The strength of the temporal bone has little to do with the pressure of the plug of dead skin within the canal to cause the eventual widening/expansion of the canal. Any pressure, even relatively minor, will stimulate the presence of osteoclasts within the temporal bone, which in turn erodes the periphery of the bone and which in turn results in the widening of the canal.

Mr Neel, thank you for sharing your video and thoughts on this patients ear and treatment, the right ear looked horrendous. Our son has a damaged trapanic membrane and permanent deafness from birth, but two years ago he developed an horrendous condition that required emergency treatment, consultation, then CAT and MRI scans and ultimately 5 hour surgery where the surgeon found a huge Cholesteatoma tumour in the inner ear which was eating away the scull bone, almost through to the brain. Trouble was he also has narcolepsy and sleep apnea due to his permanent brain damage post birth caused by gross negligence by nursing and midwife staff at a military hospital in Germany because he was born premature and had hyaline membrane disease, stage 4 air block in his lungs and palpable mass in the abdomen that later just disappeared. They missed his ear was also permanently damaged, and had hearing loss in both ears. You do an outstanding job for your patients 👏 🙌 . Keep up the great work. Best regards, Chris.

When my children were little, a couple of them really had struggles with impacted ear wax ... the pediatrician that they all saw was brilliant doc and Ioved him. Dr Gunderson suggested going to the pharmacy and purchasing Colase, a stool softener that is sold in capsules. Poke a hole in the capsule and dribble the contents into thr ear canal. This works best by placing your child's head on your lap with chosen ear up. Next use one of those aspiration bulb and drip a small amount of warm water into the ear. The Colace works to soften the wax. After 5-8 minutes, squirt more warm water into the ear and the wax and dirt will rinse right out. We had a greatly lowered occurrence of ear aches!

That was masterful work, as was yesterday's video. I'm in awe, that you could that right ear without anaesthesia and without causing pain. I'm very curious to see your new range of tools, and especially to see how they compare to others I've seen used, like the Rosen Inserter. I never would have thought I'd be excited by ear curettes, but here we are. I suppose there are many worse youtube rabbit holes I could have fallen down...

Have you ended up on cow hooves yet? I'm addicted and I have no idea how I got there...

@@Blue_Sonnet I've been following The Hoof GP since before I discovered these videos. I started with pimple popping, and now I'm watching stuff from hoof trimming to full endoscopic surgeries.

Oh my goodness that was an awesome video. The care you gave to your patient is top notch. I also enjoy seeing these type of videos. The blood doesn’t bother me. I hope this patient feels better soon. I also like the way you explain everything during the procedure. I like learning from you.

That poor patient. Their pain must have been awful. Thank goodness you were able to squeeze them in and get rid of that dead skin. Wow. 😮
