It was not Luigi's pain that causesd people to sympathize with the assassination. That was not in the news at first. People's reactions were in response to their own experiences and the experiences of their loved ones. Not because they had pain, but because they were denied treatment when they had paid into health insurance for years. Not only treatment for painful conditions, but necessary life-saving treatment.

It's SO INCREDIBLY VALIDATING hearing all of this from a Doctor. Everyday people with chronic pain have to act as if we aren't in pain - that's more draining than just dealing with the pain. Pain every day of our lives can make us cranky, but if we are cranky we could lose our jobs. It's also frustrating to try to get medication that can help with the pain, and the doctor is reluctant to prescribe it because they treat everyone suspiciously.

I agree with you 100%. I’ve been living with chronic back pain since 19 years old, I’m 43 now. I’ve had spinal fusion surgery which helped the sciatica (ironically at U Penn). But this condition affects every aspect of my life. People say I’m cranky and I agree with them, it’s to the point where I don’t even open up about my chronic pain issues because, sadly, no one really cares. It has had negative impacts on relationships, work, social interactions, just about every aspect of my life. You’re not alone Melissa thank you for sharing this comment.

Yup... I hear you. I have severe muscle and nerve pain that ruins my life. I waited 6 months to see a rheumatologist, had to cancel when the time came because I was too sick to go... had to wait another 6 months... when I got there I poured my heart out to him about where it hurts, etc. etc., he gave me some tests and xrays. Everything looked normal. So he said he couldn't help me. I asked him, what about fibromyalgia? He handed me a piece of paper where I check marked every symptom of fibromyalgia that was listed. He said that I have all the criteria for fibromyalgia but he doesn't believe in fibromyalgia. So out the door I went. I can't afford physical therapy with a copay of 35 dollars 3 times a week... I can't afford massage therapy for 90 bucks for an hour that I would need probably once a week, bare minimum. Anti inflammatory meds don't help. My doctor will only give me a small amount of pain killers and I better not ask for more.

As a chronic pain patient, my experience is that our system is a nightmare. So many pharmacists have no clue about chronic pain patients' needs, and we are vilified. We are denied medications that our physicians have authorized. It is beyond my comprehension that a pharmacist can 'overrule' a doctor and refuse to supply a prescribed medication. For heaven's sake, who has the medical degree and has examined the patient? The doctor or some pharmacist?! Absolutely infuriating.

You got to find a pain specialist to treat you now. Either your primary care Dr or call your insurance to help you find this kind of specialist . It is what I did, broke back at 18. Two surgeries and I'm 43 now. Severe arthritis, can't have anymore surgery so I must live with it. I vomited daily from pain, after vomiting in my sleep 2 different times and getting double pneumonia both times, spending days in the ICU and a week in hospital they told me to ask my PCP for a referral to a pain specialist.

Sorry for your extensive troubles. Wonder if you've considered the emerging information on how something as surprising as changing the types of foods/drinks you consume and the timing can have major impacts on not just "body" health but also mind conditions like anxiety, depression - even schizophrenia even with psychotic symptoms like hallucinations! Somebody famous (Aristotle?) over 2 million years ago said, "Let food be your cure" - and altering the diet (look up keto or something a bit milder to start with like keto-flex) is a relatively inexpensive, low side effect thing to try. It really seems to help a huge number of people and apparently there's a lot of science supporting it. And it's clear that people who have untreated/inadequately treated depression/anxiety have more trouble dealing with pain. There are probably some psychological reasons for that but biological reasons are very likely too - so fix the mood and anxiety some, and that should help get you on a virtuous cycle (opposite of a vicious cycle). And if you have depression and anxiety, you may not be sleeping well from those; if you don't get good, rejuvenating sleep, your body can't heal as well and potentially can't lessen your perception of pain signals. Don't lose hope, keep trying new things to lessen your various symptoms and it all may come together over time. Curious what the other commentor will be saying about ovarian cysts and dental issues and what the common culprit of those is...

@@tinamadden25 If you had cysts, then you likely had very elevated insulin levels. Many years ago, researchers discovered that giving patients with very high insulin levels Metformin resulted in two significant outcomes: the cysts disappeared, and so did the pain. I'm surprised you didn't experience dental issues, as high insulin levels can cause tooth sensitivity. Nevertheless, high insulin levels can wreak havoc on the body. Unfortunately, not many physicians or dentists have made this connection. I'm aware of patients who are still undergoing surgery instead of being prescribed Metformin. Another problem is that checking insulin levels has not yet become a standard practice in medicine. However, there are ongoing studies exploring the link between high insulin levels and various bodily issues.

The bear of it is, pain is subjective. We don't have a pain-o-meter to measure it, worse, people with very similar - even identical seeming injuries/diseases in all objectivea measurements can have different perceptions of pain. As an example, a military "gas chamber" full of tear gas can be excruciating for most people, outright disabling and debilitating for many, significantly disabling for others (such as barely capable of stating their name and rank) and for others, myself included, well I'm notorious for being medical coverage for such training and literally have stood in the same chamber with trainees while smoking a cigarette and casually conversing. Yes, I felt the discomfort, I was able to dismiss it and still can. I dismiss a fair amount of pain from my lumbar spine and shoulder, a hot wire, occasionally feeling electrified going down one buttocks cheek to the hell of my foot will impact mobility and function, but not as significantly in others - that doesn't mean they aren't in pain or I'm not in pain, it's individual tolerances. They vary tremendously and both do require individualized treatments. Each individual's perception and reception of pain is different. That pain in the lumbar, largely SI regional, but the leg radiation is due to a lumbar disc, the SI joint apparently also irritable easily and per multiple physical terrorists, mobile more than usual. End result, that pain, loss of proprioception (sensation of joint position and pressures exerted by muscles to hold me up and walk) and occasional loss of function briefly, resulting in my knee buckling and unceremoniously dumping me to the ground. Personally, I prefer a proper ceremony for that, lacking that, I walk with a cane. Yeah, I've a sense of humor that's EMS dark. I've also a have IBD, not IBS, Irritable Bowl Disease, not Syndrome - specifically, Crohn's disease, which was recently diagnosed. With that, there are increased incidences of other inflammatory diseases, including SI joint inflammation, arthritis, oh hell, a constellation of issues that may arise. I've also got autoimmune thyroid disease, Grave's disease. Even money, they're all linked, doctor would be able to tell if it's relevant to any potential treatment. Oh wait, a few are, courtesy of new biologics now available and in usage (and need to be blessed by the *!@&!%$!!! insurance company, just got denied one, but I actually agree with the company on efficacy grounds on their proposed and now in for blessing, erm, approval). I own firearms, am a competition shooter, a military war veteran and for insurance companies, my big guns are lawyers, courts and prior to that, their own policies and procedures. Bullets are for paper and wild game, in season, if I feel up to going out and allowing more deer to outsmart me. I always get a nice, precise pattern on paper targets, deer, well, I found a calendar written in Native Venison with Opening Day circled and a note, Bahamas written on the day prior. OK, maybe not, just seemed that way, since rifle season is after every other form of hunting season and deer aren't stupid, only dumb. My 63 year old ass ain't going that high up the mountain and dragging a carcass that far overland with my back and shoulder the way they are, I'll just buy some venison from the butcher, when I can find one nearby... Opioids and opiates, loved them in patients that needed them, loathe them in myself, as my body and them just don't get along. Massive mast cell dump of cytokines make me literally feel like I'm dying, just shy of true allergy level response, so shove those things back into the medication storage unit for me, thank you. Lidocaine patches work nicely enough for modest issues, although I can taste the infernal stuff around an hour after application, injected in the angry region seems to be of mixed value. In others, any admixture of the above would be highly effective, as it's highly variable by individual. I'm also infamous for awakening early when under anesthesia, just to be more of a complicated patient, maybe it's the thyroid or my liver is from the planet Krypton or who knows, I just let doctor know so that things get monitored a bit more closely. The last one, astonished at my rapid recovery after colonoscopy with Versed and propofol, easily recognized, as they're not easily miscible, to put it mildly. I've also not had an inclination of running around like I'm Yosemite Sam, shooting up houses, trees and people. Although, I might be swayed to give into the temptation of hot gluing some chairs to the floor while the worker is not at their desk. After all, I have a gun and know not to grab the end the glue comes out of when it's on. The rest stay in the safe. Oh, likely I am a surgical candidate, might explore that once a few might kill me issues are resolved, but before I'd consider it, I'd have to remove one cause of failed back syndrome - I'm a smoker and that is linked to failed fusion and weak fusion healing. I've also got one superpower, an innate knowledge of when to abort a medical algorithm and jump straight to DOCTOR NOW. OK, two superpowers, the above and the ability to make food, drink and money disappear nearly instantly.

Yes! I have been having a hard time getting my morphine ER (extended release) filled the last couple of months because of a shortage. Weird, I thought, but I asked my doctor to rewrite the prescription for the morphine IR (immediate release) which really doesn’t work anywhere near as well for me. But I thought it would be temporary and I could tough it out. Reader, I can’t tough it out. Not only does the IR let more pain through, my doctor miscounted the days I needed the IR prescription for and I didn’t notice it was a week short of the correct number until I filled it. So I spent last week *_WITH NO PAIN MEDS_* !! I was writhing on the bed so much, I have carpet burns all over my legs from the sheets. We don’t have legalized marijuana in this state but my husband got me hemp gummies which didn’t really help but I took them so he wouldn’t worry quite so much. Getting to the point: It turns out that the company that makes the morphine ER has discontinued it (back in March 2024) and I guess we’ve used up the supply. And why would they do that? I mean, the company didn’t reach out to me personally and let me know their reasoning but I’m gonna go ahead and assume profits were down with that drug because the *CDC* recommends such a low dose of morphine as an at-home pain killer (and that the *DEA* audits the pain clinic doctors frequently), that they just weren’t getting prescribed as much anymore. I usually think some government oversight is good. I like having my water tested so I know it’s safe. I like knowing when a hurricane is coming my way. I like potholes filled in. But I don’t like having to search for a new drug for my pain that probably won’t work as well just because the *CDC* and *DEA* cannot let go of the myth of the opioid crisis.

I would end up having to spend a couple days in bed to recover from the pain of driving for a few hours like that. Oh and I did have a problem with the fact that he would make me come every two weeks because he had his own lab and I think he liked to charge for the urine tests. I never failed them or a pill count but I have a medical cannabis card so he used that to justify the extra frequent testing. They went through his his stomach in which he has about a 10 scar on his belly and his back. My mom and I when he came out his head was the size of a pumpkin and his face was blue it was very troubling for us doctors didn’t even warn us. 😳 I busted into tears. And I believe that was to only fix a couple blown out discs. He healed from that surgery to only blow out the discs to the thoracic part of his back. And get this he blew him out during physical therapy from his first surgery. Thankfully that surgery was only about 6 hours. Four years he was on pain management medication opiates. And you know he went in to buy his grandson a rifle and was denied because of being on the medication or at least that’s what the person selling him the gun said it was. My dad got off of the pain medications. He has zero life now you can barely walk but he will never take those medication‘s again. My dad is definitely pro second amendment is a Vietnam veteran and it’s not just the insurance companies it’s not just the doctors it’s our government people need to realize that. Making anything mandatory by our lawmakers is going to create tyrants. Yes tyrants in the medical field. It’s going to make insurance companies gouge their prices and limit the care. I remember a time where car insurance was a mandatory but as soon as they made it mandatory The prices of full coverage insurance quadrupled. And now it’s so far out of control they do credit background checks. They penalize people with no claims no tickets due to the area they live in. American system is broken due to our lawmakers. If this young man targeted that CEO he was targeting the wrong person. (In no way am I saying he should target anyone with violence) he should’ve addressed this with Congress he could’ve been a great voice to fix the holes, the tyrants in the insurance companies.

Thank you for making this video! As soon as I read about this case, I started trying to understand what led to this drastic change in this individual, given that everybody from his social circle was staying how kind and good Luigi was before his back problems worsened. I knew the shift in his cognitions and mentality must have had something to do with his spinal surgery and problems. From what I could extrapolate from Luigi's reddit posts, I assume he must have been dealing with nerve damage and even possible inflamation of the nerves, and all this can directly affect the brain, and cognition subsequently.

It's wonderful just to hear hear a physician acknowledge the physical and emotional ramifications, and the difficulty of adequate pain management. Thank you. Lordy, I wish the cortisone helped. Alas, it doesn't begin to touch the pain that keeps me from living my life. I'm 42. I may have 40 more years of this, and I don't know how to face it.

I understand what this young man is going through. He was young, handsome, intelligent. He had an accident and now in serious pain. Not only will the pain affect and change him, the pain medication changed him. Pain medication will affect him through and through. You could be social and athletic, soon you find yourself withdrawn, antisocial, not enjoying any of the things you used to enjoy. This poor young man was gravely suffering for a very long time. And I worked in insurance for many years. They are FOR PROFIT. They usually deny a claim as standard practice and then see if you know enough to appeal the denial. If not, they save money. Insurance companies don’t care about you,in my opinion.

You do realize that pain when untreated or undertreared not only affects the brain but changes it too? When pain is properly treated with pain meds people are not so depressed. In fact they can at least function & continue having some quality of life. You put no treatment & undertreatment of pain with the loss of quality of life you get depression, hopelessness , mood problems (anger) & a greatly reduced social life. You get isolated. Then you get 'addict' label & gaslighting by drs too afraid of the dea to prescribe more than an ibuprofen but willing to prescribe drugs for psyche or useless gabapentin which does not help most pain patients. Insurance just ran with the opportunity to deny everything they can for pain patients. I put the biggest blame on the cdc & Andrew Kolodney & his anti opiod zealots who are all getting rich from their sick evil agenda to get every pain patient on bupe or related meds. I can certainly understand why Luigi did what he did but he should have targeted Kolodney. As a pain patient, I know very well that you don't leave a person with nothing to lose. The ceo found that out the hard way. Opiod meds need to be given back to the pain patients. Opiods are the only med a dr will take away from a patient who's condition is stablized on.. They don't take away heart meds or insulin after the patient has been stablized on it.

@karenmessinger9609 Don't forget to blame the DEA as well. These are law enforcement people, NOT medical doctors making decisions to cut the supply of pain medications, which results in shortages. The DEA terrorizes doctors who appropriately prescribe controlled substances to adequately reduce pain in their patients so that they can function and be productive. When you mention Kolodny, don't forget to mention Lembke. Lembke who has stated that chronic pain patients should not bother going to the doctor and that they should go straight to a spiritual counselor. Younger doctors truly believe the bs they are being taught about chronic pain. They actually believe that opiates don't work for chronic pain. Older doctors know this is not true because they have seen how opiates provide relief and allow their patients to function and have a life. I wish all doctors understood that chronic pain patients are pain relief seeking and that prescribed opiates do not make us high. So wrong that younger doctors do not understand the huge difference between dependence and addiction.

Dr. thank you for all of your information! You are an advocate for patients and an educator. I'm a Certified Medical Assistant having worked in primarily family practice for over 30yrs. Two yrs ago I lost my job and ability to work or basically function due to severe lumbar spondylothesis and spinal stenosis. Finally had multi level fusion and lami 6 mos ago. It was horrible and I'm just now getting some strength back. I've worked with chronic pain patients my entire career but until I experienced it myself I had no idea. I'm not surprised by this young man's drastic actions, our system is broken. However it's so sad he clearly had no advocate or like you said was extremely isolated and in that dark place with his severe pain. So very sad. I also told him I was struggling with my mental health and he told me: " Depression is a choice" then he terminated me, got paid $251 from my insurance that day, for just terminating me. I'm in the same boat with you. Yep, death becomes the focus when there's no quality of life left and the ones who can help choose to turn physiological pain into something we dream up in our heads. People who knew us before pain attest to how it changes us, not for the better. So, in addition to uncontrollable pain, they add treatment-resistent depression and PTSD. I had neither prior to my MD abuse. I was a state agent and correctional sergeant. I was not a flaky boo-boo. Please see my new comment from a few minutes ago. Gaslighting, yes, Yes, YES! I have lived with chronic pain for decades. It is truly awful. The first few years l refused to believe that it was permanent then, when l finally admitted to myself that l would be in pain every day FOR THE REST OF MY LIFE, the only thing that kept me from taking the terminal option, was that my daughter was a toddler. They say you learn to live with it but, you don’t, your life shrinks to fit the pain. Most days you can only manage to do one or two things - laundry OR cooking. Shower OR visit a friend. Groceries OR a movie. Every phone call gets cut short because of pain. Impossible to go for a walk or queue in a shop or even stand at the door to say goodbye to friends who NEVER understand that standing there for more than 30 seconds is agony. Eventually you almost never leave the house, most friends don't bother anymore cause you're not the person you were and because you're pretty much housebound you're not much of a conversationalist any more anyway. Then there's the endless chain of side effects from the pain meds that barely make a difference because they're designed for acute pain NOT chronic pain. Then the depression kicks in and the weight gain from comfort eating and lack of exercise...

She was hunched over and was breathing hard. I asked her if she was okay and she said, "Oh I just had bladder surgery and all they gave me was Tylonal 2s. I don't know what I should get here." "That's terrible and insane. You might need Rick Simpson Oil." "How much is that? I only have $30." It's so unfair we have to pay out of pocket for the only form of pain medication that's available to us in the form of medical cannabis. I still feel rage bubbling up inside of me for the poor woman. HOW could they think Tylonal 2s was appropriate post surgery. It is so unfair and sick.

I’ve had fibromyalgia since I was a kid and I broke my back years ago, so chronic pain is my life, to the point where I’m on disability for all of it. I wake up in pain, and I go to bed in pain, even though I’m actually on a low dose of pain medication. If I wasn’t on disability, though? I could never afford any of my meds. My sister has different medical conditions, some of which cause her as much pain as I’m in but just not as continually, so she can work - but it means she can’t get even the same amount of help as I do. She literally can’t afford it. She can’t get the same tests that I do, because even though we’re poor, she’s not poor enough to qualify for any financial help. The system is ridiculous. It’s cruel. And that’s not even getting into the mental health conditions that come along with this much chronic pain, or the social stigmas, or anything else that compounds the suffering you’re already in. I don’t condone violence. I don’t condone murder. But I can still recognize what might have lead to Luigi’s choice.

I have had back pain since 2019 and other lifelong health conditions that have been severely heightened due to the back problems in which I continue to chronically suffer excruciating pain everyday without access to pain medication, thus forced to live solely without the ability to travel in public due to my disabilities. My life before this combined pain was normal with work 40+ hrs a week, fun times, feeling human. Then everything I known for most of my life, GONE. Being confined to living alone due to severe mental And physical pain beyond your control is life-changing. Much so it is enough to make me be crazy but others who only see me and don't know anything about me think I really am just Crazy, which is so unfair.

Luigi is a hero. He sacrificed his freedom in order to show the world the failure of the medical system. If Dr.s can't fix it, they don't care. If insurance companies don't see it profitable, they don't care. Being upset because you're in pain isn't a mood disorder, it's a normal reaction to an abnormal condition. Dr's: if you can't fix the problem, treat the symptoms regardless of addiction. Palative care is better than no care.

I work on a unit as a new grad nurse, and we have many patients who have undergone spinal surgery. Some even multiple surgeries in a matter of years. And they all have chronic pain. Some manage it with a pain clinic but few and far between. So when they come into our unit I give pain meds on a schedule to minimize break through pain. And then they go home and their prescriptions aren't enough to manage their pain later. Especially if the surgery fails at our hospital, I've seen them return. The thing is I also have suffered with chronic issues all my life. So I can empathize with their experience and that guides my care.

Can you agree that not ALL chronic pain patients on opioids get addicted? For some of us, they've saved our lives. @@Sundaylamb3 easy to say when you are not the one whom is at a point they will do ANYTHING to relieve the constant pain! My husband asked me yesterday if I am ever not in pain. I replied, I am always in pain. Waking up, trying to function all day, then interrupted sleep, etc… it is the degree of pain that varies. ALWAYS in pain! I am at a point where I am ready to give up. Hubby surprised me with a puppy on Halloween. It has helped keep my focus on something else besides my agonizing pain. I am crying while commenting because he said to me recently, I love you and hate seeing you like this. I said some days it’s not enough and that broke his heart. It breaks mine too because he has been my rock and has kept from walking off a ledge. Poor guy feels helpless and yet he keeps trying to make it better. He is also working 6 nights a week to support us while I wait for disability to be approved. Not everyone has this kind of love in their life and that makes me cry more. Reading the comments here and seeing how many people deal with this issue is heartbreaking 💔 I am not alone yet there are days where I feel like I am. I live with chronic lower back pain at the L5, S1, which is at the same spot as Luigi’s, from a failed back surgery. There are days when I have to wake up and take a cocktail of ibuprofen and acetaminophen. I went through years of paying for pain specialists myself, because my insurance denied coverage. Two years after I stopped seeing pain specialists, I was diagnosed with chronic depression and anxiety disorder, and put on medication for both. I was at the point of considering taking my life, because I couldn’t see my future as anything other than dealing with the pain and anxiety. I lost hope in my dreams and goals, which was not like me at all. I withdrew from everything, even the people who loved me. It was a very dark time, and something extremely difficult to overcome. I came back through therapy, patience, and the consistent love of my dog. Honestly, I understand the darkness that chronic pain causes. I don’t condone murder, but I understand the darkness and the thoughts that come with it.

@ I researched doctors with expertise in my particular injury which is a severed median nerve that left me with debilitating hand pain. Like lay in the bed and cry pain. My local doctor just prescribed heavier and heavier opioids which left me sedated and in pain taking away my coping skills. I found a surgeon at a major medical school who specializes in peripheral nerve injury and pain. And he hooked me up with his pain management team and the surgeon had perfected a surgical intervention that has brought me relief. I didn’t just accept that there was nothing that could be done. And I knew where to look to find some sort of help. My surgeon is at UTSouthwestern in Dallas. I’m sure there are surgeons at other major teaching hospitals that may be able to point people with pain in the right direction.

I researched the top doctors in the country who treated my condition rather than accepting that nothing more could be done and relying on ever increasing doses of opioids. I knew where to look and self referred because of my medical background. My surgeon is at UTSouthwestern in Dallas. Several hours from my home. I’m sure other teaching hospitals around the country have experts to help other types of pain. I’m not painfree. But the surgeon works closely with the pain management doctor and I’m so much more functional now. I’m getting my life back, without excessive opioid use.

I'm a massage therapist. Totally agree with this doctor about pain.. a person cannot function properly if they are in constant pain. I have that on the first page of my website in my video.. "we must first get you out of pain before we can think of anything else." Many of my clients want to make mental, emotional, and spiritual changes.. if they are in physical pain, I tell them we have to work on that first, because if you are in pain, the brain can't think of anything else. And yes, I do distance sessions.. Reiki can be done by phone or Zoom. And yes, Reiki works on physical pain. :)
