. However, if they don't have a good long-term prognosis it's usually a waste of time and money.

Wow! I knew something didn’t seem right about how easy they make the all on four sound. I am trying to find a good prosthedontist in the seattle area but most recommend the all on four with angled abutments. Is there anyone in WA state you would recommend? I wish your practice was in my state.

Unfortunately, I do not know anyone I can recommend in your area. I wish I did. If you're able to travel to Dallas for treatment, we'd be happy to help you. We see patients all the time from out of state, and do our best to make the visits efficient and as few as possible. If you want to research on your own there in Seattle, I would advise: 1) Calling Prosthodontists (find in the American College of Prosthodontics directory at www.gotoapro.org/) in your area and say, "I need full arch (or full mouth) dental implant treatment. I’ve done my research and decided I do not want All-On-4. I’d like implants and fixed porcelain bridges, without any angled implants or angled abutments. Do you do this type of treatment?" 2) "Do you have photos and x-rays of before and after cases of this type of treatment that you’ve done that you could show me when I come for my consultation?" 3) "Do you place your own implants? (If not), "Do you work with an oral surgeon or periodontist who is able to place non-angled implants for screw-retained, fixed implant bridges? I wish you the best of success in your search!

Great v doc learned so much about history of implants why all 4 was invented for ie people with no teeth. I think it’s criminal to offer this knowing how much healthy bone and tissue would be lost forever to accommodate this procedure ! I agree this is not process as every where you look it’s one system thank you for giving us education and allowing patients to make informed decision. I am in this process after suffering advance gum disease. I want my investment to last me as it’s one time I can afford to do this.
