I had my second knee replacement on December 31st and still having issues with full extension. Calf muscles are extremely tight and the back of my heel has become tender from PT. I can feel pressure in the upper part of the knee that feels like it's preventing the extension as well. This knee was injured for quite a while prior to surgery and lacked full extension then. Is this perhaps impeding my recovery too? My therapist says it's coming along but I'm concerned since the other knee healed without issues even though it required straightening during surgery. Any ideas would be appreciated. I found your recent walking normal videos to be super helpful! Thanks for all you do.

Dawn Wagner Thanks so much for the comment. I history of trauma prior to the new year placement always makes recovery challenging. For patients who did not have full knee extension prior to surgery I always ask to see the surgical report to know if the surgeon was able to get full extension during surgery while under anesthesia. The bottom line is you can always improve. It might not happen on your timeline and it might not happen on the surgeon’s timeline but it will happen with steady perseverance. Personally I have found that compression with traction tends to work best when extension is challenging. Look back through some of the older videos and you might find some ideas. I’ll see if I can put some new ideas together next week.

I'm 8 weeks post tkr. While I have good extension (0°) and flexion (125) , my gait is still a problem. I tried your instructions and for the first time, even since before surgery, I have hope that I will walk normally. These 2 simple lessons have already helped me. Thank you so much. I now have confidence that with time and practice, I'll walk straight.

Hi Tony, thanks for the instruction. As you mentioned, I had developed what I'll call an "injured gait" due to the 5 months prior to TKR and the weeks after, learning to walk again. The limp was basically a bad habit that I developed that was an unconcious habit as much as "babying" the new knee. In addition to the exercises you suggested, I began walking backwards, especially on the treadmill (at lower speeds). Because this is not how I normally walk, it allowed me to break the bad habit as well as helping to develope some of the muscles around the knee. Litterally within a week of doing this a couple times a day, for about 5 minutes, I found that I was no longer limping while walking and my improvements came at a faster pace. Thanks again.

Thank You for this video.I’ve had both knees replaced 9 months apart .It’s difficult not to compare each knee .However my last knee replacement of being at the 3 month mark . I was beginning to lose hope of walking normal.This video along with PT is given me back hope . For the first time today someone noticed I was walking so much normal. What a relief ,really !!!!!! Thank You again .

I am watching as information and an overview of the range of experiences I may have as a total knee replacement surgery awaits me about in 5 weeks. This is very helpful because I realize each patient is unique and my experience may or may not be what someone else experences. I have had problems now for about 4 years and at nearly 85 years of age, I could no longer ignore it. I have what is described as an " abnormality" since my right knee has slowly begun to act like a " knock knee". I fell leaving church on Easter ...very embarrasing! I have not been advised to do any physical therapy prior to surgery and I am quite in favor of that approach. I do not know until my pre op visit how long I may stay in the hospital. Some patients leave in under 3 days. I hope that does not happen with me, we shall see. At any rate, I will keep up with your videos and enjoy your approach to knee issues. Thanks. 😊.

Are you able to bend now? I’m 4 weeks post Acl surgery and I can bend only 90 degrees

Thank you so much. Your guidance is very useful to patients like me who got bilateral knee replacement surgery during this covid time and couldn’t get any help from physiotherapists. I live in India. I am following all your videos. I was doing regular yoga asanas before my surgery. Now after three months i have started with simple asanas. Thank you again and bless you.

I tore my posterior cruciate a year ago, postponing surgery until it became clear I would have to have it repaired. So I limped around for about 8 months despite PT, meds and steroid injection. I am now 2 months post-op total knee replacement, having had 2weeks rehab right after surgery (Makoplasty), and I’m having three hours of PT a week. My biggest problem is not the knee, even though it’s not totally healed. The biggest problem is my hip on the opposite side, which everyone thinks is due to having favored the “bad” (operative) side for so long, stressing the opposite side. When I say “hip” I don’t mean the actual hip joint per se. The pain is in the vicinity - not the back, not the buttock. Walking more briskly, purposely digging the heels in while raising the toes/forefoot definitely helps the “hip” pain, but I can’t walk that way forever. My goniometrics on the op leg are coming along. I just don’t know what else I can do to stop the “hip” pain on the contra lateral side.

Greeting sir Orthoscopic surgery was done for knee bending. Suggest some tablets or something along with stiff knee exercises to reduce daily stiffness and reduce the process of adhesion regeneration. I exercise three times a day and when I start exercising again, my knees become stiff. It's been 45 days since I started exercising. The knee bends 120 degrees due to exercise but then becomes stiff again and the pain lasts the whole day.

Hi there appreciate all your help- I am 11 months post TKR I am doing great flexibility walking no pain just occasional discomfort some times at night- I still do excercise most days - one problem I have is that I can walk up stairs holding onto a banister my big problem is I still put 2 feet on one step coming down just dnt feel comfortable using one foot per step how can I improve - I also have the other knee waiting for a replacement an I hve lots of pain in that knee which dies not help. Please advise.thank you so much.

Great videos. I find them very useful but do you have anything pertaining to tibial plateau fractures? Non-weight bearing for 8 weeks im going on week 6. I feel im doing pretty well but sometimes it can be frustrating. My flexion cold is about 114 degrees and once warmed up im about at 125. Thank you. I appreciate your content.

Hey Tony I’m 22 day PO I’m doing well I’m doing stationary bike I want to start my walking but the surgical knee buckles every now and then, how to I prevent that? Or will it work it’s way out in matter of time? Thank you

It depends on why it buckles. The knee might be unstable. It might buckle because of a sudden and unexpected jab of pain. It could buckle due to low back spinal stenosis or other nerve related issues. What does your physical therapist say?

Hi I have osteoarthritis in both knees. Done TKR on left knee 3weeks ago, and with a gel injection to my right knee same time as recommended by doctor since too early for a right knee surgery. 7/8yrs ago my left knee was bad, doctor had recommended UKR, which i avoided, and ended up putting pressure on my right knee. Now with left TKR, my question is how to save my right knee from surgery and exercises for the same.

My second full replacement was about 4 weeks ago. The first one went pretty smoothly, but the second one has not. Specifically, my hamstring has caused me most of my pain and discomfort. This did not happen with my first replacement, done last summer. Is it common to have hamstring difficulties after a total replacement?

I was limping 15 years prior to my surgery. I'm 7 weeks post to my total knee replacement. I have to concentrate walking straight again.

Hi! I just had a bilateral TKR 5 weeks ago. I want to walk normally again. But one leg is shorter than the other, and my hips and lower back are in pain. What can I do? I'm seeing my podiatrist on Tuesday for him to evaluate the leg length discrepancy and prescribe an orthotic.

I wasn't expecting my ankle to hurt. Its really bothering me but one of these videos explained it. I feel I didn't get enough information when in hospital or before the operation. My scar is hideous! I took the dressing off after 3 weeks. It is puckered and lumpy. I'm hoping it might partly be due to soluble stitches under the skin which will eventually disappear.

I had knee replacement 17 days ago. My knee is good although with some pain and I can walk quite well but I have a lot of pain across my pelvis and lower back which sends pains down my leg. Any suggestions on how to relieve this?

I’ve got a massive bone spur keeping me from straightening my left leg. I know my calves, hamstrings and quads are tight. Is there anything preop that I can do to to help myself ahead of surgery?

When you can’t stretch then massage is your best option. Self massage, foam roll, rolling pin, etc.

I was hunched over before my surgery. I am having trouble standing upright while I walk. I fatigue and can't maintain my posture. I actually end up going to fast and fall over. I don't know if my gait is shortening as I fatigue or my back is fatiguing and subconsciously my gait is getting short. Chicken or the egg?

I had both knees replaces at the same day 4 month ago, I have difficulty walking straight and kind of walking from side to side like a duck. Due this, my lower back is very painful as if someone placed a hot steel rod. It is excruciating pain and have great difficulty moving around. Please, help. Thank you.

I got bike accident a year ago there was little injury on outside area of knee but the Pain was very very high. I have gone to Dr. Many Dr. I take X-Ray but report was good. I can't fully straight my leg knee. The distance between injured knee decrease. The Diagnose Centre Mentioned It As Ostiorthitid. I am 21. Please tell me what to do? Whenever i want to bend and straight my leg there is pain in my knee during this period of Motion. Please Suggest Me What To Do?

I had a tkr a month ago they straighten the leg as well..very severe bone chips and 40 year old scars from acl repair. .I now have a lot of pain in the front shin from below the knee to the ankle especially if I try to lengthen the stride. Have tried rolling pin massage but too painful..any suggestions..?

I have a problem in my left knee ........ diagnosis with ATT i.e Bone tuberculosis.......small operations i.e Biopsy ...... So I can't walk normally .....Biopsy done around 9 month back.....please suggest some exercise for that

Can you give some info on recovery from knee op when you have fallen arches. Is this taken into consideration during recovery/operation?

Hi Elaine! Most therapists should take this into consideration. You can discuss arch supports or different footwear. I try to address everything from the toes, feet, ankles, knees, hips, and low back.

I am at the 8 week mark and walking is difficult because I am dragging my surgical leg. I have a tendency to put my weight on my nonsurgical leg and walk peg legged. I have 120 degrees of flexion but still lots of swelling and stiffness. Does the walking ever improve? I am discouraged

Hi I'm 5 weeks out of tkr and I am swinging my leg through fromend of step to start of new one. I am bending my knee and herl toeing down but then seem to have a straight leg working so hard on knee raises and pulling up with cord whilst on my tummy but cant seem to get the walk?

@@totaltherapysolutions I am seeing physio every 2 weeks. I am going on holiday for 4 weeks and wont be seeing anyone u tol back. I am worried I am going to be practicing a bad technique whilst away thats the problem you think it feels right you need to be told what is correct and what isnt or else your muscle memory thinks its ok.

Hi..just had my second knee replacement end of Jan'20.Have a limp in affected knee, how do I overcome that? Surgeon assures me leg is same length, but I feel muscles surrounding joint have tightened and shortened..

It’s certainly possible that the muscles have tightened or the connective tissue has tightened or swelling and other factors are coming in to play. Leg length is controlled by so many variables it’s impossible to nail it down to just one. Your body is amazingly adaptive and can handle pretty significant changes. The best I can do is encourage you to be patient, consider talking to a physical therapist and at least having an evaluation done. Then look at putting a long-term plan together for what you can do on your own and what you might do with a therapist.

The trauma of surgery is significant. The thigh musculature will take time to recover. Take it slow. Give yourself time to recover. The soreness like most things will improve with time and continued effort.

Some implants will only allow a limited amount of bend ever. it is important to ask your surgeon what the implant will allow. But it is also important to understand that there are many factors that go into ROM and the main factor is tightness of the connective tissue in the knee. I would rather have a tight and stable knee than a flexible but unstable knee.
