Speaking of SNL, there's the famous accident that happened on camera when John Belushi, doing his "Samurai Stockbroker", cut Buck Henry's forehead with his katana sword!

There was one on 24. In one scene in season 2, Tony Almeida (Carlos Bernard) is seen using a cane at work. It was due to a basketball injury, but in the show, it was explained that it happened when Jack Bauer (Keifer Sutherland) knocked him down in the hallway when Tony tried to prevent Jack from leaving.

Colin who played hook on once upon a time broke his leg on set and it was I think wrote into the show

I'm really shocked Walking Dead isn't on here. Norman had a really bad concussion for the final episodes and they wrote in that he got hurt because of the bruising on his face. Like it was really bad. He was in the hospital.
