On work slots on the regime, make sure they're long enough to fit the programs you want to run. If a work slot isn't long enough, it won't let your schedule that program.

The entrance to the kitchen is too small, as long as there is congestion there will be fights. Try building your bigger canteen where they can access from most 3 sides.

Satisfied needs also really matter in reform programs. A prisoner's suppression and needs will play a role in their passing rate as it can either help or diminish the "Concentration" factor, so it's recommended to have programs/work regimes after freetime hours to make sure all those needs are met. Great video, though
