I'm going into forensic pathology after a horrific fifteen year chronic pain experience in MINNESOTA and I have been interrupted and spoken down to for 15 years and now I'm going into policy-making because empathy is the most powerful care tool. I agree on all three points. Recently, my new oncologist sat down beside me and lightly touched my hand, looked into my eyes, and then asked how I was. That meant so much to me. I then asked how he was, and we formed an instant repoire. Far too often, a doctor or nurse is standing across the room, banging away on a keyboard.

I appreciate a doctor who sits down to speak with you. It feels like they want to hear you and help you. I understand that doctors are entering chart notes in their computer while you are sharing, but some face time is appreciated. Thank you for your videos - they are quite helpful.

Thank you for your videos. I think it’s important for a doctor to sit down and listen to there patients. I have high reviews for all of my doctors that take the time to do this. It shows that they care and want to listen to my concerns. Then with this it helps me to know that I can trust them. I’ve only had to stop seeing one that didn’t do this.

I had a surgeon who did these but have dealt with people who act like they are better than their patient and act as if their patient is lying or attention seeking

Same in fact the majority of the doctors I’ve seen over many years now have been arrogant and patronising and they mostly ignored everything I said to the point where I nearly died and it happened more than once too.

Touching on your comment about the role of the anesthesiologist, I wanted to elaborate on an experience just 6 months ago. I was in pre-op for 2 abdominal surgeries and I was concerned about some thigh cramping (not Charlie Horse) so I asked him to let OR know to not attempt to straighten them. He listened intently, trusted I knew my body and said he was adding a muscle relaxant. Long story short- they said my legs seized up anyway. This Dr took the time before and after. Explained things and listened to my opinion. Granted my retired nurse but I got the impression this was the way he was with all his patients. He also checked on me and stayed with me post-op.

I too suffer from severe medical PTSD and chronic pain, among other medical “issues “…. ALL DOCTORS should be required to take certain classes at law school, and realize they should be dedicated to healing, before being a “small business owner “… The healthcare care system is a MESS and sadly it can’t be changed in the KZbin comments section… I applaud your honesty Doctor and you know how much respect I have for YOU… Chris

You are correct on all levels. I was scheduled to see my anesthesiologist the day before knee surgery. He knew im a paramedic that also performs RSI. He asked me what i wanted to be sedated with, i told him whatever he felt comfortable with under the conditions. He said that would be propofol.Then he asked me if i wanted to be intubated. I told him, no, unless he feels there might be an impending airway crash. I ony had 1 request, which made him feel good. I told him if i needed to be tubed, i wanted it done by him or respiratory, NOT a student. He laughed. My reasoning was i remember 23 years ago, as a medic student, how nervous i was the first time i entered the OR for my first live intubation. All went well, the only thing i remember the nurse handing me a cold sprite in PACU.

@warbler1984 it's common practice for paramedic students and is monitored closely by the OR staff. It's normally not allowed for emergency surgery, or any patient diagnosed with any airway abnormalities, tracheal deviation, etc.

When I got my knee replacement, I was terrified. I took a half xanax, which equalled .25 mg. while waiting in a room. Anesthesiologist walked in, introduced himself. I told him I took the xanax, b/c I thought he would want to know. He stopped in his tracks, and walked out w/out saying a word to me. Few mins later a different guy walked in, and introduced himself as my anesthesiologist (as if the first guy didn't exist). We had a good repport, and all was well, but I never knew anything about anesthesiology until your channel came along, and I found your videos. I cannot thank you enough for being honest and open, and for giving us your best. 💛

As a Canadian I am fortunate to have medical coverage. Having said that we are experiencing a storage of health care workers. Just to give a bit of a background I will try to outline what I have experienced of late. We had an amazing doctor who had to retire due to medical issues of their own. This is where things went pear shaped. When I met the new doctor I explained to them I am not someone who is at the doctors often, and because of this it is important as I see it that we have a relationship so they will be able to understand that I am only there when it is needed. And they need to be able to ask questions to read the reason why I came in. This is why I am in total agreement of the 3 steps you talk about. This aren't steps this doctor follows. 7 days post-op (a left mitral valve repair that the cardiologist response to seeing me was "How the hell did this get missed") I went to have 2 stitches removed. The question was "how is going?" I showed the incision site and said "it seems to be healing well". There were no further questions such as, how are you sleeping, how is your pain level, how are you managing etc. Because of this lack of due care I suffered through trips to the ER. Where thank God I recieved competent and good care. I get it that time is of great value and in short supply, but doctors out there please remember when you are late to start the appointment it should not cut into that patience time. Also phone call appointments are fine for prescription refills or for check in's if you need to assess if the person needs to come in to the office for a face to face. Want to cut down on waste time then make the prescription refills for longer periods and not a way to make a quick fee. And all of this comes from a pretty medically savvy person who is good at articulating and is a strong advocate. And I am having a terrible time with this doctor. I feel so sorry for others who don't have anyone to be able to help them when they are in this situation.

I just had trigger finger surgery recently. The anesthesiologist came in and sat on a low stool next to my hospital bed when interviewing me prior to going to the OR. I so wanted to ask him if he watches your videos here because I've learned so much. He was very nice and thorough. That being said, when they wheeled me to the OR and the anesthesiologist came in, it was someone different!!! I actually asked where my anesthesiologist went and the surgeon tried to assure me that the one I now have was just as qualified but it was so disconcerting. Anyway, just wanted to share my experience with the anesthesiologist situation and I'm curious why something like that would happen.

All of my doctors sit down and is a good listener, I would be a great anesthesiologist like you, I donated to the anesthesiologist Foundation again new year, I really enjoy donating to the anesthesiologist Foundation, since it was hard for me to go to college because of my cerebral palsy, my MRI went great on Dec 23 I had a great anesthesiologist he was hilarious , he told me he has a Tesla! Happy new year! You rock

I met with a surgeon for my new onset Barrett's esophagus. I'm only 29 so my GP was supportive of pursuing surgery. the surgeon sat across from me and asked about why I had seen pretty much everyone in his department. I explained that I was kinda bounced around looking for answers and then he sat next to me and started writing on a blank sheet of paper. he explained my history, which he had obviously taken the time to read. he told me what the course of action would be if we didn't do surgery and then drew pictures of how the surgery works. every time he said the word 'squeeze' when referring to what the surgery does, he would squeeze my arm a little bit to help explain. I asked if the surgery was the right thing to do and he said that my 'data sucks' and the only way to improve it at this point would be with the surgery. I have poor swallowing as well so he said he would read up on doing this operation on people with ineffective esophageal motility and also text my gp about my case to make sure everyone is on the same page. 12/10 confident in that man to give me a good hernia hotdog.

I went to a dr for years who was a cold uncaring robot. When an opening finally came up with a different dr I switched over immediately. At my first appt I was blown away. He sat down across from me, touched my hand, looked me in the eye and asked me how I was. Then listened without interruption. Thank you Lord.

I am 6’2” and have almost always been above my doctor’s eye level. It’s good to remember that less tall patients deserve this respect. I have eye-contact, caring, and time-giving doctors, but I perceive you to be exceptional in your respect for your patients and viewers. You even answered me from your practice website about United Healthcare!

The first one is so important! I had a specialist walk in the exam room, reading my chart, never looking up. He starting admonishing me about my weight. Then when he finally looked at me, he said “Oh, you are tall, never mind”. I’m 5’ 11’ not the average 5’ 5’ for a woman. I never went back.

I totally agree with you! With my doctors I’m hushed up and rushed out. I guess they’re afraid I’m going to ask for pain meds. Which I never do, btw. Thank you!

I agree with your points but I won’t hold my breath. My biggest problem is when I read the After Visit Summary online and it contains several lies or twisted words. I never said this or that, or the doctor never asked me about something, or the doc never told me about the certain diet and no I can’t agree to something that wasn’t discussed. During my last appointment, my Dr asked me if I drink alcohol. I’ve been seeing her for 7 years now! I said, “I’m STILL not a drinker.” She charged my insurance $60 for a 5-second question, calling it an “Annual Alcohol Screening.” So, padding the bill also angers me.

@@HopefulEmpath yes, I had a 🗒️ in my chart “patient non-compliant on meds” For heaven’s sake it was an IV med I had been given when hospitalized for Covid! They didn’t remove it from my med list when they released me. It took me almost a year to get someone to examine the list with me and agree that that med should not be on the list! Every time I tried to get it removed, I pointed out that it was given by IV in hospital when I had Covid. The nurses would say a doctor has to remove it, we’ll see that she does. Months went by and it never happened. I was continually flagged as non-compliant until I said either straighten this out or I change doctors. I changed anyway after they fixed it. This wasn’t my only problem with them.

I think that building trust with the patient also helps physicians avoid being sued when an error is made. People don’t want to hurt doctors they love and trust.

I believe you should have a mandatory class for all medical students. Probably as well a mandatory class for all humans too. You seem like an amazing Doctor I wish I had a primary care physician with an attitude and outlook like your's. Why is it that the best attitude doctors are in a field of work that does not include primary care? It sucks! Yhanks for what you do with the channel! Cheers!!

The real three secrets. 1. Docs today not only don’t sit eye to eye, they sit back turned or sideways to the patient as they enter everything into the computer as you sit there. Makes you feel like a bug on the ceiling. 2. Who interrupts whom is not the issue. Listening is. 3. Expectations? We aren’t even told relatively new critical info. Example: The relationship between acb medicines and dementia may not be categorically proven, but by the time it is, I’d have dementia. The a-c-b stands for “anticholinergic burden.” If that sounds ominous, it is. It means medicines that interrupt a neurotransmitter called acetylcholine. If that sounds bad, it is. This is the neurotransmitter that supports learning and memory. And if it sounds like you don’t need a doctor to tell you that wrecking that neurotransmitter is a very bad thing, it is. Dementia results. Of course, Big Pharma says no no no, take all the acb meds your doctor wants. You can find their exact names on the acb list by a simple search. All this I learned by accident, not from my physician. I no longer expect even the truth from my doctor.

I wish the orthopedic surgeon that replaced my knee could view this video. He failed on all three of your points. I was talked down to, both physically and intellectually. He also must have had some extreme time constraints given that I had about four minutes during an office visit to decide whether or not to go through with a brutal total joint replacement. While he was technically very proficient and experienced, his “bedside manner” was completely absent, even during the long recovery. He had no time to answer questions about pain meds and the expected recovery goal timeline.

You've made some very valid points. However, as a patient, my experience with physicians bringing their scheduled patient waiting room intake to an abrupt standstill due to the arrival of a pharmaceutical representative leads me to the understanding that not only will my wait time be drastically increased but my facetime with my physician be rushed. This singular action leaves me frustrated and very few, if any, of my questions asked and answered.

I agree completely with your three tips & will perhaps add one more. Talking with your doctor eye to eye is huge & truly does help make it feel more on an even level. Often times a small touch can also go a long way in promoting compassion & support. A touch on the shoulder or arm for example. Knowing when to interrupt to ask clarifying questions or to even ask what their real concern is when they ask certain questions is helpful in building that trust between patients & doctors. Making a treatment plan with the patient as a partner increases the likelihood of compliance & open, honest communication.

I had the best GP for years. He talked in his office the first time he met me, then a nurse took me to an examining room and took vitals. The doctor knocked and waited for an answer before entering the room. After his exam, I got dressed and met him in his office again where he was writing notes on the exam. He would talk with me, recommend a test or bloodwork and ask a few more questions. In the yearly exams that followed he always did that after exam meetup in his office where he would ask questions and answer any questions I had. I never felt hurried but felt respected as a person. He was a terrific doctor. When he retired he volunteered at a free clinic for the very poor!

My doctor sits at eye level but he has his face in a computer. He’s okay at times but he doesn’t answer my questions. I have DDD and scoliosis in my low back. I’ve asked him if disk replacement could be considered and he says, “maybe in the future”, then changes the subject. I have horrible sciatica that keeps me home most days. I’d like to know why not now? Why can’t he even talk with me about it?

Dr Kaveh, I am very pleased with what you mentioned of the three things you wished you learned in medical school. It is sad that too many of us are losing faith in the medical system. The only doctor and network of specialists I would trust are the ones that are oriented or practice the lifestyle I would like to live. And instead of refer and prescribe, investigate the true cause of the illnesses. It is better to seek a doctor who is interested in wellness enhancement into a patient's advanced years instead of a doctor who. is interested in sickness enhancement.

My PCP never makes me feel rushed and I appreciate her. She told me that she really wishes she could spend more time with her patients. I always thank her at the end of our appointments. And I can tell that makes her happy.

Everything depends on the DR. Some DRs are great, others not. You can get a lot more accomplished (the 3 things you mentioned) with a DR willing to listen and work with you, than a DR who just wants you in and out.

I’ve gone to the ED and I feel the doctors don’t ever take u seriously.. I always end up going twice & what they should have done the 1st time around they do it on the second visit.. after I’ve already suffered days on end.. also they say we r gonna give u fluids etc. the iv bag is still full & then they’re kicking me out.. so no fluids went it my body cuz we r getting rushed out yet we r getting charged for them.

I've no idea what your medical history or problem was but ED doctors are focussed on solving the immediate life threats. Make sure you take ownership of your health and see your GP for follow up

@@warbler1984 I had completely lost my voice, heavy cough, soreness at the back of my throat, hardest time swallowing.. hard time BREATHING, earache, no nose mucus or drip AT ALL but yet, salivating severely couldn’t close my mouth or just had to constantly keep spitting out the excessive saliva.. obviously sleeping was a huge challenge! All these symptoms the 1st time ED visit then the second time 3 days later was due to almost being delirious for not being able to sleep.. fever was picking more.. finally they ordered a chest scan & found I had as the nurse put it “traces of pneumonia” then meds in iv were ordered & that’s when not even a 1/4 of fluids were absorbed got sent home with prescription took meds & it made a slight difference, the salivating stopped two days after that second visit..cough, no voice, excessive phlegm, earache, no sleep, hard time breathing, was still there & to top it off I got pink eye about 3 days after second visit to ED.. I started just medicating myself after this I was done with Dr.s & ED visits.. I had left over meds for pink eye from my kids sinus infection & penicillin, I took it all.. inspite of being on an antibiotic I stopping it completely & took penicillin & seemed to work.. I saw a big different of how I felt in general was actually getting slome sleep.. cuz this now diagnosed pneumonia was really k*lling me I was going nuts not being able to sleep.. I was desperate if the ER can’t help then this is y people do this & start medicating themselves or does it take a 3rd visit? Do I switch hospitals? What r people suppose to do? I know what the purpose of an emergency room visit is I just wish they get it right the 1st time.. this is very common here! My daughter had a twisting ovary full on knot in progress, she was screaming at the ED & they scanned her & said she was backed up & had her take that clear liquid bottle to help her go to the bathroom!!! On her second hospital visit screaming once again finally got told there was something wrong with her ovary when we saw a specialist the picture of her fallopian tube & ovary were in a perfect gift knot! To think on her 1st visit to the emergency room told her she was just backed up.. even tho my daughter was on the floor screaming & throwing up due to the pain..! It’s ridiculous!

# 2: I'm a talker and I go in w/ a list. My MD is Great with listening, considering , and responding. He will say"Ok! Now I have some things I have to ask you!"

Eye level? They hide/burrow behind their computer with no eye contact at all. They used to sit sideways and show you the screen. But now , not even that. Also by the time the nurse and all others get done asking you their questions and the Dr fills in his/her computer data boxes, there is less than 5 min total of interaction time! ...no listening respodindg or interaction time. (Especially dificult when expensive tests are ordered and yet results are not explained.)

These are some good tips, that I'll definitely remember for when I become a doctor. As a polyglot, I think these tips could be especially helpful when deal with cross-cultural barriers, where the concept of trust may be different or more challenging when dealing with people that see the world differently due to cultural differences. I would like to also add cultural sensitivity to the list; I think that helps significantly as well. I know the last couple of times I saw a doctor, I had to look some things up, because the doctor didn't tell me certain details about the medicine that I was taking or the state of my health conditions, fortunately, by having a Pre-Med background that helped me to be aware of certain things relating to my health, that was not discussed, probably because of the doctor's time constraints. Yet most patients aren't so fortunate, and many have no scientific or medical background at all. So, I think without that extra time to discuss the nature of one's health conditions or in more detail the effects of certain medications, of course it may breed distrust. I can understand why doctors may limit their time, since Medical Insurances place constraints on how long a patient is seen by a doctor. Perhaps it might be a good idea for more primary care doctors to collaborate with pharmacists, through referring a pharmacist that might be one the same medical team, as I know is the case with my father when he goes to the V.A. Hospital for primary and specialist care. Perhaps State or Federal Legal intervention on restricting medical insurances from placing restraints of time spent between a patient and doctor might also be a way of resolving some of these issues discussed.

I am a med student who recently had to go through surgery and I think I understand both pov. I wished my doctor could talk to me more and be a bit empathetic. I had one Dr who stood at the counter which was about 5’ from me. She glanced at me but never came closer. I thought she didn’t like me and didn’t care to be bothered about me. It hurt because this was my first visit with her. It was also my last visit. I don’t need to be coddled but I do need to feel that at least for that moment, the Dr cares about my health.

I've never met with a Dr, whether my GP or consultant, who listen the only people who never interrupt are anaesthetist, but then t they can knock you out if it goes on. 😅 I wish you were with me when you're scared of what's going on with my body. Keep doing you. One day, I may be lucky enough to get someone to answer my questions without interrupting me before I have asked a simple question. So, as I said, keep doing you, and one day, you may teach the others a thing or too.😊

I no longer have any FAITH in the medical profession . I have seizures, blackouts, TIAs and fall a lot causing head trauma. ER trips are useless as Xrays are taken and then get turfed. I will no longer go to hospitals for help as none is given. there are no explanations. The blackouts are the worst as I am mobile during this and have found myself in strange places without any idea of how i GOT THERE. I sold the car to prevent mowing down my neighbors but found I use my mobility scooter in place of it. I honestly feel my next fall with with head injury will be my last. I have decided to let nature take it's course at home as I no longer have options.

I hope you are protecting your head with a helmet I had surgery on my skull for cancer and had seizures before then and were a bicycle helmet for a year and a half to avoid a head injury particularly in the area they cut a rectangle in my skull

Some Doctors think they know best for their patient. They power trip and try to play God! I had one Doctor who did not inform me of a surgical procedure he planned to do. I learned about it when I went to the pre-operative appointment. I did not have the surgery and I’ve since healed my body.

This is good knowledge. I am becoming a psychologist so I try to also study psychiatry and different doctor’s. I want to have a good understanding of biology and how illnesses affects people and it will hopefully make me a better therapist!

I had a specialist he would never look at me but look at his laptop. I asked several questions I was not impressed I excused myself* had to go to the washroom* and never looked, arrogant, gaslighting not interested in that type of doctors

I hate that hospital systems put unrealistic scheduling requirements on providers. Our providers are supposed to see a patient every ten minutes. That is not adequate time to assess and discuss treatments. It’s no surprise that people are unhappy and are right to feel that way.

Great information! I think it sucks that Dr's are expected to see so many patients in one day. Tough to have a good information exchange with they are expected to kick you out after 15 minutes or less

My new Providence Doc doesn't even have time to sit down. He comes in, glues his eyes to the computer, reads what his aide has taken down as well as my newest blood pressure numbers, etc., and twice he's announced that he's trying to stay on a schedule and only has a little time. Fortunately I am a very healthy 74 year old.!

Nothing is worse to me than my GP standing in the doorway(owning the space), glaring at me (judging) and saying “what can I do for you”, interrupting me often and then doing nothing. Then he wants to give me a hug at the end of the visit. Help!!

I believe being able to speak up for yourself comes with age. People in their 20's and 30's are not going to be as assertive as a 50- or 60-year-old will be.

I have so many bad experiences with doctors. It is important that they want to listen to you. And I notice they often want to do that at first. But then they want to fit you into a protocol. That is great if that protocol fits you. But if you have conditions that the doctor does not understand and you don't fit into a standard protocol, that is where the problems start for me. Because at that point the doctors usually are not looking any further and don't give any proper care. That is my experience. I have hEDS, mcas, dysautonomia, common variable immunodeficiency, gut motility disorder, hyperglycemia. It's a fight to get the right diagnosis and the right care. What I want doctors to know? Look beyond standard protocols if the standard protocol doesn't fit a patient. It can still be physical, despite not fitting the standard protocol.

I have allergies and cannot take certain meds and sometimes if I have never taken a med I could be allergic to it. I suffer from these allergies ,as the meds I am allergic to effects my lungs and causes mucus so horrible I have a hard time to breath.Yet the doctor actually wants me to continue with this med , I am talked down to and am ignored.

Do doctors allow patients to record their consultation? A friend's husband was diagnosed with Stage 4 cancer and when I asked some basic questions she couldn't remember what the doctor said because they were so overwhelmed with information and new medical terms and vocabulary. I thought that if they had filmed their consultation then they could review it and also get family and friends to help them process the information. So I was just wondering if doctors have a rule against that. Zoom call consultations could be viewed by other people in the room so there are exceptions to the whole privacy issue.

I have severe COPD with frequent flare-ups requiring emergency room visits and often hospitalization. My pulmonary doctor insinuated that I was just having panic attacks. Edit: I am looking for a new pulmonary dr.

I hate that my doctor doesn't read my medical record before my time starts additionally he's typing on the computer And I feel I have to let him finish before the discussion continues. I was bringing a list so he has a reference but lately he hasn't wanted to keep the list And work on a doctor's notes later. This means that I can cover less ground. As fully disabled I have a lot of issues that need revisiting But I often leave without feeling discussion has been completed. Then I can't get another appointment anytime soon. Maybe it's just the VA.

Hi Dr. Kaveh , love the video . I did 40 sessions of TMS ending October 20th , and now have been through 8 sessions of Spravato , which I really enjoy . I’m wondering how long long the neuroplasticity last and how best to take advantage of it ? Also wondering about the durability , what to expect ? I don’t mind going back for “ booster “ sessions. Any suggestions , thanks !

I wish my neurosurgeon would listen to me, period. He's left 2 disks in my neck & said i was stable. I've fallen 5 times since he did a laminectomy decompression surgery in March 2022 & he left the disks alone. One disk, C3-4 is severe on one side & C4-5 is severe on the opposite side. I've had this darn deep down in my burning sensation that has run rampant deeply in my body 🥵 & nobody wants to research why!!! I also have told them that my lower back, left hip & left leg & groin are killing me & I need to know if anyone has any idea what I'm dealing with? Somehow, I have broken a vertebrae at T5-6 & they said they won't do anything about it. Well, in 2017, on the way home from having a cervical MRI done, we were rear-ended & I took an ambulance ride to the hospital & said they saw nothing to determine my pain. Well, they never looked either. So, I'm sure that's when I broke my back. I'm entitled to a 2nd opinion. I'm changing neurologist & neurosurgeon. I've been on opioid medication since 2009 after having a failed L5-6. I'm currently in the process of getting off of it & I'm only on 4 mg a day & preparing to go down this next week if I can tolerate the withdrawal symptoms. I had a neurostimulator implanted & moved around many times because it shocked me & told them to get it out of my spine & get that darn battery pack out of my hip. I've never been so unhappy in my life. 😡

Trust in the medical system? Well I think I would refuse any future tests or scans until it's explained how the outcome would affect my treatment. Sore wrist. Got an xray. You have osteoarthritis. Take painkillers. Yeah, nothing changed there. Sore hip. Got a back xray. Mild osteoarthritis. Yes, I can take OTC painkillers, I don't need a doctor or xrays to figure that out. But I do like my GP; most conversations we are both sitting so at the same level.
