This was the absolute best conversation I have ever heard on this topic. Not only was Peter a great host as usual, but Irene was so knowledgeable and incredibly well spoken. What an absolute delight of a Podcast!

Man, when Peter was fishing for the brand Altra and about to bring them up, I wish they hadn’t pivoted away from the “cushioned” minimal shoe and spent some time talking about them. I am all for a zero drop heel that allows foot to have a more natural range of motion and strengthen that calf and Achilles. But what is wrong with having cushion underneath that? I was a little frustrated that all she wanted to talk about was the minimalist style and getting as close as possible to barefoot. More than once she brought up how important the transition is between our typical 8 mm heel drop shoes and going minimalist. It seemed like a cushion shoe like the altra would be the perfect way to slowly transition the body to that.

I was thinking the same. I use the Altra racer currently to transition to minimalist shoe running.

51:00 Ok. TOES need to be able to expand laterally properly and to bend, so as to stabilize the foot by its landing on the ball and to soften the impact, which no conventional shoes impeding toes's movement allow. Minimal running ONLY with barefoot-shoes..

Good podcast. The only issue I have is that not all injuries of the foot/ankle come from problems with foot mechanics. Many therapists are saying that hip strength has a huge influence on reducing all the injuries they talked about.

You are not necessarily incorrect But it is complicated, as the entire Posterior Chain matters. I think in context of the podcast, the importance of proper foot function is a very good place to start and I am aware that hip & core strength are an influence as well.

Did not expect a two hour podcast about feet could be interesting. But this really was. Would have been allot better with animations at some points, because I couldn't follow everything.

I’m confident minimalist shoes are better. I’m a foot and ankle surgeon. I’m even more confident, that like all the rest of medicine, people aren’t interested in doing the right thing. “Fix ‘it’ doc, but don’t change me!” Shoes are not any different than pills.

Yes sadly main stream body's culture / mind set regarding one's relation towards its own body (degraded as a tool) But: Peter's audience is not made of that kind of folks..

100% agree. “How do I change this without touching it?” Give me a pill, a brace, an injection. Just fix it so I can get back to my life. It’s what most people want, so medical and PT practices are structured that way. It’s also what insurance companies want - medical, auto, and workers. Makes it difficult for a patient such as myself to get to the root of the problem, so I can recover once and for all and never see you again. In the end I had to figure it out on my own. That lead me to minimal shoes, foot strengthening, and curing my plantar faciitis without drugs, injections, night splints or orthotics.

I’ve heard many docs say running is bad for people, so I’m not sure it’s just patients.

@@Danielle-zq7kb it’s definitely not just patients. Most docs don’t want to put in the effort because it doesn’t pay. It’s never one sided. Humans generally don’t like obstacles that have to be overcome. Quick and easy is almost always preferred, but often isn’t the best solution. Running is definitely hard on the body because it requires so much muscle and as we get older, sarcopenia, muscle loss, is a reality.

Great interview, great questions! Love the other episodes too. Perhaps I was actively listening more than other episodes because I am on the fence about a pair of Xero or inov-8, but I think this one was one of the clearest and easiest to understand yet. Keep up the great work.

The zero drop shoe with cushion is probably Altra and they mostly have a foot-shaped toebox. Unfortunately most minimalist shoes don't accommodate wide feet.

u digging altra shoes? it seems it has midfoot so doesn't qualify as minimalist

@@goutfromfriedokra7099 I like them. No they aren't considered minimalist because most models have a fair amount of cushioning (varies by model). Could be a good transition shoe, but I tried minimalist for awhile and my feet got too tired after awhile. I have 3 different Altras now.

@@Greg_ChockYou think Dr Davis is overstating the benefits of minimalist shoes? If cushioned shoes were so bad--ppl would not be buying adidas and nikes by the boatloads,and i never heard much about ppl having foot issues with those name brands

In reference to why podiatrist likely do not want to stop prescribing orthotics, their patient population is much more likely to be sedentary and nonathletic. Dr. Davis‘s patient population is much more likely to be athletic and willing to do whatever it takes to improve. I think that’s the root cause. If you were treating a patient population which is largely non-compliant, high probability of diabetes etc., it would be much safer malpractice wise to prescribe an orthotic than not. It would be difficult to discourage an orthotic from patients and encourage them to go strengthen their foot or feet, which you know they probably will not do.

But HOW do minimalist shoes or barefoot running improve or correct pronation? What is it about *those* kinds of shoes that other kinds of shoes have or don't have?

Intrinsic foot muscles are subject to more stress, as opposed to cushioning absorbing it. They hypertrophy, help to strengthen and lift the medial arch. They also prompt a midfoot/forefoot strike which caused more plantarflexion and knee flexion during contact, reducing reaction force up into the lower limb, SIJ, and lumbar

@@Rerum_Novarum I bought Xero shoes, which Peter Attia heavily promotes. But with bunions, they are not very comfortable because I also have a Morton's neuroma (from the bunion) and need cushioning. I wish minimalist shoes could help but they don't. At least not me.

@@patriciahoke4722 Diving straight into minimal shoes from cushioned shoes is akin to running 10k without ever having jogged. You need to condition and train your feet for a huge increase in stress. Start by using them for only a short period of the day. Also check out the "short foot" exercise to build arch height and intrinsic muscle strength. Try 5-10 min barefoot treadmill sessions. Apply toe spreaders and mobilise your tarsal bones using a lacrosse ball. Use banded inversion exercise to strengthen tibialis muscles that help to hold up the arch. Consider using cushioned shoes with a wide toe box when not going minimal to allow your big toe freedom to move from the valgus position that causes bunions.

@@Rerum_Novarum I don't think that was the issue. I just think because of the neuroma, the Xero shoes will never be comfortable. But maybe I'm wrong. I *always* buy shoes with a wide toe box. Of course - I have a large bunion. I take out the insert that comes with the shoes and place a forefoot only insert in, this helps. I feel like the shoes that I re-fashion are in fact minimalist, to an extent. I would love to see a gait analyst. Orthopedists don't even watch you walk anymore.

Feet are individually different and some come w painful problems and conditions. The muscular and skeletal mechanics are naturally the same. One must learn and figure out how to help himself appropriately.

I wanted minimalist shoes to start running in so I could get away from cushioned shoes and during March this I started to walk in minimalist shoes because they feel so comfortable to walk in and hearing this is kinda funny

She just said the knee takes up to 7x body weight, so the more overweight the more chance of injury. So chubby heelstrikings a match made in hell. Running’s a skill, and most of us don’t have the same instinctive form as when we were kids.
