How about the role of foot function muscular structure and the healthy whole body reliance on strong toes . The untreated broken dislocated phalanges are a captialist boon market of so many market products. The manufactured deadly shoe market is undermining quality of life harder than any other greedy profit motivated activity. And the " healthcare" industry is raping the wallets in a hoale traddition of living off others . One planet where predictable life cycle exists. Feet first for making that 100+/- journey about living not chronic misery is a possitive contribution. The foot weakened by resting the muscles and bones in a non functional position using it more like a block with no strength has got to go.

I used to have a bad bunion on my left foot and the start of one on my right. I’ve been working on my foot health for about 2 months. The right start of a bunion is completely gone and the left has significantly improved, a couple more months and I bet it’s gone as well. Two of the most important things I did which he did not mention: 1) loosen plantar fascia by rolling foot on a tennis ball 2) foam rolling and stretching calf and hamstring

I know a woman who at 94 years young - yes - ninety four years young - wears 3 and 4 inch high heel shoes all day whether at home, grocery shopping, etc., and even goes up and down three fights of stairs for exercise with them several times per day. Her posture is errect, she wears pencil skirts and beautiful tops and looks amazing. She walks very elegantly in the high heels as well, and she has no bunions and never has had them. She said she began wearing high heels when she was a teenager because her first boyfriend was 6 ft. 4 in. and she's only 5 ft. 2 in., so this is when she started wearing high heels. She ended up marrying a guy who's 6 ft. 5 in. - an inch taller than the first boyfriend! She said she hasn't been able to walk bear foot at all for decades because her feet are so used to wearing high heels, so it's normal to her. And she's never fell or twisted her ankles yet.

What a fantastic video. I am almost 10 years post bunion surgery and have noticed the bunion creep back. Was never told to do any preventative measure such as the orthotics (which I had a pair at home anyway) so i have just this second put them in my shoes to have a walk around and my feet feel secure. I have quite flat feet, so this makes sense to me. I also have quite tight legs as I carry a lot of tension so have been working on stretching daily as another preventative. Great video, the best Ive seen for actual advice that is helpful and simple.

So what I've also heard helps with bunion are barefoot shoes... Big toe box, obviously... But also strengthening the muscles needed for getting rid of flat feet. What do you think about that? Do you think it could be an alternative solution to solving bunions? Or do you think that it's a contradiction to orthopedic insoles (or orthotics?) and stable, cushioned heel? I've heard people saying it helped with their toe pain and reversed their halux vulgus a little bit.

Thank you. This makes sense. Although as a patient, the conflicting solutions out there make it so hard to understand. My podiatrist says the same as you, good shoes amd am orthotic. I had tailors bunion surgery and it still hurts in shoes. Most videos on KZbin say the opposite. Wear minimalist, flat shoes and do toe stretches to fix the bunion. They also say hip flexibility, hamstrings and ankles stiffness could cause a bunion. They say walking barefoot helps and my doc said be barefoot as little as possible? I’ve tried minimalist shoes, altras (which are zero drop with padding and a big toe box), Brooks and new balance. How do I know what is right? Why are both viewpoints so extremely opposite? Are their truths to both?

I might have an interesting thought for you. Not sure why it is not mentioned more often. Our feet did not evolve to walk on hard floors and asphalt/concrete. So I think using barefoot shoes in cities is asking for joint issues and pain. Walking barefoot at home, same thing. If you are at the beach however, look how your foot behaves, walk as much as possible on sand and softer grounds, see how it functions then and you will see what it evolved for. So I think wide toebox, zero drop, but also shitton of padding to protect the joints.
@howardjones8629
@howardjones8629 5 ай бұрын
The problem with using orthotics alone, as I have found out over the last 24 yrs, is that the muscles and tendons that support the foot, weaken so that they rely on the support from the orthotics. A combination of improvements in strength and mobility is helping with my severe bunion; but there is absolutely zero chance of anything other than surgery going to remove the calcification, deformity and damaged tendons.

The big arch can flatten as well, it's all about the subtalar joint that flattens out! Flat feet are definitely a tricky subject for sure, I should have used the term pronation but most people think of it as the foot flattening out and it gets confusing!

They then tell me if I get this orthotic things will be fixed and I will no longer be in pain. After paying for this expensive orthotic nothing has changed and I'm still having the same pain or more. 😓😡 How do I ever find a foot doctor that really wants to help and not just want to sell me an orthotic ?

I started training to spread my toes, train my ankle / arch. I soon switched to barefoot shoes. I started in September 2020 and now in December 2021 my feet are almost completely recovered from my hallux valgus problems. I would say that my best tips are shoes with a wide toe box so the toes are not steered wrong, zero drop so you do not load the forefoot incorrectly and then training the ankle to keep the leg up inside. Too bad you can not share photos here. Good video but I do not agree with the choice of shoe, just as you do not believe that it helps to sleep with aids, I do not think that the sole / shoe should be an aid but rather that the shoe should disturb the foot as little as possible.

I have heard that you should only wear barefoot shoes after you have trained your ankles. Someone with flat, over pronating feet would find barefoot shoes uncomfortable. They should use orthotics for support and gradually decrease use as their ankles strengthen through training. Is this right? Can flat, overpronating feet be cured by training?

I'm wondering what exercises I could do to help, I don't think I have severe bunions because they don't pop out hugely, but they do bulge a bit and the pain isn't terrible; but I know the problem is likely due to my habit of essentially dragging my feet when I walk. Almost like walking on my toes, and I work on my feet all the time and perhaps had too tight work shoes for a year and a half. I have Dr. Scholl's career shoes now..... I have a slight arch in my foot but I think I do have flat feet. Likely the fact that I'm always standing up doesn't help, I'm not sure but I do have that bad habit of always standing. Some years ago a coworker told me to sit down and I said I was fine, and when I'm at home-hell, I'm standing up right now!

@DrTomBiernacki
@DrTomBiernacki 4 жыл бұрын
There are good sandals with built in orthoses and ones that can hold you toe, but think of a bunion as a foot, ankle, knee hip, muscle strength, flexibility and pressure problem and not just a big toe or sandal problem.
@svenvanderzwaag1012
@svenvanderzwaag1012 2 жыл бұрын
Years of orthotics only makes the problem worse. It works in the short term, but the feet muscles get weaker and weaker. You will have to wear them for the rest of your life and eventually make everything worse. Only when I went barefoot/minimalist shoes, with correct toes, working on foot arch, ankle alignment, glute medius strength, stetching tight hamstrings/internal hip rotators, rolling a golfball/tennisball underneath the feet did it go away. These stiff pointy shoes are the reason for all these problems in the first place.

I put a 1- 1.5 inch slice between the upper and sole of my shoe at my bunion creating space so my toes aren't getting squished and reducing pain. My court shoes are not compromised.

Thank you for this.. So you say the main reason for bunions could be that i'm flat footed? I never know that bc i thought my feet are normal.. So it maybe so mild that it's not noticeable or what?
@DrTomBiernacki
@DrTomBiernacki 4 жыл бұрын
Personally its not always flat feet, but overpronation which means the knee is turning in and the foot out when standing. Flat foot is a very complex and confusing subject on its own!

I don't have flat foot, but bunion, when I move my toe left and right it doesn't pain but when I lift it up after a certain range it pains...
@DrTomBiernacki
@DrTomBiernacki 3 жыл бұрын
Specifically it's not flat foot but over pronation that can cause a bunion to occur biomechanically. We do see this with high arched feet as well. It's difficult to lump everything into he flat foot category, especially when big toe joint deformity might be hallux rigidus as well in certain people. Regularly though we do see a bunion on the tighter ankle that leads to over pronation and contracture of the fhl and ehl tendons. But again trust the person evaluating you as this is not always clear cut.

So what footedness is hereditary through my dad's ancestors. Hey was that being said I was born flat footed and so was my brother I cannot speak for my brother but on both of my feet I do have bunions. So my question is when flat footedness is a hereditary genetic thing how does one fix their flat footedness in order to recorect the bunions? Because I've been thinking of having surgery to correct the bunions so is surgery a better option? Or me or is correcting it on my own a better option?

You're so Cute, but I don't need any tricks, but, rather, some 'new' feet!!! My mom used to work for a Podiatrist. She had/has some of the worst bunions, but after assisting with the procedures in office, she opted out of them, no second-guessing. Thank goodness for the breathable, knit tops on many of the clogs, and sneakers as of late! Total game-changer as far as the pain goes. My bunions used to Throb and Spasm when I would wear the unforgiving high heels and platform shoes. Ouch!!

I have the opposite of a flat foot. I have very high arches but also have Morton's toe. I have one bunion that is much more pronounced but neither foot gives me any pain. I walk a lot for exercise and want to keep my bunions from progressing. I don't think I have arthritis. Is it the Morton's toe along with the high arches that caused the bunions? I plan to see an orthologist this year.

Can a bunion and hammertoes cause the Achilles tendon to swell and develop bumpsittat are painful? I have RA and have a severe bunion, hammertoes and thickened, painful Achilles tendon. I had the other foot fixed with titanium implants in toes 2, 3 & 4 and bursa removed under my foot. I didn't have a bunion then that was very noticeable but over the past 3 years I am developing one. I was only told to buy shoes with a wide toe box and arch support. No info on having flat feet, but I know my arches have fallen. I used to have a very high arch. Nursing wrecked my feet.

Hey Doc! Happy New Year, I hope all is well. I am a curious 17 year old, who has pretty bad bunions which are moderate but have severe sharp pains frequently throughout the day and especially when i stand on it, it has only just gotten worse where it's sort of like unbearable, I also have very flat feet doc, with no arches in them my feet tend to flatten out each time I step i think it's what makes it apply more pressurem what would you recommend, i don't want to get surgery and then 20 years later down the line its messed up somehow, god knows how... but you know what i mean i don't know if it's even possible to redo the surgery procedure and even if so how many times can you do it sort of thing ... please help me out would be more than thankful.
