Diesel is a slower burning fuel, which is why most of the time they'll rev out before you hit the physical limits of the valve train. The smoke is just fuel that didn't burn properly before the exhaust valve opened. That fuel is injected to burn under boost and/or help cool the cylinder during operation.

I think that is because they cannot run it at low rpms otherwise the torque will destroy the engine, as torque is produced at lower rpms.
