I recently ran across an article in Self magazine that I thought was great called “5 signs your Dry Eye Treatment isn’t working for you.” Because dry eye is what we do here on this channel, I thought it would be fun to go through the article and give you my take on each of their thoughts as an Optometrist & dry eye specialist.#dryeye #dryeyes #dryeyedoc #dryeyesyndrome #dryeyedrops #dryeyedisease #dryeyetreatment

So I love the idea behind this article because Dry Eye treatment can truly be so complex with so many options that it’s very easy to become overwhelmed, unsure which step to take, and it’s important to get a good sense of what has worked for you and what isn’t working so that you and your doctor can tailor your treatment to your eyes.
Ultimately, your dry eye treatment needs to work for you. It’s important that it makes a positive impact on your quality of life and day to day activities.
Make sure to check out the original article in Self magazine. I loved it and I think they take a great approach to walking you through what you should and should not expect with your Dry Eye treatment. And of course, if you feel that your Dry Eye treatment is not working well for you make sure to take notes and take those to your doctor so that a better treatment plan can be implemented.

I have eye dry syndrome and blepharitis…tried eyes drops including cequa they didn’t work now using autologous serum eye drops and my eyes are 💯 better! Almost like new!

I was on the verge of trying autologous serum drops when I learned about castor oil treatment. I had corneal erosions from severe dry eyes (from Lasik), my corneas healed with regular MGD treatment (expression of glands). I could not imagine having to do that for the rest of my life & there was no way I could try the expensive treatment options right now. When I learned about castor oil therapy from Dr.D. I use organic (hexane-free) castor oil, 1 drop in each eye every night before bed & massage in the excess. My eyes were the most miserable at night when sleeping; they would be stuck shut & occasionally I would wake up when my lids stuck to my cornea in REM sleep & would rip chunks out of my cornea. I was floored at how my eyes stayed moisturized sufficiently ALL night long, & I could easily open my eyes without pain. I also had better vision from that very first time!! I love that it's 100% natural & castor oil is full of antioxidants; it's also balancing to the lash biome. It's actually healed my eyes & improved my vision, no need for glasses (reading glasses only). I had reduced night vision & that is cleared up as well! Castor oil has been a miracle to me & I hope this info helps someone =)

I have tried many different treatments, drops and products and I haven’t found real relief. This disease is very expensive and very frustrating. It seems a symptome is always there.

Creatine might give you some relief. It's anti-inflammatory and increases energy levels. But you have to pay attention to screen time, sleep and diet as well.

You got that right. I’ve been fighting with DED, MGD, etc since July 2019. I have spent a small fortune with very minimal to no relief. Unfortunately the opthamologist I WAS seeing in 2019 never explained anything. Only gave me restasis for few months, truly did nothing and went forward with 2 cataract surgeries which could and should have never happened. Made my eyes unbearable. To ANYONE with DED , I would give my opinion and say NO to eye cataract surgeries UNTIL DED is fully controlled. Yes, I followed all instructions given. Still battling.

@@pattyschrider1681 how did you get DED? I had many risk factors and I didn’t know it. Any doctor never told me about it.

Thanks Dr, appreciate your videos and helping patients with their dry eye issues. 2022 released Systance complete preservative free emulsion seems to be the longest lasting drop ive found after literally trying them all. After a decade and ten's thousands of dollars spent, I found an overall routine that takes my unbearable dry eye to a 7/10 from a 1/10. Mind you I scored a 1ml on a shirmers test and my meioboim glands are half gone and the half were blocked. So I had no water layer or oil layer.Ill share my routine incase it helps someone in the comments. If you follow this routine you'll get an accumulated effect that gives you lasting relief. 1. Use polyvisc oinment before bed. Also use it in tiny amounts throughout the day. Dont put too much in the day or you'll obscure your vision. 2. Before bed do a heat compress that you microwave. I use "the eye doc" brand or bruener is good also. 10min of solid heat, you may have to microwave twice. 30 sec apply 5min then 10sec apply 5 min. Immediately after use the Nulids device which is an at home treatment that expresses your oil glands. Use some polyvisc on the tip to protect and lubricate before use. Nulids is quite pricey but well worth it if your an extreme case like I am. Pro tip- buy a tea infuser cage to put the used silicone tips in and boil them for 5 minutes to disinfect and resuse. Once the tips dry, store them in sterile container. Always wash hands before handling tips to avoid infection. The tips cost a fortune if you use them once and throw out like they recommend. (Dr may disagree with reusing tips, you can play it safe if you wish) After nulids do some manual expression too with clean hands. Using fingertips do small circles along lidline with gentle pressure. Then use make up remover pads with warm water to wipe your eyelashes OR if you have crusty eyelashes (blepharitis) you may benefit more by using ocusoft hypochlor acid spray on pads then wipe lashes. 3. Take krill oil supplements or better yet eat tuna or salmon 3x a week. Boost omega 3. 4. Use Systane complete preservative free during the day as needed. Its an emulsion which lasts long (mix of aqueous and oil). Best drop and Ive tried literally all other drops. NEVER USE DROPS WITH PRESERVATIVES. 5. Buy "Eye Eco onyix silicone hydrating sleep mask" moisture chamber goggles to wear while sleeping to keep your eyes moist while sleeping. Remember to put polyvisc in before putting goggles on. I've tried all sleep masks, this ones superior by far. 6. Avoid aircon or excessive heating which drys out the room. Outdoor high wind environments also. I recommend wearing 7eye sunglasses outdoors to protect from wind and create a moisture chamber. Game changer outdoors. 7. If cost isn't an issue, I recommend you do Lipiflow treatment every 6 months to properly unclog your oil glands. Then you maintain its effects by using the nulids device described above in point 2. In office treatment may cost 1-2k. 8. Position computer screens below your eye line so your looking down on them. Less eye exposed and less dry eye. Take breaks and do 20 complete blinks from time to time. 9.Avoid contact lense wear. I hope this helps someone out there suffering with severe dry eye. It's been a long painful journey for me 13 years or so but following the steps above I regained the ability to function.

thanks! yes! heat can definitely increase inflammation and redness temporarily. for some patients (ocular rosacea typically) i don't recommend hot compresses frequently because of this exact effect

I am now trying Xiidra, previously I tried Retassis. I am beginning to wonder if either of these work. I might just go back to using eye drops as I am now paying $50.00 a month for the Xiidra. It is very frustrating.

Have you ever tried creatine? CoQ10? Those supplements reduce inflammation and they boost your energy levels so your eyelids produce more secretion, hopefully. From my experience, CoQ10 works faster than creatine, but creatine, once you're closer to ending the "loading period", it gives better results. Well, I take both, but it's also important to cut down on screen time, sleep well and eat a clean diet, with no sugar or polyunsaturated vegetable oils.

Has anyone with DED used serum drops? If so, do they work for you? To HOPEFULLY control DED it does cost a LOT. EX: I’m on cequa-0$, oasis/or refresh anywhere 30-40 plus mth, hydroeye 40.00 mth, warm compress was 30.00, contour mask10-15$, lid scrubs 30+ mth, this doesn’t Inc if I get Tearcare (450-650.00). Serum drops are approximately 1300. Annually, and al extra X-rays, etc. I have paid a small fortune. I am going to try Tearcare very soon. If it works, it’s worth that. Wtd? Can’t afford all the stuff yet can’t go without it. My eyes are very deep orbits, since 2 cataract surgeries they have gone deeper. I’ve had ct scans, idk why the dr. Was an A HOLE, never saw him again and was not my fault. I will always offer one piece of advice based strictly on my experience and my opinion. Do your homework! Eye surgery with active, untreated DED is a BAD decision. Ask questions. Know before you go to opthamologist. Mine did not care. He NEVER explained ANYTHING t me, I wasn’t given an option, he just put in IOL’s. Later I was given rx for glasses. 2 yrs later, my glasses ( not cheap) burn my eyes, I am on phone minimum time then they are on fire, had to get larger TV, vision i worse now. Minimal computer time. It’s absolutely horrible. I regret surgeries. I’ve had punctal plugs twice , no difference. Eye ointment for me is not good. Takes me forever to get it out. I can’t use ceiling fan, the sleep masks are hot. I sweat. ITS MISERABLE!!! I’m going along way to see a good optometrist for Tearcare. I feel good about it and I believe she’s going to be my game changer. I’ll report back with positive results I’m sure. She’s terrific. I’m not allowing another opthamologist do surgery on my eyes again unless it’s life or death. I never learned anything about what I’m dx. With until I got m records from opthamologist and looked up each dx. And then discovered MANY DED specialist on KZbin and it all made sense. My absolute favorite and most knowledgeable is Dr D eye school. I truly believe she understands, and knows her stuff!! Thank God for her!!!
