This gentleman has a very long follow up from a nerve transfer designed to improve the ability to dorsiflex his foot (lift up the foot). We transfered a nerve from the back of his leg that used to push the foot downward and attached it to the nerve that used to pick his foot up. After about 6 months of recovery, he learned how to make his foot move again. Prior to surgery he was not able to lift his foot up at all. Now he can do this without thinking. He is now applying for the Coast Guard. This was a very successful procedure. This is a rare procedure for a foot drop.

i hadn’t heard of this issue before seeing this great vid. Also he has great athletic calves and very nice looking healthy bare feet. No socks needed with those boat shoes 😅

Thank you for sharing this video Dr. Williams. I am getting mixed reviews on the effectiveness of this procedure. The general consensus seems to be that it is highly effective. However the two doctors I consulted with about actually performing the procedure painted a less rosy picture. Perhaps this could be in order to "under promise and over deliver". Anyway, one doctor said it was "almost certain" that I would walk without a brace but that I would "never run" (at least without a brace). He described it as highly likely for basic functionality like daily walking but not, for example, sufficient for athletics. Another doctor was less even less rosy. He said 50% of his patients didn't need a brace post-procedure, 25% still need a brace for certain activities, and 25% saw little to not improvement from the procedure. Contrast this to other reviews where people are talking about playing basketball the day after coming out of the boot and it's hard to determine what I could realistically expect from the procedure. What would you say I should expect? Context, I'm 50 years old in above average shape and very active (gym and cycling multiple times per week). Good overall health.

Did he have a traction type injury to his Peroneal nerve? Also how did it take for him to at least move around with a afo after this surgey?

hi, i have recently had a very bad fall, resulting in tibia and fibula breaks in my left leg, the tibia was a complete displacement fracture pretruded threw my shine flesh. i had a tibial nailing from knee to ankle with top and bottom bolt sights. i am left with numb toes no movement in big toe, and foot drop, to which i am not receiving treament for even though it is 6 weeks post op, any abvise please.

Hi Doctor Williams, I'm going to be contacting your office this week, but I was wondering I have bilateral foot drop, I'm 38 and very physically active but this is really causing a problem. I've had several nerve conduction test studies done, but all the neurologists I've seen only offer the same solutions, an AfO and physical therapy.

My son recently severed his Popliteal artery in his right knee along with severing his ACL & PCL…he severed his nerves in his leg as well. they did emergency artery bypass utilizing the saphenous vein from his left leg and replacing the popliteal artery with the vein! They are unable to now fix his ACL & PCL since they are not able to place a tourniquet on his leg for surgery…thinking it will collapse his bypass! Is a tourniquet needed for this surgery? He has severe nerve damage and severe drop foot, that is starting to contract down & inwards! Any insight would be greatly appreciated! He is only 17 and this just happened while camping in the desert.

I am 8 months post-accident. Nobody in germany is willing to do the operation. Doctors said at Mayo Clinics they are also not willing to do this operation anymore. Sad news. Be wary of Dr. Rahul Nath he has commited criminal acts in medics in Texas before.

@@MantasSofia Hi Sofia, I guess you have the same accident? Nerve split transfers as they are sometimes called do have a too low chance to work. There is sometimes operations as nervous grafting, but in my case the to be grafted over are was too big. However don't give up hope, After one-and-half year my foot is able to lift a little again by my own regeneration! So rly take your time before doing a tendon transfer bc there actually might be repair processes, even for a very bad situation going on in your body! Btw you're hot, are you sexy single?

Hi Dr Williams, Excellent video and truly amazing! I have had the same op done over here in the UK to treat my foot drop. I had it done back in December 2015. I just had some questions regarding the recovery and return of dorsiflexion as I cannot find any information on the net. You say this gentleman had return of movement after 6 months, was this full return of dorsiflexion or slight return? What was the first signs he noticed that meant the nerve function was returning? Also what did he do in terms of therapy in the 6 months leading up to be return of movement? Also do you have other patients who have had the same op who may have taken longer to recover? I just want to know what to expect and if there is anything I could do to encourage the nerve growth and return! Thank you in advance!!

Chris Gotti yes! it’s been a year and a half so I’m still learning to control it but overall there is improvement and i’m hopeful i’ll have full control over it soon!

Chris Gotti yes! i would say it’s worth it and even if in the worst case scenario it does not work for you, you can still have a tendon transfer. i was told you can only get the nerve transfer surgery within a year of damaging the nerve. so far, i have had good results and i’m hoping for more

Hi, I have a foot drop after knee dislocation from an accident. Nerve grafting was performed after 3 months of waiting time. It's been 4 months since operation but I have not observed any improvement. In fact my ankles are getting stiff. Can you please help me with it or discuss about it?

Hello i operated tendon transfer 1 years ago. Now, my active dorsiflexion range is 10 degree. I have still too much pain when i'm walking around. I'm 26 years old. Should i expect more ROM on my toes and dorsiflexion next years ? I would be happy if you answer. Thank you so much.
