Overview:
Acute pancreatitis is an inflammatory condition of the pancreas commonly caused by gallstones and alcohol use. It presents with sudden, severe epigastric pain, nausea, vomiting, and epigastric tenderness. Diagnosis is confirmed by elevated serum lipase or amylase levels and characteristic imaging findings on contrast-enhanced CT abdomen. Severity and prognosis can be assessed using clinical scoring systems such as Ranson criteria and APACHE II.
Clinical Manifestations:

Epigastric pain: Sudden, severe pain in the upper abdomen that may radiate to the back.
Nausea and vomiting: Common symptoms accompanying the abdominal pain.
Epigastric tenderness: Tenderness on palpation of the abdomen, typically in the epigastric region.
Diagnosis:
Laboratory tests:

Elevated serum lipase or amylase levels ≥ 3× the upper limit of normal (ULN).
Other laboratory abnormalities may include elevated liver function tests and inflammatory markers.
Imaging:

Contrast-enhanced CT abdomen: Characteristic findings of acute pancreatitis include pancreatic enlargement, peripancreatic fat stranding, and fluid collections.
Management:
----------------------
Supportive care:
Fluid resuscitation: Intravenous fluids to maintain adequate hydration and restore intravascular volume.
Analgesia: Pain management with appropriate medications, such as opioids.
Antiemetics: Medications to relieve nausea and vomiting.
Early enteral nutrition: Initiation of oral or enteral feeding as tolerated to support nutritional needs.
Identification and management of underlying cause:
Biliary pancreatitis: Cholecystectomy to remove gallstones or treat underlying biliary pathology.
Hypertriglyceridemia-induced pancreatitis: Long-term lipid-lowering therapy to prevent recurrence.
Complications:

Localized complications:
Necrosis (necrotizing pancreatitis): Areas of pancreatic tissue death, which can become infected.
Pancreatic pseudocysts: Fluid-filled collections that develop in or around the pancreas.
Walled-off necrosis: Encapsulated areas of necrotic pancreatic tissue.
Systemic complications:
Sepsis: Infection spreading from the pancreas to other parts of the body.
Acute respiratory distress syndrome (ARDS): Severe lung injury causing respiratory failure.
Organ failure: Dysfunction or failure of multiple organ systems.
Shock: Life-threatening condition characterized by insufficient blood flow to organs.
Complications of acute pancreatitis are associated with significant morbidity and mortality. Prompt diagnosis, appropriate management, and close monitoring are essential to minimize complications and improve outcomes.

When I went in to the hospital, I was told I had textbook apendicitis, and a med student was even brought over to poke my abdomen to see how perfect of a case it was - then, when the doctor found out the imaging came back fine for my appendix but my pancreas was inflamed, he said it was likely caused by excessive alcohol intake and that they wouldn't give me any pain medication because it was possible I was just looking for a fix. Horrible experience and a traumatic time.

First time I felt this pain was at work. I felt stab in my stomach I went to the bathroom I thought maybe I had gas, cause that’s what it felt like. It hurt to push. i had acute pancreatitis. my pain started before Christmas and i was finally diagnosed correctly Jan 4th. My husband said i was just constipated, or that i had gas pain. dr said ulcer. ER said i was fine and sent me home. When I was finally diagnosed i was in 12/10 pain screaming. no one would listen. fentanyl was ineffective. I required 3 days hospitalized. they gave me fluids i could not eat, and i needed dilaudid every hour

My brother is in hospital with this now, he had the pain on and off for more than 6 months but was not seen by a doctor due to covid and that the pain went away, finally a doctor saw him and sent him to hospital with suspected infected gale bladder, they did scans and said 75% of his pancreas had died and they think it was to do with gale stones moving onto the pancreas, he is only 32 and has been there for 4 days so far but we still dont know if they will remove the dead flesh or not.
