Why s.cr and BUN will be high in intrinsic renal Aki although they are being excreted????

What is bun?

In intrinsic renal failure, why is there an increase in Urea/Creatinine. If there is damage to cells preventing Na absorption, so Urea can not be reabsorbed, its serum levels should fall (right?). Unless the water loss is greater than the urea/Cr loss, creating relatively high levels with background hypovolaemia. Is that right?
@linfeiyu43948 жыл бұрын
the dead cells accumulate in the tube, and increase renal tubule pressure, hence decrease the GFR, leading to decreased waste filtration, BUN would be high in serum. However for the waste which already being filtrated, they can not be reabsorbed, due to damaged lining cells decrease the reabsorption ability.

I cant understand In intrarenal, urea and creatinine cant reabsorb because it has problem itself. It means I think urine have a lot of Cr,urea Then BUN and Cr is decreased in serum So i dont understand why that two of things are increased than normal
@Anonymously10104 жыл бұрын
Bun and cr are increased because even in intra renal due to tubular and glomerular damage there is vasoconstriction of afferent glomerular capillaries by the release of unknown mediators. Thus causes decrease in GFR and thus increased levels of BUN and Cr in serum . Also from sloughed off cells and debri of tubules in ATN , there's intratubular obstruction, urine backflow and dec net ultra filteration . This inc BUN and cr in serum . Also injured kidney fails to generate and maintain a high medullary solute gradient, because the accumulation of solute in the medulla depends on normal distal nephron function. Hope you get it . Fighting 💪
