Nier Automata Ending K: you eat a mackerel that the game CLEARLY tells you it will kill you because you’re an android and you shouldn’t be eating human food. But you do and it kills you. At least the death scene says it tasted good.

I like that even though getting trapped in the vault in New Vegas was silly game ending mistake it still got a proper voice over ending sequence, I’d like to see every silly death in that game get the same treatment. “And so the courier was killed by some scrub bug out in the wasteland, ..they forgot to pause when they we’re checking their phone.. and isn’t that just a tortured metaphor for the true nature of man..”

@@Geekguru14 I did. It is difficult to go north but not impossible. If you have the Ultimate Edition, you get a bunch of weapons to include a grenade launcher which proved immensely helpful in that area. I go up there to get the Trail Carbine. Really underrated rifle.

That... That shouldn't even work. The firing sequence of an atomic weapon is both precise and VERY delicate. Sympathetic detonations are not a thing with nukes. Shooting one doesn't set it off, it breaks the thing. Also why I hate the Legends of Tomorrow tv show. The grenades were RIGHT THERE!

"Cooking" a grenade for too long. A surprisingly large number of games. To clarify this is the act of pulling the pin on the grenade but not throwing it right away to reduse the time your opponents have after you throw it. Or in this case holding it until it explodes.

I'm replaying Planescape: Torment at the moment, and found one of these I hadn't seen on previous playthroughs. There's a moment where you're given the opportunity to become the new Silent King of the undead in the Dead Nations. The catch? If you sit in the throne, you're told you'll be bound there until the end of your mortal life. That would be bad enough for most RPG protagonists, but in Planescape: Torment it's even worse - your character is on a quest to regain their lost mortality, and as a result is literally unable to die. You sit on the throne and are bound to remain there for the rest of eternity with no hope of escape. Whoops.

In "Let It Die" there is an item called the Gambleshroom. When eaten, this shroom has a chance to either fully heal you, or turn you into a frog (temporarily). If you happen to use one of these and get the latter effect while a lizard is coincidentally nearby, it can eat you.

In Fallout 4 I insta-died from drinking the water inside a nuclear reactor that was in a meltdown state. I had on my hazmat suit and popped a bunch of Rad-X first when I saw I could drink the water in there. Thought i'd take a bunch of rads, but nope, just straight to dead.

How about the old Journeyman Project game? You're informed in the opening that you're late for work and that if you use the teleporter to go somewhere other then the time travel hub you'll be "terminated". If you do decide to go somewhere else in the world you get one more message telling you to get to work. If then you choose to ignore it and leave the machine you are indeed "terminated", via disintegration.

I have something, in Skyrim if you do the Dark Brotherhood questline you’re eventually given something called Jarrin Root a deadly poison that kills everyone who consumes it. If you chose not to feed it to the emperor (later found to be an imposter) you can keep it and now have the option to consume it yourself and of course if you do, you instantly die.

In Manhunt 2, there's a stealth section in which you pass by a security booth. Inside the booth is an alarm button that you can press to alert everyone and fail. Your character even says something along the lines of "Oh, why did I just do that?" It's been years since I've played the game, but I vividly remember failing this way and feeling incredibly stupid

Omg, I played Little Big Adventure 2 when I was a kid and forgot it existed for the better part of the last 2 decades, and then recently (maybe a month or so ago) had a sudden and vivid flashback of it. The fact I had forgotten it so completely, had never seen or heard of it again in that time, and then had a sudden unprovoked memory appear out of the blue left me basically questioning my sanity as to whether it was real at all. Good to know I'm not crazy... or at least, no more crazy than I was before that memory appeared.

I got that achievment intentionally in my first playthrough, but I still fully deserve it, based on a more recent one, where I was trying to find out if, if you put something on the helicopter windows it would be visible during the flight-sequence. This resulted in me climbing on the helicopter trying to drop things just right, with the result being, well....

The Rise of the Triad ending reminded me of the game over you can get in the original System Shock. Where you are meant to disable the mining laser by firing it into the space station's own shields. But you can fire the laser before activating the shields and vaporize some unsuspecting city on Earth. Leading to evil AI SHODAN congratulating you for doing her job for her, being invited to a party and then being kicked back to the main menu.

The first Myst game: Give Sirrus or Achenar the final page of their book and end up on a prison world while they get to practice their evil laughter. And don't even get me started on the ending where you go back to Atrus without the page he requested. Atrus isn't angry; he's just disappointed.

For another Fallout entry (from Fallout 3), you can activate the Garden of Eden Creation Kit within Vault 87 if you manage to make it past the stupidly high amounts of radiation. It tells you that the kit will absorb all matter within the area for conversion into raw materials, and then asks if you're sure you want to activate the kit. If you select yes, you quickly get informed that it does mean all matter.

Saints Row 4 Jane Austin goes on two monologues about how this is the game where you basically become King of the Universe and find Johnny Gat, and there a part in the game where Zinyak gives you the option to give yourself a gameover. You know better, you know you do, but you still wonder, "....secret cutscene?" Btw... No. There is no secret cutscene. Just a game over.

You can get a Game Over in Dishonored at the Hound Pits Pub. After catching Piero peeping on Callista, you can get a Game Over if you walk in on her bathing and refuse to leave.

Attacking the Enterprise in 'Star Trek: Invasion'. Sure, you get a game over - and a stern talking-to from Worf - if you fire on any allied or neutral ship too many times. But there's something special about earning your infamy by unleashing on Starfleet's flagship during an early bonus mission, while the voice of Captain Picard is delivering exposition no less. It's an extra 'oops' moment if you've got the one-hit kill cheat enabled, and your tiny Valkyrie fighter explodes the seven hundred metre Enterprise with a single shot.

There's an old (but awesome) game called Anvil of Dawn. In the third dungeon of the game, you have to travel through an underwater labyrinth. The water elemental that takes you there tells you to avoid The Lady of the Sea because she'll suck out your soul. If you walk up to the Lady and talk to her... She sucks out your soul. This is actually really cool because if you explore around and pay attention to the items you pick up, you can learn a spell from her and survive by using an item called a Souling Figurine. This encounter is even a tutorial for the player on something they need to do to get the game's best ending.

That Little Big Adventure 2 one was interesting and not what I expected when I saw the preview. If you've got enough HP then you can survive that particular attack (might need to have picked up the tunic upgrade), and pretty sure if you've got additional lives (considering you start with about 3) it won't be game over unless you've managed to lose them all. Still a funny scene regardless. The reason I was surprised is because there IS a silly way to get an absolute Game Over regardless of lives and energy. Much later on there's an optional route through a factory involving crates and conveyor belts. One piece of machinery secures a lid on some open crates, which you can then move along a conveyor and stand on to reach higher ground. If instead you stand inside the crate as it goes under this machine, poor Twinsen is sealed inside it and it's an instant game over.

Can't believe System Shock 1 didn't make it in the list. For those who don't know, the first major thing you have to do is to prevent the station's mining laser from firing at Earth. The way you do it is raising the shields and THEN firing the laser, thus destroying it. But you can fire it BEFORE raising the shields, destroying Earth in the process. SHODAN even thanks you for that.

Crypt of the Necrodancer has the Steam achievement "Mostly Harmless". You have to go out of your way to get it, as it requires getting killed by a green slime, which is literally the only passive enemy in the game.

Refusing Snatcher’s contract in A Hat In Time. He literally tells you multiple times that you’re gonna die and gives you plenty of opportunities to change your mind. I still did it anyways though because I thought it was funny.

Giving the Enclave the code for the Purifier in Fallout 3. You give them the code, they activate the Purifier and then Colonel Autumn proceeds to kill you. Also, the Paper Mario series has quite a few of these, such as refusing to put on a space helmet, joining Dimentio, becoming the Shadow Queen's servant and let's not forget reading a ghost's diary when he clearly tells you not to.

My personal favourite 3 occasions are forgetting to wake up in Hitman: Blood Money and getting fired (hehe) into a cremation oven, landing on Midas’s hand in Tomb Raider 1 & anniversary, becoming 24 karat Lara in the process. And then probably the most common one, Panicking in a soulsborne game and hurling yourself into the abyss.

I’m playing through Kingdom Come for the first time. In the prologue, I got caught stealing and was sent to jail. Then, a short cutscene, and a game over screen. I’m so bad at the game, I got game over-ed in the prologue.

That Fahrenheit / Indigo Prophecy one reminds me of Monkey Island 3. You mix drugs and alcohol in a bar, die, get taken to a catacomb and the credits roll. Guybrush then says I'm not dead yet, wakes up and you explore the tomb for items as part of the game.

That helicopter decapitation reminds me of Left 4 Dead 2's "The parish" campaign. There too is helicopter at the end of the campaign to pick you up. I have *many* fond memories of me jumping into those blades and killing myself at which should be the end of the game

In most Lucas Arts Adventure games its impossible to die. That is mostly true for The Secret of Monkey Island. Guybrush is tied to The Idol of Many Hand pushed off a pier. Luckily Guybrush can hold his breath for 10 minutes but if you wait 10 IRL minutes Guybrush drowns. All commands turn into fun options like "Float" "Bob" "Decompose" and "Order Hint Book"
